Book excerpt: Methamphetamine (METH) abuse has become a major public health problem world-wide, as demonstrated by increases in the number of emergency room visits, substance abuse treatment episodes, and arrests attributable to METH manufacture and abuse. However, there are currently no pharmacological treatments for the wide range of symptoms associated with METH abuse. One of the reasons for this problem is that our knowledge of the cellular and molecular mechanisms underlying the development of METH-induced psychosis and dependence is limited. This book presents a review of the recent findings on METH abuse in humans. First by describing the background and physiological effects of METH in humans. Next, by discussing the clinical findings on METH abusers derived using brain imaging techniques (single photon emission computed tomography (SPECT), positron emission tomography (PET), magnetic resonance spectroscopy (MRS), and functional magnetic resonance imaging (fMRI)). Finally, by reviewing the potential therapeutic approaches for the treatment of METH abusers.

In the United States alone about 50 million people regularly smoke tobacco and another 5 million are addicted to other drugs. In a given year, millions of these individuals attemptâ€"with or without medical assistanceâ€"to quit using drugs, though relapse remains the norm. Furthermore, each year several million teenagers start smoking and nearly as many take illicit drugs for the first time. Research is advancing on promising new means of treating drug addiction using immunotherapies and sustained-release (depot) medications. The aim of this research is to develop medications that can block or significantly attenuate the psychoactive effects of such drugs as cocaine, nicotine, heroin, phencyclidine, and methamphetamine for weeks or months at a time. This represents a fundamentally new therapeutic approach that shows promise for treating drug addiction problems that were difficult to treat in the past. Despite their potential benefits, however, several characteristics of these new methods pose distinct behavioral, ethical, legal, and social challenges that require careful scrutiny. An overview of how the epidemiology of stimulant abuse is changing and pharmaceutical abuse is rising due to factors such as greater availability through family and friends who are increasingly being prescribed stimulants for conditions like weight loss or attention deficit disorder. The insight that even after long abstinence, stimulant users may remain vulnerable to amphetamine-induced psychosis, with delusions, paranoia and compulsive behavior. The insight that a comprehensive assessment of the patient involves the management of aberrant behaviors such as intoxication, violence, suicide, impaired cognitive function, and uncontrolled affective displays. A focus on treatment, emphasizing that the most important component of stimulant treatment involves behavioral therapies, often in combination with adjunctive medications. A review of the criminal justice system's shift away from punitive action and towards more human treatment, including the far-reaching benefits of medical management and treatment. Fortunately, our understanding of stimulant abuse and dependence is growing at a time when a steady stream of new users and casualties is still accumulating. Constant vigilance regarding changes in epidemiology, fluctuations in drug availability, and changes in drug trafficking patterns are essential to recognition of new drug abuse patterns and their identification and treatment. Cocaine and Methamphetamine Dependence should be on the bookshelf of residents, physicians and psychiatrists who are highly likely to come into contact with one of the millions using and abusing stimulants today.

Book excerpt: This study analyzed the pharmacological effects, situational contexts and processual dynamics of methamphetamine use, distribution, and violence, using interviews. Evidence supports previous research that suggests continuity from youth aggression to adult violence. Findings indicate that long-term influences - family, psychological/personality, and peer factors lead to the development of fairly stable, slowly changing differences between individuals in their potential for violence. Superimposed on these long-term between-individual differences are short-term within-individual variations in violence potential. For many of the sample members that engaged in violence, chronic methamphetamine use had a disorganizing effect on their cognitive functions, which in turn lead to distorted interpretations of behavior and reduced an individual's ability to use various coping devices in situations seen as threatening. The study could find no evidence of a single, uniform career path that all chronic methamphetamine users follow. Most germane to this study, it discovered that violence is not an inevitable outcome of even chronic amphetamine use.
