In accordance with the Corps of Engineers' evaluation guidelines, the size classification of this dam is small and the hazard classification is high. These classifications indicate that the Spillway Design Flood (SDF) should be in the range of one-half the Probable Maximum Flood (PMF) to the full PMF. The recommended SDF for this structure is one-half the PMF. The spillway capacity is adequate for passing only 10 percent of the PMF peak inflow without overtopping the dam. The spillway is considered to be seriously inadequate. The dam is therefore unsafe, non-emergency.

The original purpose of the impounded water was for ice harvesting and recreation. Additionally, water releases from the impoundment activated a saw mill. Presently, the lake is being used for recreation. The overall appearance of the dam is poor. Location of observed deficiencies are shown on the General Plan. The profile and typical sections of the dam are presented and are based on field survey made on the day of inspection. The survey datum for this inspection is elevation 1527 feet above mean sea level which is stamped on the USGS Bench Mark (No. TT15K, 1931) on the large sandstone slab, upstream of the service spillway. On the inspection date (12/10/1980), the lake was being drawn down for the winter. The lake level was approximately at elevation 1523.6, or 2.8 feet below its normal level. Deficiencies observed during the field inspection are described. Visible features are depicted in photographs.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Two homes, a trailer, and township road are located 1800 feet downstream from Hathaway Pond Dam. These could suffer economic damage in the event of a dam failure.

Based on the type of construction and the condition of the dam, it is judged that the dam could not withstand the depth and duration of overtopping that would occur for the PMF or the 1/2 PMF. Failure of the dam would cause an increased hazard to loss of life downstream. As a whole, the dam is judged to be in poor condition. The spillway has undergone a gradual structural failure that has resulted in a reduction of spillway capacity. Continuation of the gradual failure, or perhaps a sudden failure, appears to be possible. There are no functional outlet works for the dam. Maintenance of the project is inadequate. There is no program for regular inspection of the dam.

The dam has no functional low level outlet facility.

Book excerpt: The assessment of Pioneer Lake Dam is based upon visual observations made at the time of inspection, review of available records and data, hydraulic and hydrologic computations and past operational performance. The dam and appurtenant structures appear to be in fair condition. No major erosion was observed on the embankment crest. An erosion area was observed at the outlet for the 12 in. diameter cast iron pipe. Erosion at the outlet of the 12 in. diameter pipe exists on the downstream slope of the dam near the toe. Seepage 100 ft. right of the spillway channel was measured to be approximately 50 gallons per minute. A 30 in. corrugated metal pipe exists beneath the roadway near the outlet for the emergency spillway. It was observed during the inspection that the pipe is collapsing near the center of the pipe. Trees were observed growing along the upstream edge of the crest, and dense vegetation was observed along the entire length of the downstream slope. The Pioneer Lake Dam is a high hazard-small size dam. The recommended spillway design flood (SDF) for a dam of this size and classification is in the range of the 1/2 PMF to PMF. Due to the downstream potential for loss of life and property damage, the spillway design flood has been selected as the PMF. The spillway and reservoir are capable of controlling approximately 11% of the PMF without overtopping the embankment. Based on criteria established by the Corps of Engineers, the spillway is termed seriously inadequate. Pioneer Lake Dam is classified as unsafe, non-emergency.

Book excerpt: Lake Sophia Dam consists of an earth embankment approximately 340 feet long with a maximum height of 29 feet from the downstream toe and a crest width of 9 feet. The upstream side of the dam is protected by a small amount of riprap and the downstream face is covered with grass. The flood discharge facilities for the dam consist of a concrete open-channel spillway located near the left abutment. The outlet facilities consist of a 12-inch-diamenter corrugated metal pipe extending through the embankment along the original streambed. Based on the evaluation of existing conditions, the condition of Lake Sophia Dam is considered to be unsafe/nonemergency due to seriously inadequate spillway capacity. The flood discharge capacity was evaluated according to to the recommended procedure and was found to pass approximately 15 percent of the PMF without overtopping the dam. Results of the breach analysis indicate that downstream damage would be significantly increased due to a dam failure.

Book excerpt: Based on visual inspection, Acre Pond Dam is judged to be in fair structural condition. In the absence of an adequate spillway, the dam is subject to frequent overtopping. Based on the location of the downstream dwellings and the fact that a few lives could be lost should the dam fail, the dam is classified as a significant hazard dam. Since the spillway cannot pass the 100- year flood without overtopping the dam, the spillway is rated as inadequate. Although there is some seepage thru the dam, there is no evidence of structural instability. There is no formal inspection and maintenance program or warning system and evacuation plan in effect at Acre Pond Dam. The dam is not properly maintained.

Book excerpt: Based on a visual inspection, operational history, and available engineering data, the facility is considered to be in good condition. The size classification of the facility is intermediate and its hazard classification is considered to be high. In accordance with the recommended guidelines, the Spillway Design Flood (SDF) for the facility is the PMF (Probable Maximum Flood). Results of the hydrologic and hydraulic analysis indicate the facility will pass and/or store only about 45 percent of the PMF prior to embankment overtopping. A breach analysis indicates that failure under less than 1/2 PMF conditions could lead to increased downstream damage and potential for loss of life. Thus, based on screening criteria provided in the recommended guidelines, the spillway is considered to be seriously inadequate and the facility unsafe, non-emergency. Calculations also indicate that if the spillway were constructed in accordance with available desing drawings, the facility could pass and/or store approximately 63 percent of the PMF.

Book excerpt: Based on the evaluation of the existing conditions, the condition of Tuscarora Lake Dam is considered to be fair. Although some deficiencies were observed such as seepage from the toe and lack of upstream erosion protection, none were considered to be significant relative to the overall stability of the dam at this time. Periodic inspection and evaluation of the seepage conditions are recommended. It was found that the dam has no now level outlet facilities. The flood discharge capacity of the dam was evaluated according to the recommended criteria and the spillway was found to pass 10 percent of the PMF without overtopping the low spot of the embankment. This capacity is less than the recommended spillway capacity of 50 percent of the PMF which was based on the size and downstream hazard classification of the dam. Therefore, the spillway capacity is classified to be inadequate.
