This condition affects the quality of life and bears a substantial economic burden on society. Current drug treatments are often inadequate to treat this type of chronic pain, as they provide limited relief and are often accompanied with many undesired and difficult to manage side effects. It is imperative thus, to find new treatments that target alternative mechanisms from the currently approved drugs.The pathogenesis of neuropathic pain is very complex and involves structural, physiological and pharmacological changes throughout the neural axis (from the site of peripheral nerve injury to the spinal cord/brain). While a neuron-centric view has dominated the literature for decades and the way we approach neuropathic pain treatment, recent work has uncovered extensive neuroimmune interactions as substrates of this condition. Interactions between the immune and nervous systems occur at multiple levels, where different types of immune/glial cells and immune-derived substances are implicated in various stages of the pathogenesis. The four experimental chapters contained in this dissertation are in line with this recent recognition of the importance of neuroimmune mechanisms in neuropathic pain. In the work presented here, we sought to unravel novel neuroinflammatory mechanisms driving the aberrant pain condition in both the spinal cord and the peripheral nerve as well as to investigate the role of a potent anti-inflammatory cytokine, TGF-[beta]1, as a potential modulator of these mechanisms. Several important mechanisms have been observed: A) Modulation of the central and peripheral inflammatory reaction with TGF-[beta]1 significantly delays and attenuates mechanical allodynia and thermal hyperalgesia via immunosuppressive actions on glial cells (Chapter 2) and macrophages (Chapter 3). In the spinal cord TGF-[beta]1 inhibits microglial proliferation, astrocytic activation and release of pro-inflammatory cytokines and exerts neuroprotective effects resulting in a decrease in the production of MCP-1. At the site of the injury, exposure to TGF-[beta]1 reduces the number of macrophages, which release pro-inflammatory mediators, and modulates different populations of T-cells. B) In chapter 4 we demonstrate that sciatic nerve injury-induced neuropathic pain is associated with an increased permeability of the blood spinal cord barrier (BSCB). We observed that the spinal inflammatory reaction triggered by nerve injury is a key player in modulating this BSCB impairment. We identified MCP-1 as an endogenous trigger of BSCB leakage, which could be reversed using anti-inflammatory molecules TGF-[beta]1 and IL-10. C) Selective depletion of microglia with MAC-1-saporin results in impairment of mechanical and thermal hypersensitivities at both acute and chronic stages following nerve injury suggesting an important and persistent contribution of microglia in the pathogenesis of neuropathic pain. These results expand our understanding of the neuro-inflammatory mechanisms of neuropathic pain as well as propose that immunosuppression or blockade of the reciprocal signalling pathways between neuronal and non-neuronal cells offer new opportunities for disease modification and more successful management of pain. " --

Book excerpt: It is now well appreciated that the immune system, in addition to its traditional role in defending the organism against pathogens, communicate in a well-organized fashion with the brain to maintain homeostasis and regulate a set of neural functions. Perturbation in this brain-immune interactions due to inflammatory responses may lead to psychiatric and neurological disorders. Microglia are one of the essential cells involved in the brain-immune interactions. Microglial cells are now not simply regarded as resident tissue macrophages in the brain. These cells are derived from myeloid progenitor cells in the yolk sac in early gestation, travel to the brain parenchyma and interact actively with neurons during the critical period of neurogenesis. Microglia provide a trophic support to developing neurons and take part in the neural wiring through the activity-dependent synapse elimination via direct neuron-microglia interactions. Altered microglial functions including changes in the gene expression due to early life inflammatory events or psychological and environmental stressors can be causally related to neurodevelopmental diseases and mental health disorders. This type of alterations in the neural functions can occur in the absence of infiltration of inflammatory cells in the brain parenchyma or leptomeninges. In this sense, the pathogenetic state underlying a significant part of psychiatric and neurological diseases may be similar to “para-inflammation”, an intermediate state between homeostatic and classical inflammatory states as defined by Ruslan Medzhitov (Nature 454:428-35, 2008). Therefore, it is important to study how systemic inflammation affects brain health and how local peripheral inflammation induces changes in the brain microenvironment. Chronic pain is also induced by disturbance in otherwise well-organized multisystem interplay comprising of reciprocal neural, endocrine and immune interactions. Especially, early-life insults including exposure to immune challenges can alter the neuroanatomical components of nociception, which induces altered pain response later in life. Recently the discrete roles of microglia and blood monocyte-derived macrophages are being defined. The distinction may be further highlighted by disorders in which the brain parenchymal tissue is damaged. Therefore, studies investigating the dynamics of immune cells in traumatic brain injury and neurotropic viral infections including human immunodeficiency virus, etc. as well as neurodegenerative diseases such as amyotrophic lateral sclerosis are promising to clarify the interplay between the central nervous and immune systems. The understanding of the histological architecture providing the infrastructure of such neuro-immune interplay is also essential. This Frontiers research topic brings together fourteen articles and aims to create a platform for researchers in the field of psychoneuroimmunology to share the recent theories, hypotheses and future perspectives regarding open questions on the mechanisms of cell-cell interactions with chemical mediators among the nervous, immune and endocrine systems. We hope that this platform would reveal the relevance of the studies on multisystem interactions to enhance the understanding of the mechanisms underlying a wide variety of neurological and psychiatric disorders.

