The following items were noted: The cement joints of the spillway wing walls are badly deteriorated; A stone is missing from the right downstream side of the spillway near the top of the right wing wall; On the masonry portion of the dam, the cemented joints of the top few courses of stones are deteriorated; One of the stones is missing from the top of the downstream face; The masonry wall above the outlet works has deteriorated; Two small seeps were found in the masonry wall below the crest of the weir; three other small seeps were found on the downstream face of the dam. A small wet area was observed at the toe of the left section of the embankment; The stumps and root systems of several large trees are present in the downstream embankment. One of the stumps is partly uprooted; and There are fallen trees and other debris in the spillway discharge area.

On the downstream slope there are a large number of trees below the road; The masonry portion of th left spillway wall has collapsed for approximately 3 feet long at the crest of the spillway weir; A large number of rodent holes are scattered on the downstream face of the dam; and There is a small eroded area on the downstream face of the dam near the left abutment.

The examination of documents and the visual inspection of the Goshen Reservoir did not reveal conditions which constitute an immediate hazard to human life or property. However, the dam has some problem areas which require additional studies and remedial work. The most serious of these deficiencies are the two areas of seepage on the downstream slope of the dam. First is at the end of a grouted stone spillway channel and the second is at the toe in the center of the dam. It is recommended that within six months from notification, an engineering investigation is initiated to determine the sources of the seepage found. Using the Corps of Engineers' screening criteria for the initial review of spillway adequacy, it has been determined that the embankment would be overtopped by all storms in excess of 51% of the Probable Maximum Flood (PMF). Therefore, the spillway is adjudged as 'inadequate.' (Author).

Monitoring of the reservoir levels should be expanded to include readings during peak flow periods.

The classification of 'unsafe' applied to a dam because of a 'seriously inadequate' spillway is not meant to connote the same degree of emergency as would be associated with an 'unsafe' classification applied for a structural deficiency. It does not mean that there appears to be a serious deficiency in spillway capacity; and if a severe storm were to occur, overtopping and failure of the dam could take place significantly increasing the hazard to loss of life downstream of the dam.

Information and analysis are based on visual inspection of the dam by the performing organization. Mine Lake Dam is classified as a small dam with a significant hazard potential. The dam is constructed of masonry and is 13.9 feet high and 516 feet in length. Over 300 feet of the dam's length is less that 8 feet in height. The dam's reservoir is only 25 acres. Popolopen Dam is located one mile above Mine Lake, while Stilwell Dam and reservoir is located a few hundred feet below Mine Lake Dam. The drainage area of the dam is 8.7 square miles. The structure forms an impoundment which is used for recreational purposes on the West Point Military Reservation. Hydrology computations prepared according to the Corps of Engineers' screening criteria establishes the spillway capacity of 1,492 cfs at 9.9 percent of the PMF with the PMF discharge at 15,070 cfs and the 1/2 PMF discharge at 7,170 cfs. Since the spillway cannot pass the 1/2 PMF discharge without overtopping the dam, the spillway is inadequate. The stability analysis determined three loading cases where the dam was unstable. Each of these cases involved uplift on the base of the dam due to hydrostatic forces in combination with the PMF, seismic loadings or ice loadings. Only the ice load occurs in a normal operating situation. The condition of uplift is plausible since seepage was observed at the juncture of the masonry spillway with the bedrock.

Removal of backfill, installation of weeps and stiffening of the wall should control this problem; Seepage was observed resulting from deterioration of the construction joints and cracking of the spillway concrete. Vegetation was also noted growing in the joints of the spillway chute slabs. All construction joints require cleaning and recaulking to reduce seepage and inhibit infiltration through the spillway chute and core wall; Cracking of the exposed core wall, spillway buttresses, return walls and spillway chute slabs were observed. These areas require repair. Monitor the deterioration of the catwalk and repair as required, and Monitor the seepage observed at the base of the spillway chute and at the reservoir drain outlet. If seepage increases significantly, investigate and repair.
