Les représentations de la santé interviennent dans le comportement vis à vis des soins, mais les facteurs déterminants des orientations distinctes entre SDF "récents" et de "longue date" sont : Le degré de disqualification sociale, L'état psychologique, Le besoin de santé (dont la santé ressentie), L'offre de soins et l'opinion à l'égard du système de soins. L'opinion à l'égard du système de soins varie en fonction de la profondeur de la disqualification. L'hôpital public répond aux demandes Médico-Sociales de l'ensemble des SDF et les SDF de "longue date" s'y adressent prioritairement. Les médecins généralistes sont les référents prioritaires des SDF "récents", en particulier les médecins des Centres d'Hébergement, compétents dans les maladies induites par l'exclusion et capables de répondre à la demande sociale. Les SDF ont des a priori négatifs à l'égard des cliniques ; ils ne pensent pas y avoir leur place, bien que certains d'entre eux aient été satisfaits de leur séjour en tout point. Les autres structures de soins ou professionnels de la santé sont appréciés par les quelques SDF qui les utilisent. Ils font rarement partie des démarches de soins simplificatrices des SDF de "longue date", qui choisissent aussi le moindre coût. Et ils apprécient les consultations de médecin du monde pour cela.

The welfare state is interpreted as a general phenomenon of modernization, as a product of the increasing differentiation and the growing size of societies on the one hand, and of processes of social and political mobilization on the other. It is an important element of the structural convergence of modern societies -- by its mere weight in all countries -- and at the same time a source of divergence by the variations within its institutional structure.
