Book excerpt: Le maintien à domicile est devenu une préoccupation majeure qui anime les débats des autorités sanitaires à cause du vieillissement croissant de la population et des enjeux de la perte d'autonomie. L'objectif de cette thèse était l'évaluation du maintien à domicile des personnes âgées de plus de 75 ans à Vernon à partir de l'analyse du point de vue des intervenants et des structures d'aides Ce travail a été mené dans une logique de triangulation des données à partir de deux autres études similaires à la notre menées l'une auprès des médecins de Vernon et l'autre auprès des patients et de leur entourage. Nous avons procédé, dans une étude qualitative par des entretiens semi-directifs, à l'interview de 20 acteurs du maintien à domicile de différents services d'aides à la personne. Les deux axes de !'enquête étaient les difficultés rencontrées lors de l'exercice quotidien de leurs fonctions et sur les propositions d'optimisation du système de maintien à domicile. La première difficulté rencontrée était le manque Lie formation es aides à domicile. Ce point compromettait l'application des plans d'aides personnalisés alloués à la personne âgée dépendante. La deuxième difficulté était le manque de coordination et de communication entre tous les acteurs et les structures du maintien à domicile du fait de l'hétérogénéité de l'offre des services et l'absence de langage commun et d'outil de transmission adapté. De plus, tout le système de maintien à domicile est fragilisé par un défaut d'information du public mais aussi et malheureusement de tous les professionnels de santé et des services d'aides à la personne. La confrontation de nos données aux enquêtes faites auprès des médecins et des patients de Vernon nous a permis de constater ces mêmes carences. Toutefois, quelques propositions d'optimisation ont été formulées afin d'améliorer les conditions du maintien à domicile des personnes âgées. Parmi ces propositions, nous avons retenu les idées suivantes : développer un guichet unique pour toutes les demandes d'aides à domicile, renforcer les actions de communication autour du maintien à domicile, favoriser les rencontres entre tous les acteurs du maintien à domicile. Une note spéciale a été attribuée au SSIAD de Vernon qui semble donner satisfaction. Il faudrait essayer de s'inspirer de ce modèle pour développer les autres structures. Plusieurs pistes de réflexions ont été évoquées telles que la création de centres d'hébergement temporaire, la diffusion d'un gérontoguide à tous les acteurs du maintien à domicile, etc. Pour finir, la problématique du maintien à domicile sur Vernon se résume dans cette phrase : «pour réussir un maintien à domicile, il faut trouver la bonne adéquation entre vouloir et pouvoir rester chez soi et trouver le bon équilibre entre les besoins de la personne âgée et les moyens investis dans un plan d'aide personnalisé ».

Cette problématique doit intégrer en son sein les déterminants de chacun des acteurs enjeu. L'objet de cette thèse est d'étudier qualitativement le vécu des personnes âgées de plus de 75 ans et de leur entourage sur la commune de Vernon (Eure, 25000 habitants). Elle s'inscrit dans une étude globale, menée par 3 doctorants, sur l'analyse des points de vue des différents acteurs (la personne âgée et son entourage, le médecin généraliste et les intervenants à domicile). Les entretiens ont été réalisés, sur un mode semi directif, auprès de 13 personnes âgées et 6 aidants principaux. L'analyse des données obtenues apporte un éclairage sur la compréhension des besoins des personnes âgées, et les déterminants de la satisfaction des aides à domicile. Enfin, le croisement des résultats avec ceux des deux autres chercheurs engage des réflexions sur les thèmes de la pluralité du rôle du médecin généraliste, de l'évaluation des structures d'aide à domicile, des modes d'information et d'évaluation du système, ou encore de la problématique de la mort au domicile.
