The spillway design flood is the PMF (Probable Maximum Flood). The spillway and reservoir are capable of controlling the PMF. Based upon the criteria established by the Corps of Engineers, the spillway is termed adequate.

The dam appears to be stable, well maintained, safely operated and in good condition. Hamilton Lake Dam is a high hazard-intermediate size dam. The spillway design flood is the PMF (Probable Maximum Flood). The spillway and reservoir are capable of controlling the PMF. Based upon the criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted immediately: (1) Continue annual safety inspections and operation and maintenance inspections; and (2) A warning system should be instituted to warn downstream residents of large spillway discharges or failure of the dam.

The spillway and reservoir are capable of controlling the spillway design flood (PMF). Based on criteria established by the Corps of Engineers, the spillway is termed adequate. The following recommendations and remedial measures should be instituted as soon as possible. (1) A regularly scheduled maintenance program should be developed and implemented. (2) A warning system should be developed to warn downstream residents of large spillway discharges or imminent failure of the dam. (3) A safety inspection program should be implemented with inspections at regular intervals by qualified personnel. (4) The drainline valve should be operated on a regular basis.

Book excerpt: Shickshinny Lake Dam is an earthfill dam, 365 feet long and 33 feet high. The crest width of the dam is 17 feet. The upstream slope is 3H:1V and the downstream slope is 2.5H:1V. Both the upstream and downstream slopes are grass covered. Some riprap exists on the upstream slope. The principal spillway consists of a reinforced concrete riser and a 30 inch reinforced concrete pipe which passes under the dam. The top of the riser unit is provided with an anti-vortex device. A gate valve is provided on an inlet pipe just inside the riser unit. Based on our hydraulic and hydrologic analysis it was determined that the spillway and reservoir are capable of controlling approximately 52% of the PMF. Based on criteria established by the Corps of Engineers, the spillway is termed inadequate. No major deficiencies were observed during the inspection. Seepage was noted at the left abutment contact and did not appear to be excessive. Erosion was observed at the outlet structure for the spillway and drainline.

The axis of the dam has a 137 deg. bend in the upstream direction 860 feet from the left abutment. The emergency spillway is located on the right (north) abutment. The spillway is 170 feet wide and is open cut in shale and weathered shale. The principal spillway consists of 48 inch concrete pipe with a reinforced concrete riser inlet. Extending upstream of the riser is a 36 inch concrete pipe with a reinforced concrete reservoir drain inlet at the upstream toe of the embankment. At the downstream end of the 48 inch diameter pipe is a concrete impact stilling basin. The embankment and appurtenant structures appear to be in good condition and well maintained. However, the presence of the seepage zones and wet areas are of concern. The long term effect of these areas on the stability of the embankment is unknown. The existing spillways and reservoir are capable of controlling the PMF (probable maximum flood). Based upon criteria established by the Corps of Engineers, the spillway is termed adequate.

The riser unit is 2.5 feet by 7.5 feet and is 33 feet high. The emergency spillway is located on the left abutment and is formed by a trapezoidal shaped open cut in glacial sand clay and gravel. The bottom width of the spillway channel is 100 feet. The spillway exit channel is 482 feet long and discharges on the left hillside below the toe of the dam. The inspection and review of data of North Fork Dam did not reveal any problems which require emergency action. The dam appears to be stable, well maintained, safely operated and in good condition. However, the spillway and reservoir are capable of controlling 60% of Probable Maximum Flood without overtopping the earth embankment. Based upon criteria established by the Corps of Engineers, the spillway is termed inadequate, but not seriously inadequate. In addition, the earth berm in the emergency spillway exit channel is low and overtopping of the berm during flood conditions will cause erosion of the embankment/abutment contact. This spillway berm will be overtopped at approximately 25% of the PMF.

Book excerpt: Beaver Lake Dam is an earthfill dam with a vertical concrete wall forming the upstream slope and a vertical stone wall forming the downstream slope. The material between the two walls is earth. The crest width is approximately 12 feet. The dam is 179 feet long and approximately 9 feet high. The dam crest is curved concaved in an upstream direction. The right abutment is very gently sloping. The spillway is located in the center of the dam and is 19 feet long. There are no outlet pipes or drain facilities in the dam.

Book excerpt: Maple Lake Dam is an earthfill embankment 14.4 feet high and 283 feet long, excluding spillway. The emergency spillway is located at the left abutment, consisting of an earth channel having a trapezoidal cross section. The bottom width of the spillway at the reservoir outlet is 56 feet. A service spillway, consisting of a 12-inch riser pipe and an 8-inch diameter outlet pipe, is located approximately 100 feet from the right abutment. The outlet works was reported to consist of an 8-inch diameter asbestos cement pipe, extending upstream of the riser pipe to the reservoir and terminating with two sections of an 8-inch diameter vitrified clay pipe, having a concrete plug at the inlet. Based on visual inspection, the Maple Lake Dam is judged to be in good condition. Under the present conditions, the spillway will pass approximately 37 percent of the PMF. Should the top of the dam be restored to its design elevation, the spillway could pass the SDF without overtopping the dam.

Book excerpt: The purpose of the inspection is to determine if the dam constitutes a hazard to human life or property. The dam consists of a homogeneous earthfill embankment that is 680 feet long and 64 feet high at its maximum section. The main spillway is a drop spillway located near the left abutment of the dam. It consists of a concrete riser connected to a 30-inch diameter reinforced concrete pipe under the embankment. One 12-inch by 21-inch uncontrolled orifice is located in the upstream face of the concrete riser with the orifice crest at El. 1227.0, which is 27 feet below design elevation of the top of the dam. The auxiliary spillway is an excavated channel at the right abutment of the dam. It is grass-lined except for portions that were excavated into rock. The hazard classification is high hazard. Downstream conditions indicate that a high hazard classification is warranted for PA-468 Dam. It is owned by the Susquehanna County Commissioners. Its purpose is for flood control.
