In 1971, Volume 1 of this series was published while I was Head of the Division of Myocardial Biology in the Department of Pharmacology at Baylor College of Medicine in Houston, Texas. I dedicated that first volume to Sir Henry Hallet Dale, who died on July 23, 1968. In the Preface I pointed out that many of the pharmacological advancements that occurred during the last century were direct descendants from the classic paper published in 1910 by Professor Dale and his colleague, Dr. Barger. In this paper, the concept of "specific receptor sites" was introduced by the statement that "the relationship of the recepted mechanism to the base [i. e. , drug base] may well be one of solid solution of adsorption and, therefore, more analogous to that of an enzyme to its substrate . . . . " I also pointed out at that time that the search for drug receptors continues and that fundamental knowledge of the nature of receptors and drug-receptor interaction will eventually lead to a rational approach to drug design. Since 1971, the study of receptors and their interaction with specific chemical substances has continued at an accelerated pace and this is due, in particular, to the introduction of new and exciting methodologies. The death last year of Professor Raymond P. Ahl quist, who pioneered the introduction of specific adrenergic receptors, represents the close of yet another era.

Book excerpt: Contractility describes the relative ability of the heart to eject a stroke volume (SV) at a given prevailing afterload (arterial pressure) and preload (end-diastolic volume; EDV). Various measures of contractility are related to the fraction as the SV/EDV or the ejection fraction, and the dynamics of ejection as determined from maximum pressure rise in the ventricles or arteries or from aortic flow velocities determined by echocardiography. At the cellular level, the ultimate determinant of contractility is the relative tension generation and shortening capability of the molecular motors (myosin cross-bridges) of the sarcomeres as determined by the rates and extent of Ca activation, the turnover kinetics of the cross-bridges, and the relative Ca responsiveness of the sarcomeres. Engagement of the regulatory signaling cascades controlling contractility occurs with occupancy and signal transduction by receptors for neurohumors of the autonomic nervous system as well as growth and stress signaling pathways. Contractility is also determined by the prevailing conditions of pH, temperature, and redox state. Short-term control of contractility is fully expressed during exercise. In long-term responses to stresses on the heart, contractility is modified by cellular remodeling and altered signaling that may compensate for a time but which ultimately may fail, leading to disorders.

Stress composes the foundation of many degenerative heart diseases including atherosclerosis, spasm, thrombosis, cardiomyopathy, and congestive heart failure. Based on the concept that excessive stress may play a crucial role in the pathogenesis of ischemic heart disease, attempts were made to design methods for preventing of myocardial injury. Creation of stress reactions by repeated ischemia and reperfusion or subjecting the hearts to heat or oxidative stress enables them to meet the future stress challenge. Repeated stress exposures adapt the heart to withstand more severe stress reactions probably by upregulating the cellular defense and direct accumulation of intracellular mediators, which presumably constitute the material basis of increased adaptation to stress. Thus, the powerful cardioprotective effect of adaptation is likely to originate at the cellular and molecular levels that compose fundamental processes in the prophylaxis of such diseases.
