Provision of sustainable pain relief within health care systems is covered in a separate chapter. Since morphine and other opioid analgesics -- required for the relief of moderate-to-severe pain in children -- are listed under the 1961 UN Single Convention on Narcotic Drugs, the new guidelines include an annex explaining the Convention's requirements for the handling and procurement of opioid analgesics for the relief of pain.

Concerning the use of opioids, readers are reminded that psychological dependence does not occur in cancer patients and that the only correct dose of morphine is the one that relieves the pain. A discussion of the reasons why opioids continue to be underprescribed or difficult to obtain is followed by an explanation of the Single Convention on Narcotic Drugs.

Book excerpt: Introduction : le dépistage organisé du CCU est un moyen simple et efficace pour diminuer l'apparition de ce cancer. Cependant, il reste trop peu utilisé et est réalisé chez seulement 45% des femmes de 60 à 65 ans (contre 65% chez les femmes de 25 à 45 ans). L'objectif de cette étude était donc de recueillir les facteurs influençant la réalisation du dépistage auprès des femmes de 50 à 65 ans. Méthode : une étude qualitative a été réalisée, utilisant une approche inspirée de la théorisation ancrée. Des entretiens individuels et semi-dirigés ont été effectués, en présentiel, chez des femmes de 50 à 65 ans, sans antécédent gynécologique nécessitant un suivi particulier. Au total, 13 participantes ont été reçues en entretien de juin 2023 à février 2024. Résultats : pour les participantes, la notion de responsabilité des professionnels de santé et des femmes était un facteur majeur intervenant dans la réalisation du dépistage du CCU. Ce critère influençait les autres facteurs cités, notamment la transmission du savoir par des démarches de prévention, de rappels et d'explications. Ceci favoriserait les connaissances et permettrait d'instaurer une routine pour la pratique du dépistage du CCU, dès le plus jeune âge. Une autre notion clé était importante pour les participantes, la relation de confiance médecin - patiente. Certaines décrivaient une consultation gynécologique traumatisante, en rapport, parfois, à un manque d'échanges et d'explications de la part des professionnels. Quant à la majorité d'entre elles, elles craignaient cet examen désagréable, touchant leur intimité. Il leur était donc important de se sentir en confiance avec leur médecin ou sage-femme lors du suivi. Discussion et conclusion : contrairement aux précédentes études, aucune des participantes n'évoquaient un changement dans leur habitude de suivi en lien avec l'âge et se montraient même réticentes à son arrêt à 65 ans. Elles proposaient alors d'augmenter les démarches de prévention chez les plus jeunes afin de les initier au suivi le plus tôt possible. Le principal frein évoqué était l'examen gynécologique désagréable. Il serait intéressant d'étudier quelques pratiques diminuant cet inconfort, telle que la position en décubitus latéral ou l'auto-insertion du spéculum chez les femmes de cette tranche d'âge, qui se montrent tout de même septiques vis-à-vis des auto-prélèvements vaginaux.
