Book excerpt: The World Health Organization (WHO) and other global nutrition and health agencies recommend nutrition actions throughout the life-course to address malnutrition in all its forms. In this report, we examined how Nepal’s nutrition policies and programs addressed recommended nutrition actions, nutrition outcomes, and the determinants of these outcomes. We reviewed population-based surveys and administrative data systems in order to assess the data availability on nutrition actions, and on the indicators of determinants and outcomes. Our policy review identified a total of 53 recommended evidence-based nutrition actions, of which 50 nutrition actions were applicable in Nepal. Of these, 45 were addressed in the country’s nutrition policies and programs and some of the actions were only available in some districts. Nutrition actions that were not included in current policies and programs included calcium supplementation and advice on consuming calcium during pregnancy, and daily iron and folic acid (IFA) supplementation during childhood. Current policies addressed daily or intermittent IFA supplementation during preconception and food supplementation for malnourished lactating women during the postnatal period; however, there was no program to implement these actions. Nepal’s Multi-Sector Nutrition Plan (MSNP) recognized and addressed all key determinants of nutrition; it also expressed an intent to address all SDG nutrition targets for maternal, infant, and young child nutrition. Noncommunicable diseases (NCDs), however, were addressed separately by a multisectoral plan for NCDs. Our data review found that out of 45 actions that policies and programs addressed, population-based surveys contained data on only 27 actions and administrative data systems contained data on only 25 actions. Population-based surveys and administrative data sources contained no data on: food supplementation during adolescence; weight monitoring and various types of counseling during pregnancy; optimal timing (delayed) of umbilical cord clamping, support for breastfeeding and immediate skin-to-skin contact, optimal feeding of low-birth-weight infants and counseling of mothers of low-birth-weight infants on kangaroo mother care (KMC) during delivery and in the postpartum period; breastfeeding counseling, counseling on appropriate complementary feeding, counseling after growth monitoring, and inpatient management of severe acute malnutrition (SAM) during early childhood. Population-based surveys contained data on most of the indicators of immediate and underlying determinants, while administrative data systems did not have data on all indicators of immediate determinants. Data on all indicators of nutrition outcomes were available from population-based surveys. In conclusion, Nepal’s policy and program landscape for nutrition is robust, however the gaps in data availability for tracking progress on nutrition actions are much larger than the gap in policies and programs for addressing recommended actions. Future population-based surveys and modifications of administrative data systems should aim to fill the identified data gaps for nutrition actions.

Book excerpt: The United States Government’s (USG) Global Food Security Strategy (GFSS) is an integrated whole-of-government approach that aims to end global hunger, poverty, and malnutrition through the Feed the Future initiative. Launched in 2010 across Africa, Asia, Latin America, and the Caribbean, Feed the Future aims to stimulate agriculture-led growth, resilience, and nutrition in countries with significant needs and opportunities for improvement. Through partnerships that invest in agriculture, build resilience, and improve nutrition, the USG has provided the basic dignity of food to millions of families worldwide. This initiative has delivered impressive results, including an estimated 23.4 million people lifted to above the poverty line, 3.4 million children no longer stunted, and an estimated 5.2 million families overcoming hunger. Additionally, Feed the Future has unlocked $3.5 billion in financing for food security (2011–2019), generated $13.7 billion in agricultural sales, and developed and deployed over 1,000 innovations. However, the Coronavirus disease 2019 (COVID-19) pandemic, climate change, and international conflicts have significantly hampered the achievement of Sustainable Development Goals (SDGs). As a result, the USG has developed the Global Food Security Strategy Refresh (GFSS-R) 2022–2026 that emphasizes the following five new areas of focus: 1. Equity and inclusion 2. An ambitious approach to climate change 3. Countering the long-term impacts of COVID-19 4. Employing systems thinking and working across the food system 5. Integration of conflict management, peacebuilding, and social cohesion This strategy refresh has significant implications for the Feed the Future initiative; therefore, all Feed the Future countries have now embarked on refreshing the country strategy to incorporate new programs and initiatives aligning with the GFSS-R. The strategy involves implementing a multi-faceted whole-of-government approach, assignment of ownership, and empowerment of country leadership to lead and guide the efforts for progress. The strategy refresh invokes partnership with a wide range of stakeholders, and harnesses the advancement of science, technology, and innovation to achieve these objectives. The Feed the Future interventions in Nepal ensured a very close partnership with the Government of Nepal and emphasized greater involvement of the private sector. Nepal is one of the oldest and most successful Feed the Future countries. Although efforts have been made to ensure food security in Nepal, the country still faces nutritional deficiency. Agricultural productivity in Nepal is comparatively low, and most of the farmers are still engaged in subsistence farming. Further, Nepal is highly vulnerable to climate change, natural disasters, pests, and diseases, exacerbating food insecurity leading to the prevalence of nutrition deficiencies, especially among marginalized populations who rely heavily on agriculture for their livelihoods. The USAID mission in Nepal commissioned the IFPRI to support the drafting of the new GFSS Nepal Country Plan. In this regard, provincial consultation workshop has been planned in collaboration with IIDS to generate constructive ideas and feedback on the past and existing interventions implemented in Nepal under Feed the Future and identify a way forward for strengthening and leveraging food and agriculture system transformation in Nepal.

