OBJECTIF: Déterminer l'efficacité de la réalité virtuelle,comparativement à la kinésithérapie conventionnelle, sur la récupération motrice du membre supérieur. METHODOLOGIE DE RECHERCHE : Utilisation de bases de données PubMed, PEDro et Cochrane. CRITERE D'INCLUSION : Essais cliniques randomisés comparant la réalité virtuelle avec la kinésithérapie conventionnelle, utilisées chez des adultes en phase chronique d'accident vasculaire cérébral. CRITERE DE JUGEMENT : Récupération de la fonction motrice du membre supérieur, évalué par Fugl-Meyer-Upper-Extremity-Scale et Box & Block test. RESULTATS ET ANALYSE : Etude de 6 essais cliniques randomisés de faible puissance : les biais ont été répertoriés et doivent être pris en compte dans l'analyse des résultats. Les résultats ont montré une tendance en faveur de la réalité virtuelle pour ce qui est de l'amélioration de la motricité globale du membre supérieur, et une tendance en faveur de la kinésithérapie conventionnelle pour celle de la dextérité manuelle. Mais aucune différence d'efficacité entre les deux traitements ne s'est montrée significative et avérée. DISCUSSION : La nature ludique de ces systèmes de réalité virtuelle ainsi que le facteur temps sont deux critères à ne pas négliger, et justifient la nécessité de la continuité de la recherche scientifique, afin de préciser la différence d'efficacité avec la kinésithérapie conventionnelle sur l'amélioration de la fonction motrice du membre supérieur.

Il touche 16 millions de personnes à travers le monde et représente, en France, la première cause de handicap acquis chez l'adulte. La limitation du membre supérieur est l'atteinte la plus commune après un AVC. La pratique mentale (PM) est une technique de rééducation cognitive basée sur la répétions imaginée de tâches afind'améliorer leur exécution. Objectif : L'objectif de cette revue est de déterminer si la pratique mentale associée à une rééducation conventionnelle pourrait améliorer la motricité du membre supérieur chez les sujets post-AVC en phase subaiguë. Méthodologie de recherche : La recherche des études s'est faite sur des bases de données (PubMed, Cochrane, PEDro, Research Gate et Science Direct) et à travers les références d'autres études. Les études éligibles devaient traiter de l'efficacité de la pratique mentale sur le membre supérieur des patients en phase subaiguë, déterminée grâces aux échelles ARAT et FMA-UE. La qualité méthodologue et les biais ont été évalués avec l'échelle PEDro. Résultats et analyses : Parmi les 367 études identifiées, 5 essais cliniques randomisés (PEDro>6) ont été incluses pour une analyse qualitative. 4 études montrent une amélioration des résultats dans les groupes PM, mais elle n'est significative que pour 2 (>DMCI). Enfin,une étude ne montre pas d'amélioration par rapport au groupe contrôle. Discussion: Cette revue ne permet pas d'établir de réel bénéfice quant à l'utilisation de la PM, associée à un autre traitement, en phase subaiguë. Cet outil peut être mis en place avec des patients sans troubles cognitifs majeurs afin de stimuler la plasticité cérébrale. Il semblerait que l'amélioration de la motricité n'intervienne qu'avec des séances courtes et répétées sur des mouvements globaux simples. D'autres études sont nécessaires pour évaluer l'effet de la PM sur le long terme et sur des protocoles clairs et adaptés aux capacités du patient (temps, tâches, répétition...).

