Book excerpt: This report assesses the general condition of the dam with respect to safety, based on available data and on visual inspection, to determine if the dam poses hazards to human life or property. The dam is in the high hazard potential classification, which means that loss of life and appreciable property loss could occur in the event of failure of the dam. For its size and hazard category, this dam is required by the guidelines to pass one-half PMF to the PMF. The spillway of this dam will pass only 20 percent of the PMF without overtopping the dam. Since the spillway for Sweetwater Dam is not capable of passing a minimum of one-half (50 percent) of the PMF without overtopping the dam, the spillway is considered inadequate. The inspection team observed heavy tree growth covering the downstream slope of the dam and spillways. Potential exists for other deficiencies such as rodent holes to be found after the trees and brush have been removed. The downstream slope of the dam was eroded considerably in the area of the right abutment where flow from the right spillway has occurred int he past. Seepage and stability analyses are not on record as recommended in the guidelines, which is considered a deficiency that should be rectified.

No Name 262 Dam is a small size dam with a high hazard potential required by the guidelines to pass from one-half Probable Maximum Flood to the Probable Maximum Flood without overtopping. It was determined that the spillway will pass the Probable Maximum Flood without overtopping the dam. Our evaluation indicates that the spillway will pass the 100-year flood; that is, a flood having a 1 percent change of being equalled or exceeded during any given year. The Probable Maximum Flood is defined as the flood discharge that may be expected from the most severe combination of critical meteorological and hydrologic conditions that are reasonably possible in the region. Deficiencies noted by the inspection team were a need for periodic inspection by a qualified professional engineer; lack of a maintenance schedule; a need for a complete seepage and stability analyses of the dam embankment; brush and tree growth on the embankment crest and side slopes; and heavy vegetative growth on the spillway crests.

The purpose of the inspection was to make an assessment of the general condition of the dam with respect to safety, based upon available data and visual inspection, in order to determine if the dam poses hazards to human life or property. Failure would threaten the life and property of ten families and cause appreciable damage to two county roads. Our inspection and evaluation indicates the dam is deficient in that the spillway is inadequate. The guidelines for a dam having the above size and hazard potential require that the spillway be capable of passing a one-half PMF (Probable Maximum Flood). The Probable Maximum Flood is defined as the flood discharge that may be expected from the most severe combination of critical meteorologic and hydrologic conditions reasonably possible in the region. Considering the small volume of water impounded, the large floodplain downstream and the three groups of farm buildings downstream, one-half PMF is the appropriate spillway design flood. A 20% PMF will begin to overtop the dam. The lake and spillway are adequate to contain a 1% annual probability flood (100-year flood) without overtopping the dam.

The emergency spillway will only pass 30 percent of the PMF before the dam embankment is overtopped. Because the spillway will not pass 1/2 of the PMF without overtopping but will pass the 10-year frequency flood, the dam is classified as 'unsafe non-emergency'. Other deficiencies visually observed by the inspection team were trees growing on the upstream slope; erosion gulleys in both abutments; and erosion at the outlet of the emergency spillway due to the discharge from the emergency spillway being directed onto the downstream slope of the dam. Another deficiency found was the lack of seepage and stability analysis records.

Book excerpt: Certain deficiencies were noticed during the visual inspection and are considered to have an adverse effect on the overall safety and future operation of the dam and spillway. The upstream slope of the dam is irregular (maximum slope ratio is approximately 1v on 1.3h) beginning at a level about 12 feet below the top of the dam. Excessive irregularities of the face of the dam are indications of instability and/or erosion, both of which are considered detrimental to the safety of the dam. A substantial cover of small trees and brush that may conceal animal burrows exists on the upstream and downstream slopes of the dam. The tree roots and animal burrows can provide a pathway for lake seepage that may develop into a piping condition. The upstream slope has a grass cover to protect it from erosion by wave action. Grass cover is not considered adequate to prevent erosion by wave action for a fluctuating lake level. Loss of material can result in instability and settlement of the dam crest. A dense growth of brush and small trees obstructs the spillway crest and outlet channel.

Book excerpt: The purpose of No Name 587 Dam is to impound tailings from a barite separation and beneficiation operation. The dam completely encircles the impoundment. Runoff from the watershed is diverted around the impoundment via diversion ditches. This dam is classified as intermediate size. The U.S. Corps of Engineers has classified this dam as having high downstream hazard potential to indicate that failure of this dam could threaten life and property. The damage zone estimated by the U.S. Corps of Engineers extends approximately eight miles downstream of the dam. Information provided by the Corps of Engineers and the inspection indicates that about five dwellings, two railroad bridges, a highway bridge and the town of Mineral Point with about eighty dwellings are within this damage zone. The results of the inspection indicate the absence of facilities for discharging floodwater and inadequate freeboard and that the dam does not meet the criteria given in the Guidelines for a structure with the size and hazard potential of No Name 587 Dam. As an intermediate size dam with a high hazard potential, the Guidelines specify that the discharge capacity and/or storage capacity should be capable of safely handling the Probable Maximum Flood (PMF) without overtopping the crest.

Failure of this dam could threaten life and property. The results of the inspection and evaluation indicate that the spillway does not meet the criteria given in the Guidelines for a dam with the size and hazard potential of No Name 550 Dam. It is calculated that the spillway can pass a 100-year flood without overtopping the dam. It was also estimated that the spillway could pass 90 percent of the PMF without overtopping the dam and without significant erosion of the embankment. However, flows greater than 32 percent of the PMF would cause significant erosion of the wing dam constriction. Adequate overflow facilities and freeboard should be provided so that the impoundment can handle the PMF without overtopping the crest and without significant erosion of the embankment or spillway. Gravel has been mined from the dam crest. This has the effect of reducing available freeboard and the dam's ability to retain runoff. This is a deficiency which should be corrected.

Since the spillway of this dam will pass only 25 percent of the PMF without overtopping the dam, and since overtopping of the dam could cause breeching due to erosion it is classified as an unsafe non-emergency structure. Also, our evaluation indicates that the spillway will not pass the 100-year flood, that is a flood having a 1 percent chance of exceedence in any given year. The spillway area immediately downstream of the paved overflow section does not appear sufficiently resistant to prevent embankment erosion at high flows for an indefinite time. See page and stability analyses are not on record as recommended in the guidelines which is considered a deficiency which sould be reconciled. The valve on the 4-inch pipe through the dam is not operable and should be repaired/replaced.
