La mise en œuvre réussie de tout nouveau rôle au sein d'équipes multidisciplinaires est complexe et prend du temps, il est donc important de comprendre les facteurs qui peuvent entraver ou soutenir la mise en œuvre du rôle d'infirmière de pratique avancée dans les établissements de soins primaires. Objectif : Identifier, évaluer et synthétiser les leviers à la mise en œuvre du rôle des infirmières de pratique avancé dans les établissements de soins primaires. Méthode : Une revue de littérature systématique a été réalisée et rapportée conformément selon PRISMA. Quatre bases de données (Cochrane Library, Web of Science, Pubmed, et Embase) ont été consultées pour identifier les études publiées en anglais et en français entre juillet 2017 et septembre 2022. La sélection des études et l'évaluation méthodologique ont été effectuées par deux examinateurs de manière indépendante. Le codage déductif des freins et des leviers a été entrepris à l'aide de la classification de Yorkshire. Nous avons utilisé un codage inductif pour les freins ou les leviers qui ne pouvaient pas être classés à l'aide de codes prédéfinis. Les désaccords ont été réglés par la discussion. Les données descriptives ont été retranscrites dans différents tableaux, et les principaux résultats pour les freins et les leviers ont été rassemblés dans une synthèse narrative. Résultats : Une recherche systématique a identifié 17061 enregistrements potentiels, 229 résumés ont été examinés et 110 textes intégraux ont été récupérés. Soixante-huit articles répondaient aux critères de sélection. L'implantation des IPA rencontre des leviers variables selon l'avancement de leur implantation. Les facteurs liés à l'équipe étaient le levier les plus fréquemment signalé. Les leviers comprenaient le facteur « encadrement et direction », « procédure et politique » et « formation et expérience ». Conclusion : La prise en compte des freins à l'implantation afin de mettre en place des leviers antagonistes est nécessaire au succès de l'implantation des IPA. L'établissement de relations de collaboration avec d'autres professionnels de la santé et la négociation du rôle sont essentiels au succès de la mise en œuvre du rôle d'infirmière de pratique avancée dans les pays à implantation récente.

En acquérant des compétences relevant du champ médical, de l'infirmier en pratique avancée suivra des patients volontaires et adressés par un médecin. Une évaluation de l'impact de cette innovation de l'organisation des soins français doit être remise au parlement français en 2021. Ce travail de recherche a été mené pour recueillir des données visant à accompagner l'implémentation du dispositif et à constituer un point de départ au suivi du dispositif dans le temps. But de l'étude : explorer les représentations des différents acteurs du dispositif d'infirmier en pratique avancée pour les maladies chroniques stabilisées en soins primaires, en région Provence-Alpes-Côte d'Azur (PACA) : patients, médecins généralistes, infirmiers libéraux et étudiants infirmiers en pratique avancée. Résultats : dans cette étude qualitative prospective par théorisation ancrée, 58 entretiens individuels semi-dirigés ont été menés. Les participants ont exprimé des représentations hétérogènes du rôle de l'infirmier en pratique avancée (IPA). Ils ont fait part d'un flou autour du dispositif et ont exprimé le besoin d'une meilleure communication afin de faciliter son intégration. Le suivi des plans de soins, la prise en charge des patients à domicile, l'éducation thérapeutique et la prévention ont été identifiés comme des besoins auxquels l'infirmier en pratique avancée pourrait participer. Les zones sous-médicalisées ont été suggérées comme propice à son implémentation. La principale crainte a été le changement de répartition des tâches professionnelles avec le risque de concurrence. L'intégration de l'infirmier en pratique avancée (IPA) a été perçue comme plus simple au sein d'établissements de soins car les rôles y sont bien définis avec peu de confusion identitaire entre les acteurs contrairement au milieu libéral. La collaboration entre acteurs de santé a été évoquée comme une condition de réussite du dispositif de l'infirmier en pratique avancée. Une attitude prudente a été relevée et un besoin de rodage a émergé, les premiers infirmiers en pratique avancée étant considérés comme des précurseurs. Conclusion : la mobilisation des acteurs de santé est un critère de bonne réussite d'intégration de l'infirmier en pratique avancée, une politique de sensibilisation semble donc nécessaire. La formation des professionnels de santé dans leur cursus ou en formation continue permettrait de développer des compétences collaboratives nécessaires à l'intégration de l'infirmier en pratique avancé.

Reducing this persistent achievement gap is one of Canada's most pressing educational challenges. Numerous reports commissioned by federal and provincial governments and Aboriginal authorities have offered detailed examinations of the complex social, economic, linguistic, and cultural interrelationships that contextualize the educational environments of Aboriginal students. Many of their families struggle with the legacy of residential schools that ripped families apart and caused immeasurable damage to the social fabric. Schools serving these communities work within a context that may include poverty, learned helplessness, despair, and high levels of abuse, addictions and violence. For some communities, student suicide rates may exceed graduation rates. Yet despite many extraordinary challenges, some schools are producing tangible progress for their Aboriginal students. This report springs from a study of ten such schools in an effort to identify practices that appear to contribute to their success.
