Book excerpt: Introduction : L'implantologie est aujourd'hui une solution thérapeutique reconnue et intégrée dans le plan de traitement des réhabilitations prothétiques orales. Afin d'obtenir une ostéointégration des implants, une des conditions nécessaires est la présence d'un volume osseux suffisant permettant d'assurer le maintien de l'implant au niveau fonctionnel et esthétique. Pour ce faire, il existe plusieurs techniques d'augmentation du volume osseux déficient localisé et différents types d'apport osseux. Le « gold standard » en chirurgie orale demeure la greffe osseuse autogène. Le but de cette thèse est d'évaluer et de comparer les différents sites de prélèvement osseux intra-buccaux, en vue d'une greffe osseuse autogène pré-implantaire, à partir d'une revue systématique de littérature. Recherche : La recherche sur PubMed avec les termes Mesh a permis de sélectionner 377 articles. Suite à l'analyse des titres, 115 articles furent retenus. Ensuite, 56 articles furent éligibles après lecture des résumés. Finalement, l'analyse des textes a permis d'inclure 27 articles. Résultats : Il a été observé une augmentation significative du volume osseux avec tous les types de greffons autogènes d'environ 4,5 mm. Une légère résorption était présente après l'autogreffe quel que soit le type de greffon, elle diminuait progressivement avec le temps. La résorption était plus marquée sur les greffes de sinus (~40%) et d'augmentation verticale du volume osseux (~12,73%) comparée à la résorption horizontale (~2,6%). De plus, il a été démontré que tous les types de greffons autogènes subissaient un remodelage osseux grâce aux facteurs de croissances et aux cellules ostéogéniques qu'ils contiennent et présentaient une densité élevée de micro-vaisseaux néoformés. Le temps de cicatrisation de l'autogreffe était d'environ 3 à 5 mois. Plusieurs complications ont été observées quel que soit le type de greffon ; notamment des paresthésies des sites donneurs avec un taux plus élevé pour les prélèvements symphysaires (~32%) comparés aux prélèvements ramiques (~4%). Conclusion : le prélèvement ramique semblerait être une technique sûre et fiable avec un taux moins élevé de complications et un volume suffisant de greffon. Il devrait être considéré comme le meilleur choix pour réaliser une augmentation verticale et horizontale quel que soit le site receveur.
