Au traitement conventionnel par multi-injections s’est ajouté au début des années 1980 le traitement par pompe à insuline externe, thérapeutique de plus en plus instaurée chez l’enfant de nos jours. Le principal objectif de ce travail était de connaître le contexte et la démarche d’initiation de ce traitement à l’Hôpital d’Enfants du CHU de Dijon. Secondairement, le but était d’évaluer ce dispositif par rapport au contrôle métabolique, à la survenue de complications ainsi qu’à la qualité de vie des jeunes patients après douze mois de traitement. Cette étude a porté sur 16 dossiers de suivi sélectionnés et sur plusieurs entretiens informels entre avril et juin 2011. Malgré quelques effets indésirables persistants (réactions cutanées, douleurs à la pose du cathéter,...), les résultats montrent une amélioration du contrôle métabolique (diminution du taux moyen d’HbA1c de 8.16% à 7.7%), un nombre moyen d’épisodes hypoglycémiques en baisse et une amélioration générale de la qualité de vie de l’enfant et de sa famille.

L'objectif principal de ce travail était de décrire la population des enfants diabétiques de type 1 chez qui l'indication d'une pompe à insuline est posée, ainsi que le contexte et les modalités de mise en route du traitement à l'Hôpital d'enfants du CHU de Nancy. Les objectifs secondaires étaient d'apprécier l'efficacité, la sécurité et la fiabilité de la technique en suivant les enfants pendant un an après la mise sous pompe. Enfin, un questionnaire de satisfaction a permis aux enfants d'évaluer l'impact du traitement par pompe dans leur vie quotidienne.Cette étude rétrospective porte sur les 50 premiers enfants diabétiques de type1(âge moyen de 8,8 ans) suivis et traités par pompe à insuline externe au CHU de Nancy de 2006 à 2010. L'indication principale de mise en route du traitement est un équilibre insuffisant du diabète (48%). Pour 30% des enfants, la pompe constitue l'insulinothérapie de première intention (âge moyen 3,19 ans). Durant l'année suivant l'instauration de la pompe, l'équilibre métabolique s'améliore pour l'ensemble de la population avec une relative stabilité de l'IMC, une réduction des hypoglycémies, une absence d'épisodes d'acidocétoses, mais surtout un bienêtre de l'enfant vis-à-vis de son traitement. Tous les enfants interrogés se déclarent satisfaits de la pompe. Il existe une amélioration de l'accès au traitement par pompe à insuline chez l'enfant qui permet de prendre davantage en compte les demandes de l'enfant et de ses parents. La pompe à insuline paraît fiable et efficace mais elle semble de plus offrir à l'enfant et à sa famille une qualité de vie nettement plus appréciable.
