Book excerpt: Over the last quarter century or so, specialization within obstetrics and gynecology, and pediatrics has resulted in the development of the disciplines of maternal-fetal medicine and neonatology, respectively. A primary focus of maternal-fetal medicine has been to understand the mechanism(s) of premature delivery and develop treatment modalities for improving the length of gestation. A primary focus of neonatology has been to under stand the causes of respiratory distress in the neonate. Success has resulted, not only in the lengthening of gestation, but an improved understanding of the causes and treatment of neonatal respiratory disease. With increasing success has come the necessity to under stand the metabolic principles of the parturient, the fetal/placenta unit, and the neonate. These principles are clearly very important from multiple aspects. Increased understand ing of metabolism of the pregnant woman would explain the aberrations occurring in normal and abnormal pregnancy and improve nutritional support for the parturient. A prime example of altered metabolism is the parturient with diabetes. Understanding metabolism ofthe fetal/placenta unit is necessary to increase the probability that the fetus will be born appropriate for size irrespective of the gestational age. The various compo nents of neonatal metabolism are important, not only for understanding the changes in physiology and biochemistry occurring in the developing neonate, but the principles by which nutritional support should be provided.

Book excerpt: Provide the best care possible with expert insight and clinically relevant coverage of the physiologic changes that occur throughout all major periods of the perinatal experience — prenatal, intrapartum, postpartum, and neonatal. Maternal, Fetal & Neonatal Physiology: A Clinical Perspective, 4th Edition gives you a solid foundation for assessment and therapeutic interventions, featuring an emphasis on the evolving interrelationships between mother, fetus, and neonate and adaptations of preterm and term infants to the extrauterine environment. Solid coverage of the physiologic bases for assessment and therapeutic interventions make this an ideal resource for advanced practice. Synthesis of the latest research studies and evidence-based practice provides vital data on normal physiologic changes during the antepartum, intrapartum and postpartum periods; anatomic and functional development of the fetus; and developmental physiology of preterm and term neonates. Pharmacology tables offer quick access to key pharmacology information and drug effects with clinical examples. Coverage of pathophysiology and interventions for the pregnant woman, fetus, and newborn for selected abnormal events provides a solid understanding of physiologic adaptations and developmental physiology relating to major body systems and metabolic processes. Comprehensive tables, diagrams, and illustrations highlight important concepts and summarize key information Thoroughly updated content offers the very latest evidence-based information, contemporary research, and clinical developments related to maternal, fetal, and neonatal physiology. New coverage on the late pre-term infant provides the most current practice guidelines to promote quality care. Expanded discussions of reproductive processes reflect cutting-edge research and the clinical implications of physiologic and genetic effects brought to bear from both the female and the male. Extensive and reliable web sources allow for further study or checking for updated information. New NICHD standard definitions on fetal monitoring enable you to identify fetal heart rate patterns using standardized nomenclature.

Book excerpt: Children are the foundation of the United States, and supporting them is a key component of building a successful future. However, millions of children face health inequities that compromise their development, well-being, and long-term outcomes, despite substantial scientific evidence about how those adversities contribute to poor health. Advancements in neurobiological and socio-behavioral science show that critical biological systems develop in the prenatal through early childhood periods, and neurobiological development is extremely responsive to environmental influences during these stages. Consequently, social, economic, cultural, and environmental factors significantly affect a child's health ecosystem and ability to thrive throughout adulthood. Vibrant and Healthy Kids: Aligning Science, Practice, and Policy to Advance Health Equity builds upon and updates research from Communities in Action: Pathways to Health Equity (2017) and From Neurons to Neighborhoods: The Science of Early Childhood Development (2000). This report provides a brief overview of stressors that affect childhood development and health, a framework for applying current brain and development science to the real world, a roadmap for implementing tailored interventions, and recommendations about improving systems to better align with our understanding of the significant impact of health equity.

Available in PDF, EPUB and Kindle. Book excerpt: The increasing prevalence of preterm birth in the United States is a complex public health problem that requires multifaceted solutions. Preterm birth is a cluster of problems with a set of overlapping factors of influence. Its causes may include individual-level behavioral and psychosocial factors, sociodemographic and neighborhood characteristics, environmental exposure, medical conditions, infertility treatments, and biological factors. Many of these factors co-occur, particularly in those who are socioeconomically disadvantaged or who are members of racial and ethnic minority groups. While advances in perinatal and neonatal care have improved survival for preterm infants, those infants who do survive have a greater risk than infants born at term for developmental disabilities, health problems, and poor growth. The birth of a preterm infant can also bring considerable emotional and economic costs to families and have implications for public-sector services, such as health insurance, educational, and other social support systems. Preterm Birth assesses the problem with respect to both its causes and outcomes. This book addresses the need for research involving clinical, basic, behavioral, and social science disciplines. By defining and addressing the health and economic consequences of premature birth, this book will be of particular interest to health care professionals, public health officials, policy makers, professional associations and clinical, basic, behavioral, and social science researchers.

Book excerpt: This book provides a comprehensive review of our knowledge of vitamin B-6 requirements and metabolism throughout pregnancy and lactation. It concentrates on two primary points: adequacy of available methodologies and a presentation of the extant knowledge on vitamin B-6 metabolism and requirements in pregnancy, infancy, lactation, and in women of childbearing age. Many of the chapters offer new perspectives on how decisions have been and are being made with regard to the establishment of dietary recommendations about vitamin B-6 and the adequacy of research to support these decisions.
