In preparing a third edition, we found it necessary to revise the text extensively and partly restructure it. The reasons for this are numerous. Experience of recent years has brought technical refinements in the operative treatment of many types of small fracture. Many of these changes stem from the small-fragment-set training programs conducted in Switzerland since 1980, and also from courses and symposia that have been held in other European countries and the United States. These events were occasions for a fruitful ex change of experience with surgeons who were critical of our methods. As a result of this exchange, we perceived a need both to revise our indications and to give greater attention to alternative techniques. We also felt it necessary to respond to criticisms of the first two editions concerning the catalog-like instrument lists and illustrations, and the attention given to fundamental techniques. Many surgeons who work or would like to work with small implants, especially those practicing abroad, are inexperienced in operations on the larger bones. It is imperative that these colleagues be given a basic introduction to the "biomechanical thinking" of the Swiss Association for the Study of Internal Fixation (ASIF).

Book excerpt: The history of the origin and development of the new Classification of Fractures was described in the preface to the French edition. The history of the acceptance of this new concept dates back to 1986, when the Swiss Association for the Study of the Problems of Internal Fixation (AO) accepted the new Classification of Fractures. In the same year, the Trustees of the AO/ASIF Foundation, at their annual meeting in Montreux, adopted the new AO Classification as the basis for fracture classification to be used in the planned third edition of the AO/ASIF Manual. In August 1987, the French edition of "The Comprehensive Classification of Fractures of Long Bones" made its first appearance, coincident with the Congress of the International Society of Orthopaedic Surgery (SICOn in Munich. This precipitated a great deal of interest in the subject. This interest persisted, so that in February of 1988 the President of SICOT, Sir Dennis Paterson, formed a "Presidential Commission for Documentation and Evaluation" with Maurice E. Muller as Chairman.

Book excerpt: This book celebrates the relaunch of the African Pollen Database, presents state-of-the-art of modern and ancient pollen data from sub-Saharan Africa, and promotes Open Access science. Pollen grains are powerful tools for the study of past vegetation dynamics because they preserve well within sedimentary deposits and have a huge diversity in ornamentation that allows different taxa to be determined. The reconstruction of past vegetation from the examination of ancient pollen records thus can be used to characterize the nature of past landscapes (e.g. abundance of forests vs. grasslands), provide insights into changes in biodiversity, and gain empirical evidence of vegetation response to climatic change and human activity. In this, the 35th Volume of "Palaeoecology of Africa", we bring together new data and extensive synthetic reviews to provide novel insights into the relationships between human evolution, human activity, climate change and vegetation dynamics during the Quaternary, the last 2.6 million years. Current and ongoing climate and land-use change is exerting pressure on modern vegetation formations and threatening the livelihoods and wellbeing of many peoples in Africa. In this book the focus is on the Quaternary because it is during this geological period that the modern vegetation formations developed into their current configurations against a backdrop of high magnitude global climate change (glacial-interglacial cycles), human evolution, and a growing human land-use footprint. In this book the latest information is presented and collated from around the African continent to parameterize past vegetation states, identify the drivers of vegetation change, and assess the vegetation resilience to change. To achieve this research from two broad themes are covered: (i) the present is the key to the past (i.e. studies which improve our understanding of modern environments so that we can better interpret evidence from the past), and (ii) the past is the key to the future (i.e. studies which unlock information on how and why vegetation changed in the past so one can better anticipate trajectories of future change). This Open Access book will provide a strong foundation for future research exploring past ecological, environmental and climatic change within Africa and the surrounding islands. The book is organized regionally (covering western, eastern, central, and southern Africa) and it contains specialized articles focused on particular topics (such as modern pollen-vegetation relationships and fire as a driver of vegetation change), as well as regional and pan-African syntheses drawing together decades of research to assess key scientific questions (including the role of climate in driving vegetation change and the role of vegetation change in human evolution). These articles will be useful to students and teachers from high school to the highest level of university who are interested in the origins and dynamics of vegetation in Africa. Furthermore, it is also meant to provide societally relevant information that can act as an inspiration for the development of sustainable management practices for the future.

Book excerpt: Contexte : Les fractures extra-articulaires de l'extrémité distale du radius à déplacement postérieur sont actuellement traitées par plaques antérieures verrouillées par la plupart des chirurgiens. Ce mode d'ostéosynthèse par voie antérieure néglige la comminution de la corticale postérieure quand elle existe sans se soucier de sa bonne reconstitution. Ce travail avait pour but d'étudier le devenir de la comminution postérieure dans ce type de fracture. Méthode : Il s'agit d'une étude prospective, monocentrique menée de mars 2014 à juin 2015. Tous les patients consultant aux urgences et présentant une fracture extra-articulaire de l'extrémité distale du radius à déplacement postérieur étaient inclus. Un scanner préopératoire était idéalement réalisé pour évaluer au mieux la comminution postérieure. Toutes les fractures étaient ostéosynthésées par une plaque antérieure verrouillée. Les patients étaient revus à 1, 3 et 6 mois pour une évaluation clinique, radiologique et fonctionnelle par les scores de Quick DASH et de Mayo Clinic. Durant le suivi, un bilan biologique comprenant le dosage de la vitamine D et de la calcémie ainsi qu'une ostéodensitométrie osseuse étaient réalisés. A 6 mois post-opératoires, 1 scanner était réalisé pour évaluer le devenir de la comminution postérieure. Résultats : Vingt-neuf patients ont été inclus. Tous présentaient une comminution corticale postérieure. A 6 mois postopératoires, il a été retrouvé chez 79,3 % des patients un défect métaphysaire ovalaire. Le volume moyen de ce défect était de 1,76 ml (+/-1,52). Le contenu de ce défect avait une tonalité proche de celle de la graisse. La corticale postérieure du radius était quant à elle restituée ad integrum. Il n'a pas été mis en évidence de lien statistique entre le volume du défect et les différents paramètres étudiés (l'âge, les taux de calcium et de vitamine D, les données ostéodensitométriques, l'importance du déplacement initial et les caractéristiques de la comminution postérieure). Par contre, la densité du défect était liée négativement à l'IMC et positivement aux résultats fonctionnels. Conclusion : Dans les fractures extra-articulaires du radius distal à déplacement postérieur, persiste après la consolidation radiologique, un défect métaphysaire distal lié à la compression de l'os spongieux lors du traumatisme. La comminution postérieure présente très fréquemment dans ce type de fracture, guérit quant à elle complètement sans lésion résiduelle.
