Book excerpt: Prenatal Alcohol Exposure: A Clinician's Guide provides comprehensive, clinically relevant information for mental health practitioners and professionals in educational, vocational, legal, child welfare, and correctional settings who may encounter individuals with a "hidden disability" that compromises their success in career, family, and life. The author is a noted scholar and clinician in the field of fetal alcohol spectrum disorder (FASD), and the book fills a glaring gap in the training literature on prenatal alcohol exposure (PAE). Lacking a diagnosis and the professional assistance they so desperately need, people with FASD can experience great frustration and suffering. Many are institutionalized unnecessarily or have entered the criminal justice system. Drawing upon evidence-based research and employing the most current assessment techniques and treatment strategies, the book aims to help the reader understand that pathophysiology is key to interpreting outcomes and that the effects of PAE vary according to the differential impacts of other biopsychosocial factors. The book emphasizes the need for individual clinicians to develop expert knowledge of PAE, rather than create new alternative services or systems to cater to these patients. Beautifully written and clinically rich, the guide offers in-depth coverage of essential topics: * Through the use of clinical vignettes, the book helps clinicians work through differential diagnoses, recognize the "red flags" that individuals with PAE may exhibit, and implement evidence- and practice-based modifications to care that help patients improve and even thrive.* Over the five decades that the complications of PAE have been recognized in the scientific literature, the terms used to describe the entities have changed. The book helps readers understand this nosological evolution and recognize the bridging of FASD in the interface of PAE and its mental disorder sequelae.* The section on assessment and diagnosis covers the latest techniques, including neuroimaging and psychological testing, both of which are promising although not yet reliable, and laboratory testing for general screening, diagnostic confirmation, monitoring, and risk management.* Chapters are included on pharmacological intervention, which reviews both medications currently in use and those requiring further study and psychological treatment, which focuses on interventions to replace deficits with techniques that correct affect regulation, executive dysfunction, and impulsivity.* Strategies and programs that enhance quality of life, ensure interdependence, and are socially responsible and inclusive are explored in a chapter on the "critical success factors." Proper diagnosis of FASD is fraught with difficulty, and clinicians need specialized knowledge to navigate the red herrings and red flags. Prenatal Alcohol Exposure: A Clinician's Guide equips professionals with the requisite skills and clinical acumen to identify individuals with PAE and provide them with optimum care.

Available in PDF, EPUB and Kindle. Book excerpt: My initial purpose in writing this book was to offer readers an update of my book Fetal Alcohol Syndrome and Fetal Alcohol Effects (Plenum, 1984), which con tained a broad overview of the history (actually the absence of any history) ofthe awareness of alcohol's teratogenic potential; a review of alcohol's pharmacology, especially with respect to pregnancy; a survey of the physical and behavioral effects of prenatal alcohol exposure; and an overview of the mechanisms sus pected of being responsible for those effects. I have omitted most of the previously examined historical and pharmacologi cal information because not much of what was previously discussed needed revision. On the other hand, because much more has been learned about the consequences of prenatal alcohol exposure and its potential mechanisms of action, I have considerably expanded my discussion of these topics. In doing so, I have attempted to include as much new material as possible without (I hope) being overly pedantic and thereby losing the proverbial forest for the trees. No book is ever entirely neutral in the topics it discusses, the issues it raises, or in its conclusions. In sifting through hundreds and oftentimes thousands of articles, writers have to choose which facts to emphasize and which to ignore. Every idea cannot be chronicled and every article cannot (and should not) be cited. In going about the business of picking and choosing, however, a writer has an obligation to present the arguments for and against a particular conclusion.

Book excerpt: This book describes the characteristics of youngsters affected by prenatal drug/alcohol exposure and explores strategies to circumvent this damage in order to maximize the individual's remaining strengths. Information and suggestions are primarily for the professionals in education who can provide supportive coordination for caregivers, mental health, and medical service providers-in terms of relaying information and pinpointing techniques for learning that are the most successful for each youngster. Medical literature on the physical, cognitive, and behavioral characteristics of this population is described for readers without a medical background. Terminology that is commonly used by various disciplines, outside of education, is also included. Educational needs, successful learning environments, and instructional techniques are addressed, as well as how the environment must be adapted to in order to optimize the experiences of these youngsters. Divided into three sections, Part One presents the characteristics of youngsters prenatally drug exposed, giving the reader an understanding of possible damage. Part two presents background on the cognitive processes involved in learning. The primary focus of this section is on normal learning processes. Part three describes instructional strategies for learning and everyday life experiences those youngsters with disabilities find challenging. The book will help educators and parents to recognize deficits so that strategies can be implemented. Instructional and management recommendations are made with this in mind. It will be of interest to educators, social workers, nurses, other service and care providers, foster care workers, and parents.--Back cover.

