L’entraînement à haute intensité sur tapis roulant a démontré des bénéfices pour améliorer l’endurance à la marche chez les patients post-AVC. Cependant, les modalités de cette méthode d’entraînement ne sont pas clairement définies. Objectif L’objectif de ce travail est de déterminer quelles modalités dans le réentraînement à la marche à haute intensité permettent d’améliorer le plus efficacement l’endurance à la marche chez les patients post- AVC. Méthodologie Pour notre revue de la littérature, nous avons interrogé les bases de données PubMed, PEDro, CINAHL et The Cochrane Library de septembre 2017 à janvier 2018. Nous avons retenu les études utilisant l’entraînement à haute intensité comme intervention et le 6MWT pour évaluer l’endurance à la marche. Résultats Les sept études retenues démontrent qu’une durée d’intervention de six, 12 ou 26 semaines, trois ou cinq fois par semaine durant 25 à 40 minutes à une intensité entre 60 et 80% de la fréquence cardiaque de réserve, améliorent cliniquement et significativement l’endurance à la marche des patients post-AVC. Conclusion L’entraînement aérobie sur tapis de marche ou au sol à haute intensité semble applicable, bien toléré et sécuritaire chez les patients AVC en phase subaigüe et chronique. D’autres études sont nécessaires pour définir les modalités améliorant le plus efficacement l’endurance à la marche.

L'adhésion, la faisabilité, la sécurité et l'efficacité de ces programmes réalisés sur une courte période sont démontrées. En effet, l'entraînement pré-opératoire permet d'augmenter la consommation en oxygène au pic de l'exercice (VO2pic) en moyenne d'environ 3 ml.min-1.kg-1. Pourtant, en raison des difficultés d'accessibilité aux programmes de réhabilitation respiratoire et du risque de retard de la chirurgie, ces programmes restent insuffisamment développés. Dans ce contexte, ces programmes réalisés au domicile du patient pourraient être une alternative intéressante.Objectif : Nous avons réalisé deux études randomisées contrôlées qui avaient pour objectif d'évaluer l'adhésion à un programme d'entraînement pré-opératoire et de mesurer son effet sur la fonction pulmonaire, l'endurance des muscles respiratoires, la capacité maximale à l'exercice, la Distance de Marche parcourue en 6 minutes (D.M.6), la bioimpédancemétrie, la force maximale du quadriceps, la durée de séjour hospitalier et la morbi-mortalité. Méthodes : Sur une durée de 3 semaines et au domicile du patient, le Groupe Réentraîné (GR) réalisait soit un Entraînement pré-opératoire en Endurance des Muscles Respiratoires (E.E.M.R) (étude 1), soit un entraînement pré-opératoire intermittent à haute intensité (étude 2), surajouté à la kinésithérapie respiratoire standard. Le Groupe Contrôle ne réalisait que la kinésithérapie respiratoire standard. Les critères de jugement étaient l'adhésion, la fonction pulmonaire, l'endurance des muscles respiratoires, la capacité maximale à l'exercice, la D.M.6, la masse maigre, la force maximale du quadriceps, la durée de séjour hospitalier, les complications post-opératoires et la mortalité à 90 jours. Les évaluations étaient réalisées de manière similaire avant et après le programme alloué.Résultats : La faisabilité et l'adhésion sont bonnes pour des programmes d'entraînement pré-opératoire d'une durée courte. Nous confirmons la sécurité de ces programmes réalisés au domicile des patients. Dans l'étude 1 à la fin du test d'endurance en hyperventilation isocapnique, nous montrons une augmentation significative de la ventilation minute (+15±16 vs. -2±17 l.min-1, p = 0,004) et du temps endurant (+229±199 vs. -5±371 sec, p = 0,001) dans le GR. Dans l'étude 2, nous notons une augmentation significative de la pression expiratoire maximale (+19±23 vs. -6±24 %, p = 0,014) et de la puissance maximale aérobie (+12±11 vs. 0±10 W, p = 0,006) dans le GR. La VO2pic (+1,0±1,1 vs. 0,0±1,5 ml.min-1.kg-1, p = 0,061) tend à augmenter dans le GR. Egalement dans l'étude 1, le nombre de complications post-opératoires pulmonaires (2 vs. 10, p = 0,037) diminue significativement dans le GR.Conclusion : Ces programmes d'entraînement pré-opératoire de courte durée devraient être développés dans la pratique quotidienne, afin de permettre leur mise en place rapide par des professionnels expérimentés. En effet, ces programmes d'entraînement spécifiques pourraient permettre de pallier aux difficultés d'accessibilité et d'adhésion aux programmes de réhabilitation pré-opératoire ; et d'apporter des bénéfices significatifs sur la capacité maximale à l'exercice avant une chirurgie de résection pulmonaire pour cancer et sur la morbi-mortalité post-opératoire, comparativement à la kinésithérapie respiratoire standard.