There is an underutilized knowledge base around traditional and indigenous food systems. This includes the knowledge of nutritious species, traditional culinary preparations, and cultural practices. Greater agricultural production of underutilized species can result in more sustainable agricultural and food systems which can also help improve livelihoods and food security. Traditional and indigenous cultural practices with respect to both land and water management, as well as culinary practices, contribute to both sustainable food production and consumption. These practices require a greater evidence base in order to be incorporated into public health nutrition initiatives related to improving dietary quality, such as food-based dietary guidelines for example. An increased focus on the importance of local, traditional, and indigenous food systems and nutrition could therefore help countries to improve human nutrition and, ideally, help mitigate the global syndemic of obesity, undernutrition, and climate change. This Research Topic will focus on documenting diverse local food systems and promoting elements within them that can help improve nutrition and health – both human and planetary - in various ways including the livelihood development of knowledge holders.

Book excerpt: Investments in social assistance programs (SAPs) in low- and middle-income countries (LMICs) are increasing. As investments increase, the objectives of these programs are expanding from focusing on reducing poverty to addressing other social issues such as improving diets and nutrition. At the same time, there is increasing interest in addressing all forms of malnutrition within the framework of food systems. Given the intersections between SAPs and food systems, we reviewed the effectiveness of SAPs (agriculture asset transfers, cash transfers, in-kind transfers, vouchers, public works and school meals programs) for reducing all forms of malnutrition across the lifecycle within a food systems framework. As several programs included multiple treatment arms, each representing a unique program design, we used study arm as the unit of analysis and assessed the proportion of study arms with positive or negative program impacts on diet and nutrition outcomes among men, women and children. The majority of the studies included in this review were from evaluations of agriculture asset, cash and in-kind transfer programs. There was clear evidence of positive impacts on women’s and children’s diet-related outcomes. Very few studies assessed program impact on women’s nutritional status outcomes. However, there was some evidence of impacts on increasing body mass index and hemoglobin concentration (Hb) with in-kind transfer programs. Among children, several study arms across the agriculture asset, cash and in-kind transfer programs found positive impacts on increasing height-for-age Z-score (33%-45% of study arms) and weight-for-height Z-score (33%-50% of study arms) and decreasing the prevalence of wasting (43%-60% of study arms). Cash and in-kind transfer programs also found positive effects on reducing stunting prevalence in 33% and 45% of study arms, respectively. Lastly, a few study arms assessed program impact on increasing Hb with some evidence of positive impacts in in-kind and school feeding programs. There was a paucity of relevant evidence of the effectiveness of voucher and public works programs on diet and nutrition outcomes, for men’s outcomes and on micronutrient status. Several challenges remain in understanding the potential for SAPs to improve diet and nutrition outcomes within food systems including the heterogeneity of program and evaluation designs, populations targeted by the programs and included in evaluations and indicators used to assess impact. Addressing these challenges in future evaluations is important for informing program and policy actions to improve the effectiveness of SAPs within food systems for improving diet and nutrition outcomes across the lifecycle.

