La possibilité de mettre en libre accès des médicaments de PMF est l’aboutissement d’une longue réflexion engagée par les différents acteurs de la chaine du médicament, à travers des groupes de travail au sein de l’AFSSAPS, mais aussi de l’Ordre des Pharmaciens, des associations d’industriels du médicament (LEEM, AFIPA) et des associations de consommateurs. Le décret encadrant cette mesure marque le début d’un engagement des pouvoirs publics en faveur d’une politique prônant l’automédication comme un nouvel outil de santé publique mais aussi de maîtrise des dépenses de santé.

L'Agence Française de Sécurité Sanitaire des Produits de Santé édite une liste positive, où sont inscrits les médicaments de médication offficinale. Les critères d'éligibilité sont stricts et doivent donner aux patients un maximum d'informations pour pouvoir suivre leur traitement de manière autonome. Les pharmaciens volontaires mettent alors en place leur rayon en accès direct. Trois années après la parution du décret, la moitié des officines sont équipées d'un rayon libre accès et les patients sont entrain de changer progressivement leur comportement face à l'automédication.