Available in PDF, EPUB and Kindle. Book excerpt: It sounds simple: Women who drink while pregnant may give birth to children with defects, so women should not drink during pregnancy. Yet in the 20 years since it was first described in the medical literature, fetal alcohol syndrome (FAS) has proved to be a stubborn problem, with consequences as serious as those of the more widely publicized "crack babies." This volume discusses FAS and other possibly alcohol-related effects from two broad perspectives: diagnosis and surveillance, and prevention and treatment. In addition, it includes several real-life vignettes of FAS children. The committee examines fundamental concepts for setting diagnostic criteria in general, reviews and updates the diagnostic criteria for FAS and related conditions, and explores current research findings and problems associated with FAS epidemiology and surveillance. In addition, the book describes an integrated multidisciplinary approach to research on the prevention and treatment of FAS. The committee: Discusses levels of preventive intervention. Reviews available data about women and alcohol abuse and treatment among pregnant women. Explores the psychological and behavioral consequences of FAS at different ages. Examines the current state of knowledge about medical and therapeutic interventions, education efforts, and family support programs. This volume will be of special interest to physicians, nurses, mental health practitioners, school and public health officials, policymakers, researchers, educators, and anyone else involved in serving families and children, especially in high risk populations.

Book excerpt: This book presents clinical assessment and management solutions for those people who are exposed to Alcohol in Pregnancy. Over the last few decades we have begun to understand the enduring effects of prenatal alcohol exposure on the developing fetus. The consequence of prenatal alcohol exposure - Fetal Alcohol Spectrum Disorders is a lifelong disorder and affects children and adults. It is a condition which is significantly under-recognised for many reasons. Assessment and diagnosis requires the input of multiple different professionals, and referral pathways are often poorly developed or non-existent. Information to support and guide these professionals in practical ways, what to do and how to help, remains limited. This book seeks to fill some of that gap by offering professionals, clear and useable research-based information and guidance that will help in their practice whilst also being a useful resource for anyone new to this increasingly recognised area of work. The book is divided into four broad areas bringing together chapters authored by experts in their field including those with lived experiences. Part one focuses on presenting an overview of the condition, and approaching women about their alcohol use and risk followed by part two focusing more around diagnostic issues. Part three follows with management advice, and part four revolves around policy and health prevention in general. Each chapter is designed to offer insight but also practical tips and support in an accessible manner. The book offers an essential guide for a broad range of health and social care professionals working with this condition.

Book excerpt: Understanding Fetal Alcohol Spectrum Disorder (FASD) is a useful introduction to the most common non-genetic learning disability, which is caused by alcohol consumption during pregnancy. Written by two FASD experts, it describes how alcohol can harm the foetus and disrupt development, and explains how FASD affects individuals at different stages of their lives. With the aid of simple, illustrative diagrams, photographs and charts, it shows how you can identify FASD and gives guidance on how mothers at risk can be helped. It also provides advice for parents or carers on how children, young people and adults with FASD can be best supported. Accessible and informative, this is the essential guide to FASD for social workers, family placement teams, child protection workers, foster carers, adoptive parents, midwives and teachers.

Book excerpt: I have written this book because I felt there was a need to bring together in one place the vast amount of research that has been published in the past 10 years concerning alcohol's effects on the conceptus. My hope is that this book will be of value to the many clinicians, basic research scientists, social workers, and others who are interested in this important issue. The number of such publications continues to grow each year, and it was a very difficult task to review even a proportion of them completely. While I have cited many of these research studies, only those that in my very subjective opinion warranted extended coverage were discussed. In this regard, much of my . ask was facilitated by the many excellent reviews that have already been written. To those whose important contributions have not been discussed or cited, I offer my sincere apologies. Omission of these publications in most cases reflects my own personal interests or awareness, rather than merit.

Book excerpt: Recognition of the relationship between alcohol abuse and adverse prenatal outcomes is reflected in the warning labels on every alcoholic beverage sold in the United States. Because alcohol abuse has serious consequences for both individuals and society as a whole, much research has been devoted to this problem. Fetal Alcohol Syndrome provides straightforward facts regarding the impact of alcohol consumption as it affects the development of the embryo and fetus. Surveying current research of fetal alcohol syndrome and its related problems, the book addresses the immediate effects on development at various stages. Long-term action of prenatal alcohol exposure later in life is also considered. A chapter devoted to assessing the behavior of children who were prenatally exposed to alcohol emphasizes the necessity of longitudinal studies of fetal alcohol syndrome. This important reference offers a thorough overview of a problem that cannot be ignored.
