Book excerpt: Implant-retained removable prosthesis are a predictable and successful treatment that when performed with two or three unsplinted implants in the mandible, have replaced conventional dentures as a better standard for rehabilitation.Although there is available evidence that immediate loading of unsplinted implants in the anterior mandible for overdentures is a procedure described in the literature, case reports depicting its predictability are still scarce.This case series evaluated the implant success rate of immediately loading of two and three unsplinted conical dental implants retaining a mandibular overdenture in cases of moderate and severe bone resorption.All implants remained osseointegrated and showed an overall good response of the peri- implant tissue with no signs of marginal bone change (one clinical case exhibited limited bone loss, not influencing the final outcome), nor patients discomfort.The results of this study suggest a favorable implant survival and success rate regarding mandibular overdentures retained with two or three immediately loaded unsplinted conical threaded implants with a platform switching design and a sandblasted surface .

Although the clinical effectiveness of this intervention has been assessed in many studies, there is limited evidence regarding Patient Reported Outcomes (PROs) with this treatment. Therefore, the purpose of this study is to evaluate patients' ratings of their satisfaction and quality of life after receiving 4 MDI retained mandibular overdentures.Methods: Eleven eligible edentate individuals (F=5, M=6; ages 54-84 years; mean =68.8) who responded to an advertisement for implant retention for dental prostheses in a local Montreal magazine were selected to participate in this quasi-experimental study (pre-post-intervention design). After signing an informed consent, each received 4 MDIs (3M ESPE) using a standardized non-invasive flapless protocol. Implants were loaded immediately after placement using the patients' original dentures. Participants were asked to complete the OHIP-20 questionnaire and rate their satisfaction on a 100 mm Visual Analogue Scale (VAS) instrument, consisting of nine different domains (ease of cleaning, general satisfaction, ability to speak, comfort, aesthetics, retention and stability, ability to chew, function and oral condition), at baseline and 6 months following implant placement.Results: The OHRQoL scores demonstrated a moderate to large degree of improvement in of all the OHIP-20 domains (ES; 0.52-1.03). The largest change was found in the Handicap domain (ES=1.03) and the smallest change was in the Physical Pain domain (ES=0.52). In the view of the above, the aim of the current study was to compare the masticatory efficiency of the single implant supported mandibular overdenture with that of conventional complete denture within the same subject.

The primary complaints among the patients referred to the clinic for treatment related to poor retention of the mandibular denture, instability, denture sores and phonetic problems. However, many patients have problems adapting to their complete dentures, especially to the mandibular prosthesis. The mandibular two-implant overdentures can be successful and are the first choice for prosthodontic treatment of edentulous mandibles in elderly patients. In these cases, adequate patient satisfaction can be related to improved denture retention and also to the stability achieved.Given that immediate loading implants supporting mandibular overdentures have comparable survival rates to those of conventional loading, both clinicians and patients would prefer these immediate loading protocols. However, given the uncomfortable post-surgical period from the patients' point of view, it may be advantageous to consider an early loading protocol.This study was designed as an equivalence trial. The purpose of this clinical trial was to compare the radiographic and clinical outcomes of immediate and early interforaminal loading protocols at 48 h and 2 weeks post-surgery, respectively, with two-splinted narrow-diameter implants for mandibular implant overdentures in thin, non-augmented residual ridge situations. Material and methods: Each edentulous patient included in the study received 2 implants (Tissue Level Standard Plus Implants, u00d8 3.3mm RN, SLActiveuf8e8, Roxoliduf8e8, either 10, 12, or 14 mm in length, Institute Straumann AG, Basel, Switzerland) in the interforaminal region. If primary stability after implant placement was achieved, patients were randomized either to the immediate (48h post-surgery) or the early (2 weeks after surgery) loading group. Prosthetic rehabilitation used SynOctau00ae bar prosthetic system, with a titanium Dolder bar. Implant survival and radiographic changes in crestal bone level were evaluated after three-years.Results: Twenty-four patients, with a mean age of 67u00b19 years, were included. After 3 years, 20 patients were available to follow-up (1 patient passed away before the 1st year appointment, 2 patients withdrew consent after the 1st year follow-up and 1 patient moved abroad after the 2nd year follow-up, all from the immediate loading group). Both the immediate and early loading groups presented 100% survival rate. In the immediate group, 9 patients with 18 implants, mean bone level change from surgery/loading to the third-year evaluation was 0.25u00b11.23mm (p=0.322). In the early loading group, 11 patients (22 implants), from loading to 3-years mean bone level change was 1.02u00b11.57mm (p=0.010). Though after 3 years marginal bone level changes were 0.76mm (95%CI: [-0.09, 1.61]) lower in the immediate loading group, no statistically significant differences could be found between groups (p=0.079). In fact, the estimated mean difference in marginal bone levels (DIB) of the two groups after 3 years of follow-up, obtained from a mixed effects model accounting for the variability of individual responses (implant clustering within patients) was -0.10mm (95%CI: [-0.60, 0.40]), p=0.68.Conclusions: Both protocols, immediate (48h) and early loading (2 weeks) with an implant overdenture over 2 splinted implants placed in the interforaminal region using narrow-diameter tissue level implants Straumannu00ae Standard Plus SLActiveuf8e8, Roxoliduf8ea, in an elderly population showed similar successful clinical results at three-years follow-up.

