The inspection consisted of: Visual inspection of the retaining structure, abutments and toe; Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works; Observations affecting the runoff potential of the drainage basin; and Evaluation of the downstream area hazard potential. The dam appeared to be in fair condition. From a brief survey conducted during the inspection, it was noted that the low spot on the top of dam existed adjacent to either edge of the spillway on the retaining walls. The earthen embankment section rises gently from the spillway to either abutment.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the facility is considered to be in good condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.

Book excerpt: The assessment of Sugar Creek Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The inspection and review of Sugar Creek Dam did not reveal any problems which would require emergency action. The dam appears to be in fair condition and in need of minor maintenance. The Sugar Creek Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to PMF. The PMF has been selected as the spillway design flood based on the downstream potential for loss of life. The spillway and reservoir are considered capable of controlling approximately 50% of the PMF. The 0.1 feet of overtopping of the embankment crest during the 1/2 PMF event for a duration of 1 hour was not considered sufficient to cause failure of the structure. Based on the judgement of the evaluating engineer, the location and duration of the overtopping as well as the existence of a concrete curb on the upstream and downstream edge of the embankment crest would sufficiently contain the overtopping to the spillway area minimizing damage to the embankment and spillway. The spillway is considered inadequate.

The masonry gravity section of the embankment has no significant deviations from the OCE guideline for stability, since the toe pressure is well below the allowable.

The condition of the embankment is considered to be poor. The dam is old and in a general state of disrepair. The crest of the dam is irregular and the center of the dam appears to have settled. The stone wall along the downstream toe is irregular and bulging. The toe of the wall is swampy, caused by a general underseepage. In view of these observations, the overall stability of the dam is considered to be questionable, requiring further investigation and implementation of measures to improve the stability of the dam. The spillway capacity was evaluated according to the recommended procedure and was found to pass less than 10 percent of the Probable Maximum Flood (PMF) without overtopping the embankment. This capacity is less than the required spillway capacity of one-half PMF relative to the size and hazard classification of the dam. Because the spillway capacity is less than 50 percent of the PMF and it is estimated that failure of the dam due to overtopping would significantly increase the downstream hazard of loss of life compared to that which would exist just before failure, the spillway is considered to be seriously inadequate, and consequently the condition of the dam is considered to be unsafe/nonemergency.

The Roscoe Burgess Dam can be located on the Overton U.S.G.S. 7.5 minute quadrangle. The onsite inspection of the Roscoe Burgess Dam consisted of: (1) Visual inspection of the retaining structure, abutments and toe. (2) Examination of the spillway facilities, exposed portion of any outlet works and other appurtenant works. (3) Observations affecting the runoff potential of the drainage basin. (4) Evaluation of the downstream area hazard potential. In general, the dam and appurtenant structures appear to be in fair condition. No major erosion areas were observed during the inspection. No seepage was observed along the downstream slope of the structure or along the toe area. Dense brush exists along the downstream slope of the structure and hampered attempts at close visual inspection of the downstream slope and toe area. The brush and small trees on the downstream slope and in the area of the toe of the dam should be removed under the direction of a registered professional engineer. Once the slope and toe area are cleared, the area should be reinspected to verify that no seepage existed at the structure. No drainline was observed at the structure. This is considered a deficiency in some means should be provided to drain the reservoir.

Book excerpt: The assessment of Brown's Lake Dam is based upon visual observations made at the time of inspection, hydraulic and hydrologic computations and past operational performance. The inspection and review of data of the Brown's Lake Dam did not reveal any problems which would require emergency action. The dam appears to be in good condition and adequately maintained. Brown's Lake Dam is a high hazard-small size dam. The spillway design flood (SDF) for a dam of this size and classification is 1/2 PMF to the PMF. the PMF has been selected as the spillway design flood based on downstream potential for loss of life. The spillway and reservoir are capable of controlling the spillway design flood (PMF). Based on criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted as soon as possible. (1) A regularly scheduled maintenance program should be developed and implemented. (2) A warning system should be developed to warn downstream residents of large spillway discharges or imminent failure of the dam. (3) A safety inspection program should be implemented with inspections at regular intervals by qualified personnel. (4) The drainline valve should be operated on a regular basis.

The facility is provided with an uncontrolled, rectangular, concrete chute channel spillway located at the left abutment. The spillway is constructed with a 75-foot long, broad crested weir having a 35-foot breadth. Drawdown capability is provided by means of an 18-inch diameter reinforced concrete conduit controlled at the inlet by an 18-inch diameter sluice gate. Based on a visual inspection, operational history, and available engineering data, the dam is considered to be in fair condition. The facility will pass and/or store only about 43 percent of the PMF prior to embankment overtopping. (Author).
