We believe the recent technological revolutions in anatomy and physiology make the present moment especially propitious for this effort. In planning this book we had the choices of producing an edited volume with invited chapter authors or of writing the book ourselves. Edited volumes offer the opportunity for expression of expertise in each chapter but, we felt, would not allow the development of our ideas on the potential and actual unity of the field and would not allow the expression of coherence that can be obtained only with one or two voices, but which may be quite difficult with a chorus assembled and performing together for the first time. (Unlike musical works, there is very little precedent for rehearsals and repeated performances for authors of edited volumes or even for the existence of conductors able to induce a single rhythm and vision of the composition. ) We thus decided on a monograph. The primary goal was to communicate the current realities and the future possibilities of unifying basic studies on anatomy and cellular physiology with investigations of the behavioral and physi ological events of waking and sleep. In keeping with this goal we cross-reference the basic cellular physiology in the latter chapters, and, in the last chapter, we take up possible links to relevant clinical phenomenology.

My thesis work had been listed in a nationally available posting without any address. The Brain Inf- mation Service, thinking they knew where I was, listed “the other” Jerome Siegel’s Delaware address for reprint requests. I soon received a letter from Jerry along with the requests he had received and we have remained in c- tact ever since. I am occasionally reminded of my namesake when I meet a new colleague who is impressed that someone “so young” published a paper in Science in 1965 (one year out of high school, if it had been me). I entered the field in the early 1970s just as he left. My interests in REM sleep and brainstem mechanisms have been eerily similar to his (and he also did po- doctoral work at UCLA), so our research contributions can be distinguished easily only by my use of my middle initial (which has occasionally been om- ted from my publications). So, my namesake and I both have an interest in seeing to it that no one “brings shame to the name. ” The current work certainly fulfills that dictum. This is a very unusual book, both in its scope and in its approach to the - terial.

It is not surprising to see such effects of sleep on the body as it leads to significant changes in the electrical activity of the brain in general. Electrical signal changes in the brain (sleep-wakefulness rhythm) are regulated by neurohormonal molecules and their receptors in the body. Neurotransmitters that control sleep and wakefulness can be listed as ,ÄúGlutamate, Acetylcholine, Histamine, Norepinephrine and GABA,Äù. Main hormones are: Melatonin, Corticotropin Releasing Hormone (CRH), cortisol, prolactin, Growth Hormone (GH), Insulin like Growth Factor (IGF-1, Somatomedin-C), Follicle Stimulating Hormone (FSH) and Luteinizing Hormone (LH), progesterone, estrogen, testosterone, catecholamines, leptin and neuropeptide Y,Ä≥. The effects of pharmacological agents on sleep and wakefulness cycles are materialized through the following molecules and their receptors: Hypnotics (GABA A agonists, benzodiazepines, gabapentin, tiagabine), sedative antidepressants (tricyclic antidepressants, trazadone, mitrazapine), antihistamines, medications used for the treatment of sleeplessness (melatonin and melatonin analogues), amphetamine (most commonly used stimulant), secretion of monoamines (dopamine), non-amphetamine stimulants used in the treatment of hypersomnia and narcolepsy (modafinil, bupropion, selegiline, caffeine) and other substances (alcohol, nicotine, anesthetics). To the extent we can conceptualize the physiological mechanisms of these basic molecules listed above and the regions they affect, we can appreciate the effects of these substances on sleep physiology and sleep disorders.
