Book excerpt: Although the United States Military medical forces have historically had patient care and medical research as primary responsibilities, they have recently been afforded the opportunity to contribute to our national security by participating in humanitarian or civic action programs or by conducting ventures which lead to training of friendly nations' medical services. The Army Medical Department (AMEDD) has been involved with formal, established programs beginning at the close of World War II, continuing through Korea and Vietnam, and extending to the low intensity conflict ongoing in Central America. In addition, it has participated in a number of ad hoc exercises in various other areas on the globe. It is my contention that, for the most part, substantive benefit was realized to some degree from all of these events. However, some were more effective than others in terms of promoting our national security strategy. This paper will review some military medical endeavors, comment about shortfalls as well as positive points, and offer recommendations for future success.

Beyond enhancing security, prosperity, and democracy, a vigorous international health policy, actively supported by a robust program of military medical research, will enhance U.S. global leadership. Giving higher priority to global health in foreign policy is good for the United States and good for the world.

Designed as they are for the information and instruction of student medical officers at the Army Medical School, they cannot profess to give more than a definitive outline of the history of a great subject. But with the aid of the footnote references, derived from the matchless resources of the Surgeon General's Library, it is conceivable that the senior officer may be in position to expand the theme indefinitely for purposes of lecturing, writing or otherwise. During the World War the United States Army, particularly its Medical Corps, had an opportunity to achieve great results on a grand scale such as had never been offered it before in all its history. One effect of this great expansion, this unique opportunity to think in large terms, was to dispense, for the time being at least, with certain obsolete or obsolescent conventions of the service which had tended to narrow the viewpoint of the individual and, in extreme cases, demonstrably to engender bitter hatred against the Army in certain quarters. The merest glance at these pages will convince any candid reader that the part played by medical personnel in the maintenance of military morale is of extraordinary moment. It was to forward this motif that this book was written, in the hope and belief that a glimpse of the "ampler aether" which is the history of his profession will convince the young medical officer that, in successful military operations of modern type, patriotism is the motor power, and military administration the mechanism by means of which great things are to be accomplished and victories won. The proximity of the front and the colossal numbers of wounded created greater public awareness of the impact of the war than had been seen in previous conflicts, with serious political consequences. Frequently referred to as 'our wounded', the central place of the soldier in society, as a symbol of the war's shifting meaning, drew contradictory responses of compassion, heroism, and censure. Wounds also stirred romantic and sexual responses. This volume reveals the paradoxical situation of the increasing political demand levied on citizen soldiers concurrent with the rise in medical humanitarianism and war-related charitable voluntarism. The physical gestures and poignant sounds of the suffering men reached across the classes, giving rise to convictions about patient rights, which at times conflicted with the military's pragmatism. Why, then, did patients represent military medicine, doctors and nurses in a negative light? This book listens to the voices of wounded soldiers, placing their personal experience of pain within the social, cultural, and political contexts of military medical institutions. The author reveals how the wounded and disabled found culturally creative ways to express their pain, negotiate power relations, manage systemic tensions, and enact forms of 'soft resistance' against the societal and military expectations of masculinity when confronted by men in pain. The lessons learned are then looked at in the context of recent medical humanitarian action in honduras.
