Presenta la metodologia de evaluacion "ex-ante" para el sector salud: definicion del problema; diagnostico de la situacion; plantemiento de alternativas de solucion (empleando diversos modelos de calculo de costos); seleccion de la mejor alternativa (mediante esos mismos modelos); encuadramiento de los proyectos en las prioridades sectoriales (construyendo una matriz de prioridades de intervencion y otro, de indicadores de salud); y, elaboracion del proyecto definitivo. tambien se describen y ejemplifican los pasos a seguir en la evaluacion "ex-post" (de procesos y de resultados e impacto): identificacion con precision de los objetivos del proyecto; traduccion de los mismos e dimensiones operacionales o metas; especificacion de las alternativas que seran evaluadas y medicion de los recursos afectados en valores monetarios (costos por unidad de resultados); valoracion comparativa de las alternativas (matriz de costo-efectividad). Tambieen debe considerarse la necesidad de valorar los costos y la efectividad en un continuum temporal. Finalmente, se mencionan algunas limitaciones del analisis de costo-efectividad (AU).

A wealth of documents dealing with HTA have been published over the last three decades. HTA allegedly is one of the bedrocks of regulation and medical decision making. However, counter vailing visions contend that geographical variations in the role that HTA is actually playing within countries pinpoints specific room for improvement. Given our social preferences, cherry-picking HTA’s features and successes over the last decades moves it away from its possibility frontier. Some of the most noteworthy hindrances that HTA faces, in several countries, to making headway towards its consolidation as an efficient tool for regulation and decision making are as follows: insufficient resources, delays in assessment, inadequate priority setting, regulatory capture, public distrust, actual influence on regulatory decisions, the need for strengthening international cooperation and harmony, the lack of sound and consistent assessments of diagnostic tests, medical devices and surgical innovations and limited dissemination. Time has come for HTA to take a renewed stand. There is a pressing need to submit HTA to in-depth critical scrutiny.