The US Government’s Global Food Security Strategy (GFSS) is an integrated whole-of-government approach that aims to end global hunger, poverty, and malnutrition through Feed the Future (FtF) initiatives. Several innovations in food and agriculture sectors have been successfully implemented in Nepal, being one of the oldest and most successful Feed the Future countries. However, more remains to be done to ensure food security in the nation. Farmers are still reliant on subsistence farming, leading to low agricultural productivity in the country. Rapid feminization in agriculture, owing to high outmigration and impact of climate change and natural disasters, has further burdened the poor, women, and marginalized groups in achieving food security. Additionally, the COVID-19 pandemic has had a significant negative impact on the country’s economy, thwarting the achievement of Sustainable Development Goals (SDGs). This National Consultation Workshop was designed with an objective to inform the GFSS-R 2022– 2026, with emphasis on i) equity and inclusion, ii) an ambitious approach to climate change, iii) countering the long-term impacts of COVID-19, iv) employing system thinking and working across food systems, and v) integration of conflict management, peace-building, and social cohesion. The Whole System in the Room (WSR) approach was applied to facilitate the workshop with 116 participants representing government, non-government organizations (NGOs), private sectors, and farmers from the federal, provincial, and local levels. Discussions during the consultation workshop were mostly centered on the proposed five themes: (i) Risk and Resilience Capacity, (ii) Food Systems Innovations, (iii) Markets and Trade, (iv) Food Systems Outputs and Outcomes, and (v) Food System Governance and Control. Additional sub-themes were identified to capture the status of each of the themes, and to address their gaps and constraints along with recommendations required to transform the food and agriculture system of Nepal. The participants have proposed specific prioritized recommendations from the list that was generated from the consultation. The summary of these prioritized recommendations for all five themes is given below.

Book excerpt: Updates for many countries have made it possible to estimate hunger in the world with greater accuracy this year. In particular, newly accessible data enabled the revision of the entire series of undernourishment estimates for China back to 2000, resulting in a substantial downward shift of the series of the number of undernourished in the world. Nevertheless, the revision confirms the trend reported in past editions: the number of people affected by hunger globally has been slowly on the rise since 2014. The report also shows that the burden of malnutrition in all its forms continues to be a challenge. There has been some progress for child stunting, low birthweight and exclusive breastfeeding, but at a pace that is still too slow. Childhood overweight is not improving and adult obesity is on the rise in all regions. The report complements the usual assessment of food security and nutrition with projections of what the world may look like in 2030, if trends of the last decade continue. Projections show that the world is not on track to achieve Zero Hunger by 2030 and, despite some progress, most indicators are also not on track to meet global nutrition targets. The food security and nutritional status of the most vulnerable population groups is likely to deteriorate further due to the health and socio economic impacts of the COVID-19 pandemic. The report puts a spotlight on diet quality as a critical link between food security and nutrition. Meeting SDG 2 targets will only be possible if people have enough food to eat and if what they are eating is nutritious and affordable. The report also introduces new analysis of the cost and affordability of healthy diets around the world, by region and in different development contexts. It presents valuations of the health and climate-change costs associated with current food consumption patterns, as well as the potential cost savings if food consumption patterns were to shift towards healthy diets that include sustainability considerations. The report then concludes with a discussion of the policies and strategies to transform food systems to ensure affordable healthy diets, as part of the required efforts to end both hunger and all forms of malnutrition.

In reponse to the last two APRC's as well as in line with the ZHC, and more recently the new FAO Strategic framework, this publication provides a basis for FAO and others to understand the scope and depth of FLW issues, how these can be addressed, including some good practices from Thailand, China and Nepal.
