Book excerpt: Introduction : le vieillissement de la population est un phénomène qui s'accélère dans la plupart des pays développés. L'allongement de l'espérance de vie reste néanmoins associé à des problématiques spécifiques, comme la perte d'autonomie ou le développement de maladies neurologiques ou psychiatriques. Au global, plus de 20% des adultes de 60 ans et plus souffrent d'un trouble de santé mentale ou neurologique selon l'Organisation mondiale de la santé. La prise en charge des personnes âgées présentant des troubles psychiatriques reste insuffisante et son organisation complexe. Peu d'études rapportent les effets du maintien en institution de ces patients. Méthode : étude observationnelle transversale sur 117 personnes admises en EHPAD entre le 1er mars 2016 et le 1er mars 2019 sous la convention GHU-CASVP. L'objectif primaire est d'estimer le risque de sortie et d'analyser les facteurs associés à une sortie d'EHPAD. La consommation de soins, le profil clinique des vivants, l'avis des patients et soignants est également analysés. Résultats : parmi les 117 admis, 17 sont sorties et 31 décédés. Le risque de sortie à un an est de 7% et de 12.6% à 5ans. En analyse multivarié, les facteurs associés à une augmentation du risque de sortie sont le sexe masculin et "l'antécédent de mesures de contention ou d'isolement". Le nombre annuel de jours d'hospitalisation et les actes ambulatoires diminuent significativement suite à l'admission en EHPAD. Conclusion : le risque de sortie est relativement faible suite à l'admission en EHPAD. Par ailleurs, la majorité des résidents (91%) sont satisfaits en EHPAD alors que les soignants (82%) estiment que l'EHPAD n'est pas un lieu adapté pour ces personnes atteintes de troubles psychiatriques. Cette perception différente entre résidents et soignants est surement dû aux difficultés de prise en charge.
