Book excerpt: The development of new therapeutic strategies and the minimization of both direct and indirect costs represent crucial goals in the management of chronic diseases, particularly when these are characterized by a high degree of disability. Chronic respiratory insufficiency (CRI) represents an example of a persistent disease worldwide, for which home management (i.e., daily nursing and treatment) was introduced more than two decades ago according to traditional operating protocols. "Home long-term oxygen treatment" (H-LTOT) was expected to produce significant clinical improvements, together with a substantial drop in CRI social costs (e.g., hospital admissions, number of exacerbations, pharmaceutical costs, and patient's reduced productivity). The present volume describes the evolution in the home management of severe CRI over the last two decades in Italy. It reviews a range of topics including the epidemiological aspects, complicating events, current systems for oxygen delivery with the most convenient interfaces, changing approaches to the patient--caregiver relationship, and the economic burden. Particular attention is paid to the new trends in telemedicine, which is regarded as the future step in respiratory medicine for home-assisted and home-ventilated patients. Data concerning the new role of nursing, the patient's expectation of life, and the patient's, family's, and doctor's perspective are also reported, together with an update on the economic impact of telemedicine and the continuing improvements in the quality of telematic H-OTLT.

Book excerpt: Oxygen is an essential component of oxidative metabolism and is used as a treatment in patients with chronic lung disease. Chronic obstructive lung disease (COPD) is the most common chronic lung disease in the US and is the most common indication for supplemental oxygen. Criteria have been developed to help identify patients with COPD who may benefit from long-term oxygen therapy (LTOT). These criteria are based on the known detrimental pathophysiological effects of chronic hypoxemia. The hemoglobin molecule carries the majority of oxygen in the blood and is exquisitely designed to remain more than 90% saturated with oxygen provided the partial pressure of oxygen (PaO2) in arterial blood is close to or above 55 to 60 mm Hg. Once the PaO2 in arterial blood starts to fall below the 55 to 60 mm Hg threshold there is a steep reduction in the saturation of the hemoglobin molecule. Hemoglobin saturations of less than 90% are thought to result in a significant reduction in tissue oxygen delivery. By increasing the concentration of oxygen in inhaled air, supplemental oxygen improves both the PaO2 of oxygen dissolved in arterial blood and the degree of oxygen saturation of the hemoglobin molecule (SaO2). These effects in turn improve the oxygen delivery to organs and tissues allowing oxidative metabolism to continue in the setting of chronic lung disease. The criteria based on these physiological observations have been developed in an attempt to identify patients who may benefit from supplemental oxygen by preventing deaths due to chronic hypoxemia and heart disease related to cor pulmonale. It is generally accepted that COPD patients with PaO2 d 55 mmHg, or those with PaO2 of 56 to 59 mmHg with evidence of end organ disease (e.g., pulmonary hypertension, cor pulmonale, polycythemia (hematocrit > 55%), arrhythmias, congestive heart failure, or impaired mental status) benefit from supplemental oxygen. The cost to the health care system of providing supplemental oxygen to patients with COPD for up to 24 hours per day for many years is substantial. These costs include the cost of oxygen itself, the oxygen delivery systems to allow the patients to use the oxygen during activities of daily living or at night, and the many private vendors who service the equipment and monitor the patients. CMS has developed a national coverage policy in 1985 based on discussions with experts and clinical evidence. CMS presently requested a technology assessment from AHRQ to summarize the available clinical and scientific evidence on the appropriateness and effective use of LTOT in patients with COPD. This summary will be presented as background for discussion to an expert working group convened by NHLBI to discuss LTOT. In particular, CMS would like the report to address the following issues: 1. Identify and analyze studies of long-term oxygen use that may have substantial impact on public health or health care costs.2. Identify and analyze studies that examine the efficacy or effectiveness of long-term oxygen on specific indications, in particular COPD.3. Identify and analyze studies to determine if long-term oxygen therapy is beneficial, and if there are any adverse effects to long-term oxygen therapy.4. Identify and analyze studies that examine the impact of long-term oxygen therapy on the progression of COPD.5. Address any addition specific assessment questions formulated by the project team in consultation with AHRQ.

Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy. Book excerpt: Long term oxygen therapy (LTOT) has been shown to improve patient survival rates, decrease hospitalizations, and improve health-related quality of life. Management of LTOT can pose a challenge to providers, due to the complex insurance regulations, documentation guidelines, and limited time or resources that can result in patients receiving no therapy, inappropriate oxygen devices, or inadequate prescriptions. The purpose of the project was to evaluate the effects of Oxygen Therapy Clinic (OTC) on quality of life in long term oxygen therapy patients. An OTC provided a standardized program for the active management of LTOT patients. The OTC provided coordination of services among providers that included patient assessment, diagnostic testing, LTOT delivery device selection, and patient education. The sample included 11 patients referred to the OTC that underwent evaluation and diagnostic testing. Qualified patients were further assessed utilizing St. George’s Respiratory Questionnaire (SGQR) upon initial evaluation and then reassessed at follow up. SGQR results were analyzed to obtain pre and post total quality of life score, symptoms score, activity score, and impact scores. Although findings were not statistically significant, further data analysis did reveal a modest improvement in overall quality of life, symptoms, and impact scores. Results activities and mobility may have been further limited by the initiation of LTOT. Generalizability of findings is limited based on sample size, and further studies are necessary to evaluate the overall impact of an OTC on quality of life.
