Available in PDF, EPUB and Kindle. Book excerpt: Written by renowned basic and clinical scientist, Raymond D. Kent, the Handbook on Children’s Speech: Development, Disorders, and Variations provides comprehensive and systematic coverage of the topic of how speech develops and how it can be disordered or otherwise, a departure from typical patterns of a mainstream dialect. The book emphasizes relevant biology and psychology of speech development; contemporary models of spoken language; typical speech development; bilingualism and dialect; motor learning and motor control; clinical assessments of articulation, phonology, voice, prosody, and intelligibility; populations in which speech disorders and differences often occur; and methods and strategies for prevention and treatment. The Handbook covers both speech development and pediatric speech disorders focusing on the ages of birth to puberty. Because speech disorders in children occur against a complex developmental background, the understanding of these disorders requires knowledge about how speech develops and how it is affected in children with disorders or with exposure to languages other than American English. A major theme of the book is that speech development is a constructive, goal-directed phenomenon that weaves together several different processes having their own individual trajectories that generally reach maturity only in puberty or adolescence. For clinicians, researchers, and students, this book will serve as a valuable compendium of the many different tools and methods that have been developed to study speech development in diverse populations and to assess and treat speech disorders and variations.

Available in PDF, EPUB and Kindle. Book excerpt: Pediatric Dysphagia: Etiologies, Diagnosis, and Management is a comprehensive professional reference on the topic of pediatric feeding and swallowing disorders. Given that these disorders derive from abnormalities in the function and/or structure of the airway and digestive systems, multiple clinical specialists may be involved in the evaluation and management of affected children at any given point in time. Therefore, this text includes significant contributions from a wide range of experts in pediatric dysphagia, including all members of the Interdisciplinary Feeding Team at Cincinnati Children’s Medical Center. These experts present an in-depth description of their roles in the diagnosis and management of dysphagic children, providing the reader with an understanding of why a multidisciplinary model of care is key to the optimization of outcomes. Pediatric Dysphagia is divided into five parts. In Part I, readers are provided with an overview of the embryologic development of aerodigestive structures that relate to swallowing, an introduction to neural organization related to swallowing function and physiologic aspects of swallowing, a synopsis of oral motor development, a discussion of the various etiologic categories of feeding and swallowing disorders, and an overview of genetic disorders associated with feeding and swallowing issues. Part II covers the clinical and instrumental assessment of patients, including the interdisciplinary feeding team infrastructure and function, the roles of individual members of the feeding team, the specific diagnostic tests commonly used in the assessment of feeding and swallowing issues, the classification of neonatal intensive care units, and the assessment and management of feeding and swallowing issues encountered in the neonatal intensive care unit. Part III focuses on the management of pediatric dysphagia, covering a wide range of treatment strategies and interventions for children with various categories of feeding disorders. Part IV includes an introduction to the concept of evidence-based practice and the application of evidence-based strategies in the management of dysphagia. Part V presents a brief overview of the role of ethics in healthcare and ethical considerations in the treatment of dysphagic children. In summary, the overall aim of this comprehensive text is to provide all pediatric professionals involved in the care of dysphagic patients with a basic understanding of the complexity of this disorder, the anatomic, neurologic, and physiologic components involved in this disorder, an overview of the diverse population of children who suffer with this disorder, and with a wide range of management approaches based on patient needs and capabilities. The authors also address clinical problem solving and decision making, inspiring readers to develop multidisciplinary models of care at their own institutions.

Available in PDF, EPUB and Kindle. Book excerpt: Although speech-language pathologists are expected to be able to administer and interpret oral examinations, there are currently no screening tests available that provide careful administration instructions and data for intra-examiner and inter-examiner reliability. The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training. The time required to administer the examination typically ranged from 5 to 10 minutes. (Author/GK)

Book excerpt: SSI-4: stuttering Severity instrument - Fourth Edition is a reliable and valid norm-referenced stuttering assessment that can be used for both clinical and search purposes. It measures stuttering severity in both children and adults in the four areas of speech behavior: (1) frequency, (2) duration, (3) physical concomitants, and (4) naturalness of the individual's speech. Frequency is expressed in percent syllables stuttered and converted to scale scores of 2-18. Duration is timed to the nearest one tenth of a second and converted to scale scores of 2-18. The four types of Physical Concomitants are and converted to scale scores of 0-20. The SSI-4 can also be used in conjunction with the stuttering prediction instruments for Young Children (SPI). SSI-4 was normed on a sample of 72 preschool-aged children, 139 school-aged children, and 60 adults. It has four components, each of which is used to assess and monitor the stuttering severity in both children and adults for clinical and research use: (1) Examiner's Manual and Picture Plates, (2) Test Record and Frequency Computation Forms, (3) Subjective stuttering Scales, and (4) Computerized Scoring of stuttering Severity (Software Version 2.0). The Computerized Scoring of the stuttering Severity (Version 2; CSSS-2.0) software is provided to facilitate the calculation of frequency and duration. The CSSS-2.0 automatically produces a record of the percentage of syllables stuttered (frequency) and the duration of the three longest stuttering events. Book excerpt: The Rossetti Infant-Toddler Language Scale is designed to provide the clinician with a comprehensive, easy-to-administer, and relevant tool to assess the preverbal and verbal aspects of communication and interaction in the young child. The Rossetti Infant-Toddler Language Scale is a criterion referenced instrument that assesses Interaction-Attachment, Pragmatics, Gesture, Play, Language Comprehension, and Language Expression. Behaviors can be directly elicited from the child, directly observed, or reported by parent or caregiver to credit the child's performance. All carry equal weight when scoring the scale. Results reflect the child's mastery of skills in each of the areas assessed at three-month intervals across developmental domains tested. The test items guide you in directing and structuring observations in order to monitor the child's progress in language developmental areas. The items are based on a compilation of author observation and descriptions from developmental hierarchies and behaviors recognized and used by leading authorities in the field of infant-toddler assessment. Only items considered discriminating and representative of a skill at an age were included. Severity rating guidelines help you interpret the results and confidently communicate the severity of the child's delay. Parent Questionnaire and report questions are in English and Spanish, found on the CD-ROM for quick printing.
