The size classification of the facility is small and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility ranges between the 1/2 PMF (Probable Maximum Flood) and the PMF. Due to the high potential for damage to downstream structures and possibly loss of life, the SDF is considered to be the PMF. Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 77 percent of the PMF prior to embankment overtopping. Consequently the spillway is assessed as being inadequate, but not seriously inadequate. Deficiencies noted by the inspection team included the following: (1) large trees rooted within the downstream embankment face; (2) a damaged and inadequate spillway discharge channel; and (3) a bent gate stem and partially obstructed outlet associated with the blowoff conduit.

Book excerpt: Memorial Lake Dam was constructed in 1945 as a recreational facility for the Indiantown Gap Military Reservation. The 760 foot long rolled earthfill embankment has a maximum fill height of 37 feet above the original streambed. Reference is made to Appendix F, Plates III and IV, for a general plan and typical section. A cutoff trench was excavated along the centerline of the dam and a concrete cutoff wall was placed in this trench. A 110 foot long ogee spillway was excavated in the left abutment. It has a weir crest elevation of 430.0, which is 8 feet below the top of dam. The ogee discharges the water to an eight foot deep stilling basin. A control tower is located 12.5 feet upstream from the centerline dam. Flow through the 48-inch conduit is controlled inside the tower by a 48-inch sluice gate or by a gated weir also located inside the tower and near normal pool elevation.(Author).

Book excerpt: Lake Jean Dam is a zoned earth embankment approximately 26 feet high and 780 long, including spillway. The emergency spillway is an uncontrolled, rectangular, concrete and rock cut chute channel located at the right abutment. The crest of the spillway is 20 feet long and is spanned by a concrete bridge which supports a portion of the bituminous roadway that covers the embankment crest. Drawdown capability is provided by means of a 30-inch diameter cast iron blowoff pipe controlled at the inlet by a 30-inch diameter sluice gate. The facility consists of a main dam (Lake Jean Dam) and two appurtenant saddle dams (east and west dikes). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store about 73 percent of the PMF prior to overtopping the east dike. Thus, based on the screening criteria contained in the recommended guidelines, the spillway is considered to be inadequate, but not seriously inadequate. Should the saddle dams be regraded to their design elevation, the spillway would then pass floods in excess of the PMF and would be considered adequate.

Hydraulic/hydrologic evaluations revealed that the spillway capacity is less than the peak inflow to the impoundment during the 100-year flood. A spillway design flood (SDF) in the range of the 50- year flood to the 100-year flood is required for Warner Dam. The 100-year flood was chosen as the SDF. The spillway is therefore considered 'Inadequate.' It is recommended that the owner immediately develop remedial measures to provide adequate spillway capacity. The inspection revealed certain items of remedial work which should be performed by the owner without delay. These include: (1) Provide adequate spillway capacity; (2) Repair the spalling on the downstream face of the concrete corewall; (3) Ensure that the outlet works are operable, or develop an emergency drawdown plan; and (4) Cut all trees on the embankment at ground level. All trees with a trunk diameter greater than 3 inches should have their root systems removed. All resultant areas of erosion and cavities should be filled, graded, compacted and seeded. In addition, the following operational measures are recommended to be undertaken by the owner: (1) Develop a detailed emergency operation and warning system; (2) During periods of unusually heavy rainfall, provide around-the-clock surveillance of the dam; and (3) When warning of a storm of major proportions is given by the National Weather Service, activate the emergency operation and warning system.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

The facility is constructed with a free overfall, rectangular, concrete chute channel spillway located at the right abutment. Discharges are controlled by a round crested, triangular shaped weir structure. An intake tower (currently dilapidated, but, scheduled for the renovation) is located near the axis of the dam on the upstream slope. Drawdown capability is provided from the tower through an 18-inch diameter cast iron pipe. Auxiliary drawdown may be available through a 12-inch diameter pipe that originates in the spillway forebay; however, its operability is unknown. Based on a visual inspection, operational history and hydrologic/hydraulic analysis, the dam is considered to be in fair condition. The facility will pass and/or store only about 40 percent of the PMF prior to embankment overtopping.

Book excerpt: The purpose of the inspection is to determine if the dam constitutes a hazard to human life or property. The dam consists of a homogeneous earthfill embankment that is 680 feet long and 64 feet high at its maximum section. The main spillway is a drop spillway located near the left abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. One 12-inch by 21-inch uncontrolled orifice is located in the upstream face of the concrete riser with the orifice crest at El. 1227.0, which is 27 feet below design elevation of the top of the dam. The auxiliary spillway is an excavated channel at the right abutment of the dam. It is grass-lined except for portions that were excavated into rock. The hazard classification is high hazard. Downstream conditions indicate that a high hazard classification is warranted for PA-468 Dam. It is owned by the Susquehanna County Commissioners. Its purpose is for flood control.
