Book excerpt: Introduction : La fracture de hanche chez les personnes âgées entraîne des conséquences importantes en termes de morbi-mortalité. En Septembre 2015, à l'initiative des gériatres et après accord des orthopédistes, anesthésistes, urgentistes et de l'Institution, des lits d'Orthogériatrie ont été créés au sein du Pôle de Gériatrie du CHU de Poitiers avec pour souhait d'y admettre les patients fracturés avec un profil gériatrique, c'est-à-dire polypathologiques. L'objectif de notre étude est une évaluation de la mortalité à 3 mois d'une chirurgie d'une fracture de hanche des patients de plus de 75 ans après hospitalisation en Orthogériatrie ou en Orthopédie-Traumatologie. Méthode : Il s'agit d'une étude monocentrique observationnelle avec évaluation prospective de la morbi-mortalité à 3 mois et analyse de données rétrospectives des séjours enregistrés entre le 15 Septembre 2015 et le 30 Avril 2016. Tous les patients de plus de 75 ans se présentant aux urgences du CHU pour fracture de hanche avec décision d'un traitement chirurgical ont été inclus. Le critère de jugement principal était la mortalité à 3 mois. Les critères de jugement secondaires correspondaient au délai de prise en charge chirurgicale après admission aux Urgences, à l'évaluation du profil clinique du patient (score de comorbidités de Charlson, nombre de traitements chroniques), au changement du lieu de vie à 3 mois et à l'existence d'une nouvelle fracture ou d'une récidive d'hospitalisation sur un suivi de 3 mois. Résultats : Au total, 152 patients ont été inclus, 76 patients hospitalisés en Orthogériatrie et 76 en Orthopédie-Traumatologie. Les 2 groupes n'étaient pas différents à l'inclusion en termes de sex-ratio (avec une majorité de femmes), de moyenne d'âge 87,38±6,12 ans et 88,37±5,75 ans respectivement, ni de lieu de vie. Sur l'ensemble des patients, la mortalité à 3 mois était de 13,82%, sans différence significative entre les 2 groupes (15,79% en Orthopédie-Traumatologie et 11,84% en Orthogériatrie, p=0,48). Le délai de prise en charge chirurgicale était de 0,85 jours pour les patients admis en Orthogériatrie et de 2,01 jours pour ceux admis en Orthopédie-Traumatologie (p=0,009), avec un total de 13,16% des patients opérés à plus de 48h de leur arrivée aux urgences. Il n'y avait pas de différence significative concernant l'évolution du lieu de vie (p=0,09), l'existence d'une nouvelle fracture (p=0,74) ou le recours à une nouvelle hospitalisation au cours du suivi ( 23,53% en Orthogériatrie et 28,99% en Orthopédie-Traumatologie, p=0,47). Conclusion : Notre étude montre que le taux de mortalité à 3 mois est identique dans les 2 groupes. En revanche, le profil des patients admis en Orthogériatrie n'est pas plus morbide que celui des patients admis en Orthopédie-Traumatologie. Le délai de prise en charge chirurgicale est précoce au CHU de Poitiers, avec un délai inférieur aux chiffres nationaux, probablement favorisé par la dynamique de cette filière orthogériatrique.

L'intérêt de l'analgésie locorégionale (ALR) dans sa prise en charge périopératoire n'a pas été clairement évalué. L'objectif de ce travail était d'évaluer la morbi-mortalité post-opératoire des patients de plus de 65 ans opérés pour une FH, en comparant les patients ayant bénéficié d'une ALR (groupe ALR) versus sans ALR (groupe contrôle). Matériel et Méthodes : Il s'agissait d'une étude rétrospective, monocentrique incluant les patients de plus de 65 ans opérés d'une FH de décembre 2011 à avril 2013. La réalisation de l'ALR était laissée au libre choix du MAR. Le paramètre principal était le nombre de complications postopératoires (cardiologiques, respiratoires, neurologiques et les décès en cours d'hospitalisation). Une analyse multivariée a été réalisée à la recherche de facteurs aggravants. Les variables qualitatives étaient analysées par test de Chi2 ou par test exact de Fisher, quantitatives par test z ou Mann and Whitney selon conditions. Résultats : 98 patients inclus: 34 (groupe ALR) et 64 (groupe contrôle). Les ALR réalisées étaient des blocs interscaléniques. Le taux de complications post-opératoires était significativement inférieur dans le groupe ALR vs groupe contrôle: 5,9% vs 25% (p=0,027). Une majorité des complications était la survenue de troubles confusionnels (39%). Après analyses uni et multivariées, la survenue des complications étaient liées au score ASA à 3 : OR : 7,11 (p=0,009). Le sexe féminin était un facteur protecteur : OR à 0,15 (p-0,027). L'ALR tendait à diminuer le risque de complications : OR 0,2 (13=0,057). Conclusion : Dans ce travail monocentrique, rétrospectif, la réalisation d'une ALR dans la prise en charge péri-opératoire d'un patient opéré d'une FH semble diminuer la survenue de complications post-opératoires notamment en limitant la survenue des troubles cognitifs. Un travail prospectif et randomisé permettrait de confirmer ou non ce résultat.
