Ces lieux de vie sous-tendent l’accompagnement de fin de vie des résidents. Notre objectif est de faire un état des lieux de la fréquence et des modalités de prise en charge palliative au sein des MAS, de mieux faire connaître la loi Léonetti et la possibilité de recours à des structures d’aides extérieures (HAD, EMSP, Réseaux de soins palliatifs). Méthode : cette étude observationnelle s’est déroulée dans les dix MAS du Calvados par le biais d’entretiens avec les équipes dirigeantes et soignantes à partir de questionnaires. Résultats : l'étude met en évidence la rareté des prises en charge palliatives en MAS. La majorité des décès des résidents ont lieu dans les structures hospitalières. Les soignants souhaitent en majorité accompagner les résidents en fin de vie dans la MAS mais relèvent des difficultés : le manque d’infirmier la nuit, l’intégration du handicap rendant l’évaluation de la douleur difficile, l’importante charge affective... Les soins palliatifs sont fréquemment assimilés à la fin de vie et la loi Léonetti est méconnue des soignants. Discussion : avec le vieillissement des personnes handicapées, les MAS seront plus confrontées à des accompagnements de fin de vie dans les années à venir. Le recours à des structures extérieures et les formations des soignants pourraient favoriser les prises en charge palliatives dans les MAS. Conclusion : l'accompagnement de fin de vie des résidents et la diffusion de la culture palliative en MAS nécessite des aménagements, mais ne sont possibles qu’avec une volonté commune des équipes médicales, paramédicales et dirigeantes.
