Until now, the efforts of Japanese surgical oncologists in multimodality therapy have not been fully understood because they have not been adequately reported in English. This volume now presents the experience and expertise of Japanese surgeons who utilize preoperative or intraoperative chemotherapy, or a combination of systemic and regional chemotherapy. Their collected work provides valuable insight in the Japanese perspective and success in treating gastric cancer. Included in the appendix is an extensive tabular analysis of the gastric cancer database at the Cancer Institute Hospital of Tokyo. Probably the largest such database in the world from a single institution, the analyses present the state of the art in treatment of gastric cancer.

While, historically, surgical resection has been the sole treatment for these tumors, multimodal therapies have meanwhile proven their efficacy. At present, pre- and postoperative chemotherapy and radiotherapy, targeted drug therapy, and stage-specific surgical approaches are all indispensable cornerstones of an individualized treatment for upper gastrointestinal malignancies. With such multimodal treatment, better outcomes comprising improved quality of life and prolonged survival have been achieved for patients. However, for many tumor entities and stages, the ideal combination and sequence of treatments is still being evaluated in clinical trials. Moreover, the value of novel approaches such as immunotherapy or robotic surgery remains a matter of research. In this Special Issue of Cancers, up-to-date original research, short communications, and comprehensive review articles on all modalities playing a role in the treatment of upper gastrointestinal malignancies have been published.

In recent years numerous therapeutic approaches of pre-, peri-, intra-and postoperative treatment have been in vestigated with regard to their use in combination with surgi cal intervention. It now is time to analyze and to define the state of our knowledge. For tumors of the gastrointestinal tract there are several encouraging therapeutic approaches, such as preop erative chemotherapy in esophageal and perioperative chemotherapy in colon cancer. For some special tumors, like anal carcinoma, we have clearly defined combined therapies which even now must be viewed as standard treatment. It is also time to demonstrate the results of several clini cal studies that have been conducted within the last few years that combined surgical efforts with pre- or postinter ventional chemotherapy or radiotherapy. It is necessary to evaluate whether these trials contribute to progress in onco logical therapy. The editors of this volume - surgeons at the university hospitals of Heidelberg and Zurich - must be given the mer it of achieving these goals. It was especially appropriate for the Department of Surgery in Heidelberg, in close coopera tion with the Comprehensive Cancer Center Heidelberg/ Mannheim, to prepare a review of our present knowledge of surgical oncology as it is in the tradition of attempting to combine different therapeutic approaches to cancer therapy.

The main aim of the text is to propose a comprehensive, state-of-the art review of this field, and will serve as a valuable resource for oncologists, pathologists, surgeons and researchers with an interest in gastric cancer. The first chapters will review the interplay between pathogens and especially H. pylori, inflammation, genomic instability, and tumorigenesis. Secondly, an exhaustive overview of the diagnosis of gastric cancer from a clinical/endoscopic and pathologic perspectives will be illustrated. Two specific chapters will deal with tissue and serum biomarkers with an important role in diagnosis as well as prognostic and predictive markers of response to specific therapy. A central section of the book will focus on treatment options available today for patients. Surgery is the cornerstone of treatment for gastric cancer; however, the prognosis of patients with locally advanced disease is still poor even after curative resection. Therefore, in order to improve the prognosis of patients, especially with advanced stage, a multimodal strategy is advisable and is thoroughly addressed in the appropriate section. With the advent of next-generation sequencing and new preclinical model technologies, the genomic landscape of gastric cancer has been delineated and molecular characterizations finalized to novel therapeutic targets of each molecular subtype have been identified. These advances are making it feasible to integrate clinical and phenotype-based diagnostic and therapeutic methods and are described in a dedicated section. The text will conclude with chapters focusing on the future direction of gastric cancer research in the precision medicine era: an in-depth analysis of non-coding RNAs and their role in gastric cancer, the impact of immunomodulation/immunotherapy and the perspectives of nano-medicine will be finally proposed to the audience. A brief review of the existing literature addressing the above mentioned topics will be present in each chapter. This text will serve as a useful resource for physicians and researchers dealing with, and interested in, this challenging malignancy. It will provide a concise yet comprehensive summary of the current status of the field that will help guide patient management and stimulate investigative efforts. All chapters will be written by experts in their fields and will include the most up to date scientific and clinical information.

Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.
