Book excerpt: Despite some improvement in theatre utilisation, waiting lists and the length of time which patients have to wait for treatment in Northern Ireland remain the worst in the UK. Using operating theatres to their optimum could directly contribute to their reduction. Following on from a Northern Ireland Audit Office report (HCP 552, session 2002-03; ISBN 0102924333) published in April 2003, the Committee's report examines options for maximising operating theatre capacity and reducing waiting lists; better theatre management and control; and the shortage of theatre staff and the limited availability of beds.

Book excerpt: The National Programme for Information Technology in the NHS (the Programme) is a ten year programme to use information technology (IT) to reform the way the NHS in England uses information, and hence to improve services and the quality of patient care. The core of the Programme will be the NHS Care Records Service, but other elements include x-rays accessible by computer, electronic transmission of prescriptions, and booking of first outpatient appointments. The Programme was launched in 2002, and is now run by an agency, NHS Connecting for Health. This report examines: the progress made in delivering the systems against the original plans and costs (part 1); steps taken by the Department of Health, the agency and the NHS to deliver the Programme (part 2); how the IT systems have been procured (part 3); how the NHS is preparing to use the systems (part 4). The NAO estimates the gross cost of the Programme will be £12.4 billion to 2013-14. Although the pilot NHS Care Records Service will not be in place until late 2006, almost two years late, and other milestones have been deferred, the NAO reports substantial progress with the Programme. Management systems are in place, contracts were placed quickly and achieved large reductions in prices from bidders, and contract terms include important safeguards to secure value for money. Deployments of operational systems have begun, and several additional tasks have been delivered that were outside the original brief. Three key areas are identified which present significant challenges to the successful implementation of the Programme: ensuring IT suppliers continue to deliver systems that meet the needs of the NHS, to agreed timescales without further slippage; ensuring NHS organizations play a full part in implementing the systems; winning the support of NHS staff and the public in making the best use of the systems to improve services. The NAO report makes a number of recommendations for future management of the Programme.

Available in PDF, EPUB and Kindle. Book excerpt: Each year, hospital-acquired infections, prescribing and treatment errors, lost documents and test reports, communication failures, and other problems have caused thousands of deaths in the United States, added millions of days to patients' hospital stays, and cost Americans tens of billions of dollars. Despite (and sometimes because of) new medical information technology and numerous well-intentioned initiatives to address these problems, threats to patient safety remain, and in some areas are on the rise. In First, Do Less Harm, twelve health care professionals and researchers plus two former patients look at patient safety from a variety of perspectives, finding many of the proposed solutions to be inadequate or impractical. Several contributors to this book attribute the failure to confront patient safety concerns to the influence of the "market model" on medicine and emphasize the need for hospital-wide teamwork and greater involvement from frontline workers (from janitors and aides to nurses and physicians) in planning, implementing, and evaluating effective safety initiatives. Several chapters in First, Do Less Harm focus on the critical role of interprofessional and occupational practice in patient safety. Rather than focusing on the usual suspects-physicians, safety champions, or high level management-these chapters expand the list of "stakeholders" and patient safety advocates to include nurses, patient care assistants, and other staff, as well as the health care unions that may represent them. First, Do Less Harm also highlights workplace issues that negatively affect safety: including sleeplessness, excessive workloads, outsourcing of hospital cleaning, and lack of teamwork between physicians and other health care staff. In two chapters, experts explain why the promise of health care information technology to fix safety problems remains unrealized, with examples that are at once humorous and frightening. A book that will be required reading for physicians, nurses, hospital administrators, public health officers, quality and risk managers, healthcare educators, economists, and policymakers, First, Do Less Harm concludes with a list of twenty-seven paradoxes and challenges facing everyone interested in making care safe for both patients and those who care for them.

