In Minimally Invasive Surgery (MIS), tools go through narrow openings and manipulate soft organs that can move, deform, or change stiffness. There are limitations on modern laparoscopic and robot-assisted surgical systems due to restricted access through Trocar ports, lack of haptic feedback, and difficulties with rigid robot tools operating inside a confined space filled with organs. Also, many control algorithms suffer from stability problems in the presence of unexpected conditions. Yet biological "manipulators", like the octopus arm can manipulate objects while controlling the stiffness of selected body parts and being inherently compliant when interacting with objects. STIFF-FLOP robot is an innovative soft robotic arm that can squeeze through a standard MIS, reconfigure itself and stiffen by hydrostatic actuation to perform compliant force control tasks while facing unexpected situations. One of these areas which is slowly gaining popularity among the users group is the notion of service robotics. This book is an in vestigation and exploration of engineering principles in the design and development of mechanisms and robotic devices that can be used in the field of surgery. Specifically the results of this book can be used for designing tools for class of Minimally Invasive Surgery (MIS). Generally, Minimal Invasive Surgery (MIS), e. g. laparoscopic surgery, is performed by using long surgical tools, that are inserted through small incisions at the ports of entry to the body (e. g. abdominal wall) for reaching the surgical site. The main drawback of current designs of en doscopic tools is that they are not able to extend all the movements and sensory capabilities of the surgeon's hand to the surgical site. By im proving surgical procedures, training, and more practice, it is possible for surgeons to reduce completion time for each task and increase their level of skill. However, even in the best cases the level of performance of a surgeon in Minimally Invasive Surgery is still a fraction of the con ventional surgery. Any dramatically improvement is usually driven by introduction of new tools or systems that in turn bring totally new pro cedures and set of skills.

Book excerpt: This thesis lays the groundwork for the automatic supervision of the laser incision process, which aims to complement surgeons’ perception of the state of tissues and enhance their control over laser incisions. The research problem is formulated as the estimation of variables that are representative of the state of tissues during laser cutting. Prior research in this area leveraged numerical computation methods that bear a high computational cost and are not straightforward to use in a surgical setting. This book proposes a novel solution to this problem, using models inspired by the ability of experienced surgeons to perform precise and clean laser cutting. It shows that these new models, which were extracted from experimental data using statistical learning techniques, are straightforward to use in a surgical setup, allowing greater precision in laser-based surgical procedures.

Book excerpt: Cette thèse propose et analyse un nouveau robot parallèle cinématiquement redondant (KRP) avec tous les actionneurs situés à la base. La structure de base du robot provient de travaux antérieurs, c'est-à-dire que trois jambes avec 3 degrés de liberté (ddls) chacune sont attachées à la plate-forme mobile par trois liaisons redondantes, ce qui donne un robot KRP avec (6+3) degrés de liberté. Cette thèse se concentre sur quatre thèmes. Tout d'abord, un nouveau robot parallèle à 3 ddls utilisé comme jambe du robot KRP est proposé, et tous les actionneurs sont situés à la base afin de réduire l'inertie des parties mobiles. De plus, les actionneurs sont placés près les uns des autres pour minimiser l'encombrement et produire un espace de travail relativement grand. Les problèmes cinématiques tels que la modélisation cinématique, le problème inverse/direct, l'analyse des singularités et l'espace de travail sont analysés en détail. En outre, pour simplifier l'analyse de l'espace de travail et agrandir l'espace de travail, une conception améliorée du robot à 3 ddls est proposée. L'établissement de modèles dynamiques pour les robots parallèles est un défi en raison des relations complexes entre les vitesses et les forces induites par leur structure en boucle fermée. Cette thèse propose plusieurs méthodes de modélisation simplifiées pour les robots parallèles. Le premier type est dérivé de l'approche lagrangienne et l'idée centrale est de réduire la complexité des expressions représentant l'énergie pour le robot. Par conséquent, ces méthodes ont une bonne généralité et sont disponibles pour divers robots parallèles. Cependant, pour certains robots parallèles (par exemple, le robot à 3 ddls proposé), même en utilisant la méthode simplifiée, les calculs d'énergie sont légèrement complexes. Compte tenu de cela, une autre méthode de modélisation simplifiée basée sur l'approche de Newton-Euler est proposée et analysée. Étant donné que le nouveau robot KRP a une structure similaire à celle des robots KRP précédents, la plupart des méthodes analysées dans les travaux précédents peuvent être directement utilisées. Par conséquent, cette thèse ne résout que brièvement le problème inverse/direct, explique la singularité du point de vue de la force, donne le modèle dynamique et montre la structure et le modèle CAO du prototype du nouveau robot KRP. Enfin, cette thèse propose une méthode générale de contrôle de préhension par laquelle le robot KRP peut faire fonctionner à distance une pince pour générer les forces de préhension requises tout en déplaçant la plate-forme, toutes les actions étant pilotées par les mêmes actionneurs. Le modèle de contrôle de préhension se compose d'un modèle de contrôle du mouvement des jambes et d'un modèle de contrôle de la force des jambes, où la force de préhension peut être contrôlée indépendamment. En conséquence, le modèle de contrôle en force utilise un contrôleur en boucle ouverte, simple à mettre en œuvre et efficace.

Book excerpt: An important obstacle in Minimally Invasive Surgery (MIS) is the significant degradation of haptic feedback (sensation of touch) to the surgeon about surgical instrument's interaction with tissue. This monograph is concerned with devices and methods required for incorporating haptic feedback in master-slave robotic MIS systems. In terms of devices, novel mechanisms are designed including a surgical end-effector (slave) with full force sensing capabilities and a surgeon-robot interface (master) with full force feedback capabilities. Using the master-slave system, various haptic teleoperation control schemes are compared in terms of stability and performance, and passivity-based time delay compensation for haptic teleoperation over a long distance is investigated. The monograph also compares haptic feedback with visual feedback and with substitution for haptic feedback by other sensory cues in terms of surgical task performance.