Book excerpt: Contexte : l'AVC constitue la 1ère cause de handicap acquis chez l'adulte, la 2ème cause de démence et la 2ème cause de mortalité en France. Sa fréquence, sa gravité et son coût en font un véritable enjeu de santé publique. A la suite d'un AVC, le patient hémiplégique développe généralement une réaction dite de « non-utilisation acquise » du membre parétique, ayant pour conséquence d'entraver fortement sa capacité de récupération. Afin que le patient soit contraint d'utiliser son membre lésé, la thérapie par contrainte induite a été mise en place, mais à ce jour son efficacité n'a été prouvée qu'à la phase chronique de l'AVC. Objectif : l'objectif de cette revue de littérature est de déterminer si la thérapie par contrainte induite permet d'améliorer la récupération motrice du membre supérieur parétique à la phase subaiguë d'un AVC. Méthode : les bases de données PubMed, PEDro, Cochrane Library et Kinedoc ont été investiguées. Seuls les ECR strictement conformes aux critères d'éligibilité précédemment définis ont été inclus. La qualité méthodologique des études sélectionnées fut évaluée grâce à l'échelle PEDro. Une fois l'extraction des données effectuée, les résultats ont été analysés de manière qualitative/narrative. Résultats : un total de 4 études de 316 participants, jugées de moyenne à faible qualité méthodologique ont été incluses. D'après les résultats obtenus il semblerait que cette thérapie ait un bénéfice sur la récupération de la fonction motrice du membre hémiplégique, cependant, bien que les changements dans le groupe témoin aient été sensiblement plus faibles que le groupe expérimental, les participants au groupe contrôle ont également montré une amélioration significative de la plupart des résultats aux différentes périodes de suivi. Parmi l'ensemble des outils de mesure, seuls les résultats à l'échelle MAS furent en défaveur du groupe expérimental. Discussion : l'évaluation de la qualité des preuves a démontré que ces résultats étaient généralement soit statistiquement non significatifs, soit cliniquement non pertinents, de plus les études présentent un risque élevé de biais pouvant fausser les résultats. Les données actuelles de la science ne présentent donc pas assez de preuves pour conclure sur l'efficacité de la thérapie par contrainte induite à la phase subaiguë d'un AVC.

Book excerpt: Introduction : Les Accidents vasculaires cérébraux (AVC) constituent la première cause de handicap de l'adulte dans les pays développés, principalement par les déficits moteurs qu'ils engendrent. La récupération d'une fonctionnalité motrice du membre supérieur est souvent rare lorsqu'il existe une paralysie initiale complète. Cette faible efficacité des programmes actuels de rééducation doit conduire au développement de nouvelles approches rééducatives plus performantes. L'objet de cette étude est l'évaluation thérapeutique d'une technique de rééducation visuomotrice du membre supérieur à la phase sub-aiguë de l'AVC. Matériel et méthode : Nous avons réalisé une étude monocentrique en simple aveugle, randomisée et contrôlée comprenant un groupe thérapeutique de 7 patients et un groupe contrôle de 8 patients. Les sujets inclus présentaient un déficit moteur hémicorporel initialement complet, en rapport avec un premier AVC ischémique. Ils ont suivi un programme de rééducation standard auquel s'ajoutait un programme de 30 séances de rééducation visuomotrice. Pour le groupe thérapeutique, une rééducation visuomotrice comportant une correction du rétrocontrôle visuel a été appliquée, tandis que le groupe contrôle a reçu une rééducation sans modification du rétrocontrôle visuel. Les 2 groupes ne se différenciaient donc que par le type de rétrocontrôle visuel. La rééducation s'est déroulée sur six semaines. Les patients ont été évalués par le bilan 400 points, le Minnesota Test, le Perdue Pegboard Test et le Box and Block Test à l'inclusion (1 mois post AVC), en fin de rééducation (environ 3 mois post AVC), et au sixième mois post AVC. Résultats : Le principal résultat est une différence significative (p=0.0372) en faveur du groupe thérapeutique pour l'évolution du bilan 400 points (capacité globale) entre le début et la fin de la rééducation. Les autres tests ne montrent pas de différences significatives malgré une évolution moyenne en faveur du groupe thérapeutique. Les éventuels facteurs confondants (l'âge, le sexe, le côté atteint, la localisation lésionnelle) ne diffèrent pas significativement selon le groupe. Conclusion : Dans la population étudiée, la rééducation visuomotrice avec correction du rétrocontrôle visuel lors du mouvement de préhension améliore significativement le pronostic fonctionnel du membre supérieur après un AVC. Ces premiers résultats positifs sur un faible effectif doivent encore être validés par une étude multicentrique portant sur une plus large population.
