Book excerpt: This book, designed for ophthalmologists and particularly for refractive surgeons, aims to develop practical and clinical skills in readers so that they are able to make the right decisions in managing keratoconus. There are four main chapters. The first is devoted to diagnosis and describes the clinical signs, the appearances on microscopy, and the significance of corneal hysteresis and corneal topography. Classifications and patterns of keratoconus are then presented, and the author offers his own new classification of topographic patterns. The third chapter addresses the management of the disease in detail. Both non-interventional and interventional approaches are discussed with due attention to contraindications and complications. In addition, a novel classification of management parameters is suggested, and a systematic approach to management, formulated. In the final chapter this approach is applied to nine cases that serve as clinical examples.

Book excerpt: Over the recent years, keratoconus and keratoectasis have become increasingly important diseases in ophthalmic practice due to the increasing number of keratorefractive surgery cases and the availability of new treatment modalities other than corneal transplantation. For both the genetic form of the disease (keratoconus) and the acquired form (keratoectasia), new modalities of diagnosis and treatments have become available, enabling physicians to treat these two ecstatic corneal diseases earlier and effectively, delaying or even avoiding major surgeries such as corneal transplantation. This book is a concise, well-illustrated and clinically indispensable guide for treating these two important corneal diseases.

Book excerpt: As a degenerative disorder of the eye, keratoconus can cause substantial distortion of vision, with multiple images, streaking, and sensitivity to light all reported by patients. Keratoconus: Recent Advances in Diagnosis and Treatment updates ophthalmologists about the innovations that have occurred within the last decade, discussing the diagnostic imaging techniques that have been developed for keratoconus diagnosis, understanding of how examination techniques are related to the evolution of keratoconus, and how to indicate the different therapeutic tools that have been created for keratoconus over the last several years. Additionally, fundamentals for new diagnostic elements, based on the mathematical, physical and biomechanical data are analyzed in depth for a better understanding of the essential diagnostic steps for the clinician to guide patients towards the most adequate therapeutic tool in the case. Modern keratoplasty techniques, assisted by femtosecond lasers or other devices, are also covered and these techniques, along with the emerging conservative treatments, have added to more precise control of the evolution of the disease.

The cornea is the clear tissue covering the front of the eye. In keratoconus, the shape of the cornea slowly changes from round to conical (PubMed Health). In the early stages, keratoconus can be corrected with the use of lenses, but in the later stages, surgery is the only option. Holcomb C3-R® is a non-surgical technique that involves placing drops of the vitamin Riboflavin into the eye and using a UV light to activate them. The drops strengthen the fibres of the cornea and over time, preserve sight (American Keratoconus Association). This book is a concise guide to the technique of Holcomb C3-R® to treat keratoconus. Authored by the inventor of the procedure, Brian S Boxer Wachler, the text is based on his first hand clinical experience and explains the history and basic science of cross linking, the development and naming of the technique, its uses and complications, and first patient results. Book excerpt: Since the advent of keratorefractive surgery and its rising popularity, irregular astigmatism has become an increasingly important issue for ophthalmologists and optometrists. Despite the success of LASIK and PRK, there are more and more patients experiencing visual quality problems due to the non-physiological modification of the corneal structure. Among these complications, irregular astigmatism is perhaps the most difficult to treat. Every physician who performs keratorefractive surgery understands that in reducing a patient.s refractive error, there is a possibility of creating irreversible effects on visual quality. This text is intended to help prevent and reduce the iatrogenic creation of irregular astigmatism, as well as provide effective treatment when postoperative complications do arise. With the maturation of keratorefractive technology in recent years, effective treatment for iatrogenic irregular astigmatism is quickly developing. It provides everything refractive surgeons, ophthalmologists, and optometrists need to know about this important topic.

Book excerpt: This is the much needed guide for ophthalmologists and patients who want to treat and understand about keratoconus a non-inflammatory, degenerative disease that compromises the structural integrity of the collagen matrix within the corneal stroma. The hallmark characteristic is the development of a localized, cone-shaped estasia, bulge or hernia that is accompanied by thinning of the stroma in the area of the cone. This leads to increased irregular astigmatism which cannot be easily corrected and causes retinal image blur and poor visual acuity. This book brings a new understanding to treat this disorder. Previously hard contacts and cornea transplant used to be the only option to treat keratoconus but now a variety of options are available to both stabilize the disease and improve the eyesight. This is the most comprehensive publication on the latest advances in keratoconu treatments. It covers to define keratoconus and its causes and diagnose with clinical signs, corneal topography devices, and keratoconus after refractive surgery treatment such as C3-R, Intacs, Ferrara Rings, CK and penetrating keratoplasty, contact lenses, improving in corrected vision with vision ICL, Venisyse and PRK. This book provides a comprehensive list of scientific references from pre-reviewed journals. For opthalmologists and optometrists, these references are important for augmenting their fund of knowledge and helpful in understanding the background for keratoconus treatments.

Book excerpt: In the early stages of keratoconus, visual acuity may be normal and slit-lamp examination is not able to identify the disease phenotype. Early diagnosis of keratoconus is of major importance for effective treatment. That is why corneal topography and tomography has become the gold standard examination for diagnosis of corneal ectasia. Corneal topography is a non-contact imaging tool that gives information about the anterior surface of the cornea. Corneal tomography evaluates both anterior and posterior corneal surfaces and is considered mandatory for preoperatory evaluation before refractive surgery. Until recently, Kmax was the main indicator for the progression of keratoconus, but recent studies showed that posterior elevation represents the first signal of an ectatic corneal disease. Cross-linking is considered the gold standard in the treatment of keratoconus and newer techniques allow this procedure to be selective on the areas of corneal thinning.
