Book excerpt: Introduction Environ 50% des patients arrêtent un traitement prescrit au long cours dans l'année qui suit son initiation. Cependant, un même patient peut être observant pour certains de ses médicaments et en avoir arrêté d'autres. Objectif Nous avons cherché à évaluer l'observance, médicament par médicament, de patients prenant des traitements au long cours et à la comparer à l'importance de ces mêmes médicaments, évaluée par les médecins traitants. Méthodes Nous avons réalisé une étude multicentrique dans des cabinets de médecine générale et des hôpitaux d'Ile de France. Nous avons inclus des patients prenant des médicaments au long cours. Pour chacun de leur traitement, ils ont évalué leur observance à l'aide de l'échelle en une question de Lu et al. Pour chaque patient inclus, le médecin traitant évaluait l'importance de chacun des médicaments à l'aide d'une échelle numérique. Nous avons comparé l'importance des médicaments, selon les médecins, à l'observance déclarée des patients dans des sous groupes définis par classe médicamenteuse. Résultats Au total, 243 patients et 57 médecins (évaluant 484 médicaments chez 123 patients) ont été inclus en 2014. La majorité des 961 médicaments était à visée cardiovasculaire (29.3%). Les patients déclaraient une « excellente observance » pour 67.1% de leurs médicaments ; et les médecins évaluaient 34.6% de ces médicaments comme « essentiels ». Nous avons trouvé une faible concordance entre importance des médicaments estimée par les médecins et observance des patients (ICC = 0.07 IC 95% [0.01-0.14]). L'analyse en sous groupe a permis de comprendre ce résultat en identifiant 3 profils : 1) les traitements « importants selon le médecin » et en général bien pris par les patients (par exemple les traitements anticoagulants ou les hormones thyroïdiennes); 2) les traitements considérés comme « secondaires » par les médecins mais pour lesquels les patients avaient une excellente observance (par exemple les traitement hypolipémiants) ; et 3) les traitements jugés prioritaires par les patients pour lesquels leur observance était faible. Conclusion : Il existe une faible concordance entre importance des médicaments, évaluée par les médecins, et observance du traitement selon les patients. Ce résultat dépend principalement de la classe médicamenteuse étudiée. Ces résultats suggèrent un défaut de communication entre médecin et patient notamment sur l'identification des médicaments secondaires.
