Book excerpt: This report assesses the general condition of the dam with aspect to safety, based on available data and on visual inspection, to determine if the dam poses hazards to human life or property. The St. Louis District (SLD), Corps of Engineers, has classified this dam as having a significant hazard potential. Based on the visual inspection and analysis of available data, the dam is judged to be in very poor condition. This assessment is based primarily on the history of the recent breach and seepage which developed at this dam, possibly along piping paths resulting from decaying root systems. The size of the trees on the embankment and the number of stumps where trees have been cut down indicate that similar seepage areas will likely continue to appear and pose a hazard of dam failure. Hydraulic/hydrologic analyses of the dam and appurtenant structures indicate the dam will be overtopped by a flood greater than 10 percent of the Probable Maximum Flood (PMF). The 1 percent probability-of-occurrence flood (occurs on the average once every 100 years) will overtop the dam by 0.7 ft. The relatively narrow embankment section and moderately erodible soil used in construction of the dam suggest overtopping would likely produce an effective breach of this dam. Seepage and stability analyses comparable to the requirements of the 'Recommended Guidelines for Safety Inspection of Dams' were not available. This is considered a deficiency which should be rectified.

Book excerpt: This dam is classified as a small size dam with a high downstream hazard potential. Failure would threaten the life and property of approximately 5 families downstream of the dam. The spillway does not meet the criteria set forth in the guidelines for a dam having the above mentioned size classification and hazard potential. According to the guidelines, the spillway is required to pass the Probable Maximum Flood (PMF) without the dam embankment being overtopped. The spillway will only pass 5 percent of the PMF before the dam embankment is overtopped. Because the spillway will not pass one-half of the PMF without overtopping, the dam is classified as 'unsafe Non-emergency'. The spillway will not pass the 100-year flood without overtopping, which is a flood that has a 1 percent chance of being exceeded in any given year. Other deficiencies visually observed by the inspection team were trees and bushes next to the concrete core wall, in and adjacent to the principal spillway, and on the downstream embankment slope; erosion gullies on the downstream embankment slope; and seepage. Another deficiency found was the lack of seepage and stability analysis records.

Book excerpt: The dam is an earth dam, constructed of weathered rock and soil from the vicinity of the dam, impounding a lake for recreational purposes. When the reservoir was first filled, the owner noticed a loss of water and drained the lake to perform repairs. The reservoir was still nearly empty at the time of the visual inspection. The spillways are at the left abutment of the dam (as the observer faces downstream). The main spillway is a 24-in. diameter corrugated steel pipe passing through the embankment near the left abutment. The auxiliary spillway is an uncontrolled, unlined, irregularly-shaped open channel in the left abutment. Our inspection and evaluation indicate the embankment is in generally good condition. No evidence was found of significant erosion, slope instability, disruption of the vertical or horizontal alignment of the dam, or detrimental vegetation. Hydrologic/hydraulic analyses indicate that a 1 percent probability-of-occurrence event (100-year flood) will not result in overtopping of the dam. These analyses also indicate that the dam will be overtopped by a flood event which produces greater than 14 percent of the Probable Maximum Flood (PMF).

The combined spillways will pass 25 percent of the Probable Maximum Flood without overtopping. The Probable Maximum Flood is defined as the flood discharge that may be expected from the most severe combination of critical meteorologic and hydrologic conditions that are reasonably possible in the region. The guidelines require that a dam of small size with a high downstream hazard potential pass 50 to 100 percent of the PMF. Considering the low height of dam (26 ft) and the small storage capacity (64 acre-ft) 50 percent of the PMF has been determined to be the appropriate spillway design flood. The embankment was in good condition.

The assessment was based on an evaluation of the available data, a visual inspection and an evaluation of the hydrology and hydraulics of the site to determine if the dam poses hazards to human life or property. The purpose of the dam is to impound lead tailings. Inspection and evaluation indicated that the combined capacity of the spillway and 48-inch diameter concrete outlet does not meet the criteria given in the Guidelines for a dam of the size and hazard potential of Main Tailings Dam. As a small dam with a high hazard potential, it is required by the Guidelines to pass to 50 100 percent of the Probable Maximum Flood (PMF) without overtopping the crest. The spillway design flood adopted for this dam is 100 percent of the PMF because the consequences of failure are serious since the dam is located about 1/4-mile from a residential area of Fredericktown. The PMF is the flood that maybe expected from the most severe combination of critical meteorologic and hydrologic conditions that is reasonably possible in the region. It was calculated that the spillway and outlet could pass a 100-year flood (a flood having a 1 percent change of being equalled or exceeded in any 1 year) without overtopping the dam.