Available in PDF, EPUB and Kindle. Book excerpt: The importance of good nutrition for individual health and well-being is widely recognized, yet for a significant number of people who rely on institutions for food and nutrition, this importance has not always been a primary consideration. People, therefore, may find themselves consuming food they would not ordinarily choose to eat, with, in some cases, restricted choices precluding individual preferences and compromising health. In recent years, there have been major advances in the quality of catering in some areas, particularly schools. Other institutions which have not been thrust into the media spotlight have fared less well in terms of policy drive and commitment. This insightful new book looks in detail at five institutions: schools, hospitals, care homes for the elderly, prisons and the armed forces. As well as providing a fascinating history of the provision of food in each institution, each section considers: current policy and standards and their implementation adequacy of food provided with regard to the health status and dietary requirements of the people in the care of each institution efficiency of catering organization and issues relating to contract tendering, expenditure and procurement A broad spectrum of further relevant issues is also covered, including the meaning of food to those in institutions and determinants of choice.

Book excerpt: This timely study analyzes social, economic, political, provider, and patient factors shaping collective patient involvement in European health care from the postwar period to the present day. Examining representative countries England, the Netherlands, Germany, and Sweden, it documents the roles of providers and legislatures in facilitating consumer involvement, and the varied forms of patient input into hospital operations. These findings are compared and contrasted against the intent and ideals behind patient involvement to assess the effectiveness of implementation policy, strengths and drawbacks of patient participation, and patient satisfaction and outcomes. The book’s conclusions identify emerging forms of patient participation and predict the impact of health policy on the future of European collective patient involvement. Included in the coverage: · Patient involvement: who, what for, and in what way? · The Netherlands: the legislative process to collective patient involvement · England: formal means of public involvement—a continuous story of discontinuity · Germany: Joint Federal Committee—the “Little Legislator” · Sweden: reasons for a late emergence of patient involvement · Lessons to be learned from implementing patient involvement The Evolution and Everyday Practice of Collective Patient Involvement in Europe will interest and inspire scholars and researchers in diverse fields, including social policy, sociology, political sciences, and nursing studies, as well as patient organizations, policymakers, and healthcare providers.

This book was released on 2005-06-24 with total page 288 pages. Available in PDF, EPUB and Kindle. Book excerpt: This joint report was prepared by the National Audit Office and the Audit Commission, and contains the findings from the NAO's audit of the NHS summarized accounts and the Audit Commission's appointed auditors' work on the 2003-04 accounts of individual NHS organizations. The report outlines the financial issues facing individual NHS organizations, with an overview of the effects of these issues at national level and how this will affect the national health economy. In 2003-04 the NHS spent a total of £63 billion, with expenditure costs in the NHS rising by 7.3 per cent each year. This will increase the costs to £76 billion for the 2005-06, and £93 billion for 2007-08 periods. Alongside this increasing expenditure, the Government has set out various reform plans, including the establishment of the NHS Foundation Trusts, new staffing contracts, the development of the information technology infrastructure, and the way hospitals are funded. In the Summary of the financial performance for 2003-04 period, the number of bodies failing to achieve a financial balance had increased, along with an increase in the number of bodies with significant financial deficits. In all, 106 NHS bodies failed to achieve an in-year financial balance, and 14% of the Primary Care Trusts failed to keep expenditure within their resource limit, also a small number of NHS bodies are struggling to manage large deficits. The report advocates four key themes for the improvement of financial management: the role of the Board - who should display better oversight and improve their financial acumen; forecasting - NHS bodies should continually test whether cost savings programmes are realistic, and take account more effectively for risk factors in their financial planning, as well as set realistic budgets at the beginning of the year; earlier preparation of accounts - improvements in financial reporting, and the provision of financial information throughout the year should closely reflect the standard and range of information required in the annual accounts; transparency - that boards, managers, stakeholders would benefit from clarity in the way the accounts are organized, and that the amount of financial support received by the trusts should be clearly stated. With the introduction of Payment by Results and the use of independent healthcare providers the income received by NHS Trusts is no longer certain. So overall improvements in their financial forecasting and modeling, with NHS Trusts in particular developing their commercial financial skills, would be beneficial especially if they intend to become foundation trusts.
