Clinical near-infrared spectroscopy (NIRS) is used to monitor changes in cerebral blood oxygenation due to functional activation. It was hypothesized that visually-cued emotional induction tasks can elicit detectable activity in the prefrontal cortex. Data were collected from eleven participants as they performed positively and negatively-valenced emotional induction tasks. Baseline and activation trials were classified offline with accuracies from 75.0--96.7% after applying a feature selection algorithm to determine optimal performance parameters for each participant. Feature selection identified common discriminatory features across participants and relationships between performance parameters. Additionally, classification accuracy was used to quantify NIRS hemodynamic response latency. Significant increases in classification rates were found as early as 2.5 s after initial stimulus presentation. These results suggest the potential application of emotional induction as a NIRS-BCI control paradigm.

Book excerpt: Near-infrared spectroscopy (NIRS) can detect a fast optical signal (FOS), corresponding to optical property changes in neuronal tissue during neuronal activation. The FOS has high temporal and spatial resolution, but has a low signal-to-noise ratio. The FOS has yet to be automatically classi- fied, hence its value as a BCI control signal remains unknown. During offline and online sessions, participants performed a visual oddball task. In offline sessions, the FOS was validated with elec- troencephalography (EEG) measurements of event-related potential (ERPs). Spectral relationships between FOS and ERP oddball responses were found in upper delta and theta bands using coherence and Granger causality metrics. A temporal FOS-ERP correlation was also found 200 ms after oddball presentation. Offline and online FOS classification of oddballs versus frequent responses was achieved with average balanced accuracies of 62 Âą 5% and 63 Âą 6%, respectively. FOS classification results were above chance but did not reach the threshold (>70%) for effective BCI communication.

Book excerpt: The main objective of this PhD research has been to utilize machine learning techniques on near infrared spectroscopy (NIRS) signals, for the development of highly accurate and clinically practical biomarkers for the objective assessment of pain perception. While advances in medical imaging technology have significantly improved the scientific knowledge in regards to the brain's response to noxious stimuli, there remains an unmet clinical need for a practical, inexpensive tool for the reliable and objective assessment of pain perception. Even though functional imaging modalities such as fMRI and PET scans deliver superior spatial information, they are not readily accessible for routine clinical use. On the other hand NIRS is non-invasive, safe, portable and affordable with a short setup time. These features make NIRS ideal for clinical applications. In this thesis we used the cold pressor test to induce different levels of pain in healthy subjects while the NIRS signal was recorded from the frontal regions of the brain. We extracted 54 features from each dataset and used machine learning techniques, logistic regression and support vector machine, to classify the signals based on the self-reported pain scores. To select the model for machine learning, we developed our feature selection algorithm based on a RFE-SVM (recursive feature elimination - support vector machine) method to find subsets of feature space with the highest classification capability. Through this process we identified a subset of 10 features which could distinguish high-pain from low-pain stimuli with an accuracy of 85% (Leave-one-out cross validation). Moreover we applied functional data analysis on the collected NIRS data and converted discrete samples to continuous curves. This time we used the same RFE-SVM method on the coefficients of fDA bases (as opposed to extracted features) and we achieved 94% of accuracy to classify low-pain high-pain signals. Then using machine learning techniques (k-means and hierarchical clustering) we found clusters in the data which covered low pain and high pain groups with an accuracy of 91.2%. The center of these clusters can represent the prototype NIRS response of that pain level. Our approaches provided trial-by-trial predictions of pain level from NIRS measurement for each individual (as opposed to methods based on responses averaged across many trials and subjects), and thus, represent a step towards the goal of establishing an objective clinical bio marker of pain perception. Further refinement of proposed methods, including incorporating more datasets and employing other noxious stimuli, is required to make the NIRS technique a powerful clinical tool for pain assessment.

Book excerpt: Near-infrared spectroscopy (NIRS) has been gaining increasing attention as a brain-computer interface (BCI) modality. However, communication rates offered by this technology are not yet practical for clinical implementation. This thesis investigates the feasibility of enhancing the speed and accuracy attainable by NIRS-BCIs. To date, most studies have utilized activity from a single cortical region, particularly the anterior prefrontal cortex (aPFC), to discern two non-motor-based mental states. However, cognitive processes require coordination between several, spatially-distributed areas. Additional cortical information may help improve classification of mental states, particularly combinations of more than two. Furthermore, testing has largely focused on offline analyses, despite the possibility of enhanced cortical control attainable with feedback. Lastly, NIRS-BCI work has primarily involved able-bodied participants, most of which were offline. In an initial offline study, we investigated the value of utilizing cortical activity measured from multiple brain regions to differentiate all possible combinations of two cognitive tasks and rest. With prefrontal and parietal measurements, average binary and ternary accuracies exceeded 80% and 70%, respectively. Accuracies and information transfer rates (ITRs) improved significantly with the inclusion of measurements beyond the aPFC. A second study was conducted to evaluate ternary classification accuracies achievable in an online system that offered classification feedback. An average online accuracy of 74.2 Âą 14.8% was attained, representing further improvements in NIRS-BCI speed and accuracy. In a final study, we tested an online system developed for a candidate BCI user with severe, congenital muscular impairments. The binary system made use of prefrontal measurements beyond the aPFC and was tailored for the user prior to each online session. Offline and online accuracies of 66.9 Âą 9.11% and 63.33%, respectively, were achieved. Collectively, these findings demonstrate the value of considering multiple brain regions and offering online classification feedback to realize a faster and more accurate NIRS-BCI.
