Book excerpt: Les infections du site opératoires sont des infections nosocomiales fréquentes et graves. Le but de cette étude est d'identifier des facteurs de risques propres aux ISO, en dehors des risques chirurgicaux connus. Patients et méthodes : Il s'agit d'une étude rétrospective pour laquelle nous avons analysé 93 dossiers de patients opérés d'un pontage vasculaire périphérique. L'ensemble des paramètres descriptif du patient, de l'intervention ainsi que l'évolution post opératoire du site chirurgical ont été relevés afin de déterminer le nombre d'infections du site opératoire et les facteurs de risque qui s'y rattachaient. Résultats : Pour ces patients, nous avons déterminé 21 % d'infections du site opératoire dont : 80 % d'ISO classe I( n = 16 ), 5 % d'ISO classe II ( n = 1) et 15 % d'ISO classe III ( n = 3) selon la classification du CDC. Trente cinq pour-cent de ces infections sont associées à des bactériémies. Parmis les paramètres périopératoires relevés, l'hypertension artérielle et le score ASA sont statistiquement rattachés aux ISO ( respectivement p = 0.0316, p = 0.0081 ). Le diabète ( p = 0.95 ) et l'obésité ( p= 0.53 ) ne sont pas rattachés aux ISO dans notre série. Discussion : Les grands risques infectieux connus sont impliqués dans la genèse des ISO même si la littérature n'est pas univoque. D'autres risques infectieux tels que :l'hypothermie, la transfusion ou l'hypertension artérielle semblent spécifiquement impliqués dans les ISO. Conclusion : Au regard de notre étude, l'HTA apparaît comme un facteur de risque des ISO ; cette hypothèse doit faire l'objet d'un travail prospectif pour être confirmée.

Les infections du site opératoire (ISO) sont fréquentes (prévalence : 4,5%), graves (létalité : 2,5% à 5%), coûteuses (environ 1 semaine d'hospitalisation supplémentaire, environ 3 000 dollars) et en partie évitables (Senic Project). La législation française, l'évolution jurisprudentielle des responsabilités, l'accréditation et les contraintes budgétaires font de la surveillance épidémiologique des ISO un enjeu de management des risques nosocomiaux. Dans un établissement de santé, l'organisation de la surveillance nécessite une bonne définition préalable des objectifs, des moyens nécessaires et des méthodes envisagées pour les atteindre. L'efficience de la méthode de surveillance choisie dépend en partie du suivi des patients après leur sortie de l'hôpital. La seconde partie de ce travail est une étude descriptive des IS0 diagnostiquées lors de ce suivi. Matériels et méthodes : Nous avons utilisé les données recueillies par INCISO, réseau de surveillance des ISO coordonné par le C-CLIN Paris Nord. Résultats : 15 085 interventions de chirurgie digestive ont été étudiées. La durée moyenne de séjour était de 9,5 jours. 759 ISO ont été diagnostiquées, 35,4% après la sortie de l'hôpital. 57% des ISO à NNIS 0 l'ont été après la sortie. La proportion d'ISO après sortie était inversement proportionnelle à la durée d'hospitalisation et le délai médian de diagnostic était de 12,5 jours. La plupart des facteurs de risque classiques d'ISO étaient associés après analyses unifactorielles, au risque d'ISO après sortie (NNIS index, classe de contamination, score ASA, durée d'intervention, expérience du chirurgien, urgence, chirurgie coelioscopique). Conclusions : Le taux d'incidence des ISO à NNIS 0 est un indicateur de qualité des soins chirurgicaux. Le suivi après sortie des patients opérés est le gage d'une surveillance épidémiologique de qualité procurant des taux d'incidence d'IS0 à NNIS 0 fiable.

L'antibioprophylaxie permet de s'opposer à la prolifération bactérienne et donc de diminuer le risque d'infection. Son administration suit des règles strictes permettant d'encadrer cette pratique essentielle à l'opération. Dans le cadre d'une évaluation des pratiques cliniques auprès des équipes de soins, un audit est réalisé au sein du service de I'UHLIN (Unité d'Hygiène et de Lutte contre les Infections Nosocomiales) de l'hôpital Saint-Antoine, afin d'évaluer le respect des recommandations établies dans les services de chirurgie. Après avoir récolté et analysé les résultats de 96 interventions chirurgicales, des écarts aux protocoles sont décelés dans les différents services de chirurgie. Ces résultats sont comparés aux résultats de l'enquête nationale réalisée sur la même année. 51% (49/96) des prescriptions ont été jugées conformes au référentiel, comparé à 41% pour les résultats nationaux. Les critères d'évaluation de l'ATBP sont majoritairement respectés à l'exception du délai injection-incision mais parfois aussi à l'absence de poursuite de l'antibioprophylaxie en salle de surveillance post-interventionnelle. La conformité du délai à l'hôpital Saint-Antoine est de 66% (40/60) comparé à 41% pour les résultats nationaux. Les recommandations pour l'ATBP en chirurgie sont légèrement mieux respectées à l'hôpital Saint-Antoine, comparé à la moyenne des établissements de santé français, mais restent assez bas. La bonne pratique de l'antibioprophylaxie en chirurgie est un défi du quotidien qui doit être perfectionner afin de lutter contre les infections et de limiter l'apparition de souches bactériennes résistantes.
