Book excerpt: Magnetic resonance imaging (MRI) is now considered the imaging modality of choice for the majority of disorders affecting the central nervous system. This is particularly true for gray and white matter disorders, thanks to the superb soft tis sue contrast in MRI which allows gray matter, unmyelinated, and myelinated white matter to be distinguished and their respective disorders identified. The pre sent book is devoted to the disorders of myelin and myelination. A growing amount of detailed in vivo information about myelin, myelination, and myelin dis orders has been derived both from MRI and from MR spectroscopy (MRS). This prompted us to review the clinical, laboratory, biochemical, and pathological data on this subject in order to integrate all available information and to provide im proved insights into normal and disordered myelin and myelination. We will show how the synthesis of all available information contributes to the interpretation of MR images. After a brief historical review about the increasing knowledge on myelin and my elin disorders, we propose a new classification of myelin disorders based on the subcellular localization of the enzymatic defects as far as the inborn errors of me tabolism are concerned. This classification serves as a guide throughout the book. All items of the classification will be discussed and, whenever relevant and possi ble, be illustrated by MR images.

The symposium was a part of the 22nd Canadian Congress of Neurological Sciences meeting and was sponsored by funds from the Multiple Sclerosis Society of Canada and the Medical Research Council of Canada. The presentations covered five major topics: basic neurobiology, molecular biology, the role of viruses in demyelination, immune function and dysfunction in multiple sclerosis, and clinical magnetic resonance imaging studies. It was heartening to note that scientists from several different disciplines were working towards a common end-point: the understanding and treatment of multiple sclerosis. In this book, speakers at the symposium have each presented a chapter of their findings and discussions. In addition, some non-participants at the symposium have been invited to submit chapters in order to give this volume a more complete scope. It is hoped that the reader will find this book a useful reference for several subjects of interest to multiple sclerosis. Book excerpt: My colleagues and I have been gratified by how rapidly the first edition of Myelin has been aeeepted as a standard referenee work by myelin researehers. This is undoubtedly the primary faetor aecounting for the high rate of reeidivism among the authors with respeet to preparation of a seeond edition; eleven of the original twelve contributors were again involved. Four new authors (Wendy Cammer, Marjorie Lees, Ute Traugott, and Seymour Greenfield) have also eontributed to the present volume. This new edition retains many aspects of the format of the original, even including use of the same chapter headings. Thus, comments in the preface to the first edition concerning the level at which material is presented and the relationships between chapters covering the "basic" and "c1inical" material are still applicable. Despite certain similarities in overall organization, comparison of the present edition with the original demonstrates that our eHorts to restriet expansion in the length of the text were not notably successful; the new edition is mueh lengthier than the original. Nevertheless, the increase in text length is modest relative to the increase in information in the seven years separating the preparation of the two editions. Most of the material in the individual chapters has had to be completely redone in order to include this new information. For example, our view of the metabolism of myelin increasingly must take cognizance of a very rapidly turning over pool of certain of its components.

Myelin is a nervous tissue structure that is most suitable as a subject of biochemical investigation. It is easy to isolate in a highly purified form, is rather stable even after death, and is af fected by a variety of neurological and other diseases. Its lesions are particularly important in the study of multiple sclerosis, a disease which is relatively prevalent in Finland and has therefore been of interest to a great many Finnish scientists. The first half of this book is concerned with the biochemical composition and molecular organization of myelin, the second half with the experimental and clinical aspects of demyelination. The comments given after each of the presentations at the symposium were not recorded as such, but each author was requested to modify his or her paper accordingly. Book excerpt: The fundamental roles of Schwann cells during peripheral nerve formation and regeneration have been recognized for more than 100 years, but the cellular and molecular mechanisms that integrate Schwann cell and axonal functions continue to be elucidated. Derived from the embryonic neural crest, Schwann cells differentiate into myelinating cells or bundle multiple unmyelinated axons into Remak fibers. Axons dictate which differentiation path Schwann cells follow, and recent studies have established that axonal neuregulin1 signaling via ErbB2/B3 receptors on Schwann cells is essential for Schwann cell myelination. Extracellular matrix production and interactions mediated by specific integrin and dystroglycan complexes are also critical requisites for Schwann cell–axon interactions. Myelination entails expansion and specialization of the Schwann cell plasma membrane over millimeter distances. Many of the myelin-specific proteins have been identified, and transgenic manipulation of myelin genes have provided novel insights into myelin protein function, including maintenance of axonal integrity and survival. Cellular events that facilitate myelination, including microtubule-based protein and mRNA targeting, and actin based locomotion, have also begun to be understood. Arguably, the most remarkable facet of Schwann cell biology, however, is their vigorous response to axonal damage. Degradation of myelin, dedifferentiation, division, production of axonotrophic factors, and remyelination all underpin the substantial regenerative capacity of the Schwann cells and peripheral nerves. Many of these properties are not shared by CNS fibers, which are myelinated by oligodendrocytes. Dissecting the molecular mechanisms responsible for the complex biology of Schwann cells continues to have practical benefits in identifying novel therapeutic targets not only for Schwann cell-specific diseases but other disorders in which axons degenerate.

Current research on oligodendrocyte biology, the pathology of MS, imaging of lesions and the biology of remyelination are paving the way toward opening this new translational frontier. Professor Duncan and Professor Franklin have assembled a broad group of experts in the fields of glial cell biology, neuropathology, radiology and clinical neurology to provide the background toward taking remyelination from experimented models into MS patients.