Brain inflammation (neuroinflammation), imbalances in brain neuronal integrity and neurotransmitter systems, and cognitive impairment are characteristic features of post-operative conditions, sepsis, liver diseases, diabetes and other disorders characterized by immune and metabolic dysregulation. Derangements in cytokine release also play a pivotal role in depression. Characteristic brain reactive antibodies in autoimmune conditions, including systemic lupus erythematosus and neuromyelitis optica, significantly contribute to brain pathology and cognitive impairment. These studies, and the simultaneous characterization of neuro-protective cytokines, identified new therapeutic approaches for treating neurological complications in inflammatory and autoimmune disorders. This Frontiers Research Topic is a forum for publishing research findings and methodological and conceptual advances at the intersection of immunology and neuroscience. We hope that presenting new insight into bi-directional neuro-immune communication in inflammation and autoimmunity will foster further collaborations and facilitate the development of new efficient therapeutic strategies.

Inflammation in the dorsal root ganglia (DRG) in response to SCI may contribute to nociceptor dysfunction and pain development. Little is known of the immune signaling events that transpire in the DRG after SCI that could induce pathological changes in the nociceptor and drive pain. This body of work examined the role of macrophages in the DRG in the development of neuropathic pain after SCI. Macrophages were found to infiltrate the DRG early after SCI, in response to the rapid but transient elevation of a chemokine and macrophage chemoattractant, CCL2. The magnitude of increase in CCL2 differed in rats with similar injuries, and affected the inflammatory nature of the macrophage response. After pain establishment, macrophage presence was exacerbated in rats with SCI that develop pain, and DRG immune environment was found to be pro-inflammatory. Inhibiting the CCL2-mediated macrophage response prior to pain development induced pain rather than preventing it, and this was found to be due to a decrease in pro- as well as anti-inflammatory cytokines and macrophages. These studies serve as a basis for ongoing work exploring the effect of macrophage phenotype and secretome on pathological nociceptor dysfunction. Understanding the neuroimmune interactions between macrophages and nociceptors in the DRG may lead to the development of pain preventative and pain modulatory therapies.

The International Association for the Study of Pain (IASP) defines neuropathic pain as 'pain arising as a direct consequence of a lesion or disease affecting the nervous system'. It is important to note that with neuropathy the chronic pain is not a symptom of injury, but rather the pain is itself a disease process. Novel interactions between the nervous system and elements of the immune system may be key facets to a chronic disease state. One of particular note is the recent finding supporting an interaction between an immune response protein high mobility group box 1 (HMGB1) and Toll like receptor 4 (TLR4). HMGB1 is an endogenous ligand for TLR4 that influences the induction of cytokines in many non-neuronal cells. After tissue damage or injury, HMGB1 may function as a neuromodulatory cytokine and influence the production of pro-nociceptive mediators altering the state of sensory neurons. Very little is known about the HMGB1-TLR4 interaction in sensory neurons and whether chronic changes in endogenous HMGB1 signaling influence the establishment of neuropathic pain. This thesis aims to determine whether a physiologically relevant neuroimmune interaction involving endogenous HMGB1 and TLR4 in the dorsal root ganglia is altered following a tibial nerve injury model of neuropathic pain. I hypothesized that sensitization of sensory neurons following a peripheral nerve injury is dependent on endogenous HMGB1 and TLR4. The studies presented here demonstrate that HMGB1 undergoes subcellular redistribution from the nucleus to the cytoplasm in primary afferent neurons following peripheral nerve injury. Further, the presence of extracellular HMGB1 may directly contribute to peripheral sensitization and injury-induced tactile hyperalgesia. Though thought to be important as a pivotal receptor for HMGB1 activation, neuronal protein expression of TLR4 does not appear to influence the effects of HMGB1-dependent behavioral changes following peripheral nerve injury. Taken together, these findings suggest that extracellular HMGB1 may serve as an important endogenous cytokine that contributes to ongoing pain hypersensitivity in a rodent model of neuropathic pain.