Book excerpt: This study examined the effect of using two anterior denture-supporting mini implants (mIMP2) compared to four (two anterior and two posterior) denture-supporting mini-implants (mIMP4) on the stress experienced by the mini-implants and the strain experienced by the peri-implant and analogous posterior bone. The stress and strain values were also compared to known values of strength for the mini-implants and physiologic strain values. Finite element modeling was used to study the effects. Two models were created identically, with the exception of the number of denture-supporting mini-implants used, and contained modeled mucosa, cortical bone, cancellous bone, and the mini-implants. The models were loaded over one second with three 70N loads bilaterally and results with collected using a PostView program. The anterior mini-implants in both models showed stresses that would not put them at risk for fracture. The posterior mini-implants in mIMP4 experienced stresses above their yield point, indicating that they could be at risk for fracture. Strain in the peri-implant bone anteriorly and the posterior bone in mIMP2 was in the physiologic range. The posterior peri implant bone in mIMP4 showed strains that were above physiologic, which indicates this region could be at risk for bone resorption. Based on the limitations of this study, posterior mini-implants and posterior peri implant bone could be at risk for fracture and pathologic resorption. Dentists must weigh the risks and benefits of using mini-implants to support overdentures in each clinical case to determine if the treatment could be successful in light of the potential negative consequences.

It shows positive results in terms of increasing prothesis stability and retention and consequently improving the chewing function and patientu2019s quality of life.Purpose: The aim of this study was to describe a series of cases of mandibular overdentures retained by two unsplited implants and evaluate their survival rate.MATERIALS AND METHODS: A retrospective case series of 13 patients rehabilitated with mandibular overdentures retained by two implants, at a Post-graduation Clinic in Lisbon between October 2017 and April 2019, was performed. Demographic data (age and gender), patient motivation and status of the prior dentition were described. The following aspects related to the rehabilitation were also registered: brand, number, diameter and length of the implants, prosthetic restoration (fabrication of a new immediate prothesis or adaptation of the old one), number of appointments after loading, complications, necessity of rebase, frequency of changing rubers, among others.The clinical protocol consisted of 2 implants placed at 32 and 42 positions to retain a mandibular overdenture with the connection of Locator attachments (torqued to 25Ncm). Descriptive statistical analysis was performed. RESULTs: The sample was composed by 13 patients (53,8% women and 46,2% men), with mean age 65.38 years +/- 2,505 (53-79 years). During the period of 31 months, 26 implants (76,9% Bru00e5nemark System MkIIITiUnite, Nobel BiocareTM and 23,1% TSIII SA FIXTURE, OSSTEM) with diameter from 3.3 to 4mm (media of 3,76 +- 0,05) and length from 8.5 to 13mm (media of 11,85 +-0,24) were placed in 32 and 42 positions to retain mandibular overdentures. Till date all implants remained osteointegrated and so the survival rate is 100%. In 9 patients an immediate loading protocol was used (61,5%, with an insertion torque>32Ncm) with the immediate connection of the attachments and overdenture placement. In 2 of them the old prothesis was temporary adapted and in 7 a new immediate prothesis was fabricated. All prosthetic restorations were still functionally in place at the last follow-up appointment.CONCLUSIONS: The results of this case series suggest that favorable implant survival rates can be achieved with mandibular overdentures retained by two implants. An improvement in edentulous patients quality of life can be achieved with this treatment modality.
