Malgré une remise en cause récente de sa balance bénéfice/risque, le dépistage mammographique systématique semble être le seul moyen d’en diminuer la mortalité. La France a donc généralisé depuis 2004 un dépistage organisé (DO) pour lequel, tous les 2 ans, les femmes de 50 à 74 ans sont invitées à bénéficier d’une mammographie. Le taux de participation dans le Val-de-Marne était de 45,3% en 2007-2008, en dessous de l’objectif recommandé pour obtenir une baisse de la mortalité (70%). le but de notre enquête était de connaitre dans le Val-de-Marne les raisons de la non participation des femmes et d’évaluer leur rapport au DO. Pour cela, un questionnaire a été envoyé à 7000 d’entre elles. 468 réponses exploitables ont été reçues (7%). Les femmes répondantes étaient plutôt jeunes, actives, de bon niveau socio-économique et suivies sur le plan médical. Elles avaient une bonne connaissance des modalités du DO par les médias (77%) mais peu y avaient été orientées par un médecin (20,6%). Les motifs principaux de non participation étaient: la réalisation d’un dépistage individuel (DI) (39,4%), l’oubli (11,4%), une pathologie nécessitant un suivi particulier (6,1%), le fait que le dépistage n’était pas prioritaire (6,1%), une défiance vis-à-vis du DO (5,2%), la peur de la découverte d’un cancer (4,4%). quatre typologies de femmes non répondeuses au DO ont été déduites des réponses. Le principal obstacle au DO semble être dans notre étude la réalisation du DI ; le conseil du médecin assurant le suivi régulier est un facteur prépondérant pour orienter vers le DO. Afin d’atteindre l’objectif de 70% de DO, le généraliste et le gynécologue devraient être réinvestis dans le dispositif actuellement tourné vers les femmes et les radiologues.

Critères de non-inclusion : articles non disponibles en texte entier, dépistage organisé des cancers en France sans différenciation. Résultats : Parmi les 222 références trouvées, 23 ont été retenues. De nouveaux freins à la participation au dépistage organisé du cancer du sein en France sont apparus autour de la peur du surdiagnostic et du surtraitement. Il n'existe pas d'étude nationale étudiant ces freins. L'HAS recommande une modification de l'organisation du dépistage organisé. Conclusion : La présence de freins liée à la controverse du dépistage organisé du cancer du sein entraîne la nécessité pour le médecin généraliste d'adapter son discours malgré sa posture d'incertitude afin de délivrer une information honnête et éclairée.

Nous avons réalisé 3 focus groups constitués de 4 à 6 femmes recrutées à partir du travail de Marina Brodbeck. Les femmes attachent une grande attention à leur santé et à un dépistage personnalisé. Pour elles, le sein est surtout l’organe de l’allaitement. Elles ont peur du cancer et plus spécifiquement de localisation gynécologique. La mammographie est bien connotée et acceptée. Elles ne connaissent pas parfaitement les modalités du DOCS : la double lecture est souvent ignorée, le courrier d’invitation leur semble peu explicite, elles expriment des doutes ou craintes concernant la fiabilité des centres ou des appareils de mammographies, et de la peur d’un changement de centre lors du passage au DOCS. La discordance entre la date de réception du courrier et la réalisation de leur dernière mammographie, le caractère généralisé non individuel constituent des freins importants au DOCS. Ces éléments pourraient conduire à des actions permettant d’améliorer la participation au DOCS notamment en y impliquant plus explicitement les médecins cliniciens.

Les femmes sont partagées sur l’importance donnée à leur santé et ont tendance à s’adapter à leur état quel qu’il soit. Pour elles, le sein est l’organe de l’allaitement et de la séduction. Une dualité vis-à-vis du cancer partage les femmes entre celles qui en ont très peur et celles qui ne se sentent pas concernées. La mammographie est considérée comme un examen agressif. Les modalités du DOCS sont assez bien connues ; ses avantages résident dans la qualité des procédures et sa gratuité, son inconvénient principal dans le caractère non personnalisé. Les femmes réagissent souvent négativement au relais médiatique des messages et aux coûts induits. Ces éléments pourraient conduire à des actions permettant d’améliorer la participation au DOCS ; la diversité des freins à la participation nécessiterait un relais de messages individualisés par des médecins traitants impliqués plus explicitement dans le dépistage.

The change is powered by what Topol calls medicine's "Gutenberg moment." Much as the printing press took learning out of the hands of a priestly class, the mobile internet is doing the same for medicine, giving us unprecedented control over our healthcare. With smartphones in hand, we are no longer beholden to an impersonal and paternalistic system in which "doctor knows best." Medicine has been digitized, Topol argues; now it will be democratized. Computers will replace physicians for many diagnostic tasks, citizen science will give rise to citizen medicine, and enormous data sets will give us new means to attack conditions that have long been incurable.
