Kidney is the preferential colonization site where Leptospira accumulates during systemic infection, primarily in the interstitium and in renal tubule epithelial cells. In acute kidney injury (AKI), the main histological findings are acute tubulo-interstitial nephritis and acute tubular necrosis, characterized by hypokalemia and non-oliguria. The injury caused by leptospira outer membrane protein components involves innate immunity Toll-like receptor pathways and activation of complement and immune cascades in the kidney, together with extracellular matrix accumulation when the kidney infection progress to chronic kidney disease (CKD). Significantly, recent studies indicate that leptospira may induce CKD in humans either with AKI-CKD continuum or subclinical leptospira infection leading to CKD. Leptospirosis kidney disease thus represents a newly discovered and previously overlooked area requiring intensive medical and public health attention and investigation. In this book, leading experts in the field contribute their cutting-edge clinical and research experiences for the understanding of leptospirosis kidney disease. The outstanding feature of this publication is emphasizing kidney injury and disease induced by leptospirosis, an underestimated infection involving multiple organs. Its symptoms may mimic those of a number of other unrelated infections such as influenza meningitis hepatitis dengue or viral haemorrhagic fevers. It is important to distinguish leptospirosis from these diseases. For this reason new diagnostic methods have been developed in recent years. In humid tropical and subtropical areas where most developing countries are located leptospirosis poses a greater health problem than in areas with a temperate climate. Because leptospirosis is easily overlooked and consequently underreported in many parts of the world it is necessary to increase awareness and knowledge of the disease as a public health threat. The aim of these guidelines is to assist in this process. The target groups to which these guidelines are directed consist of health workers clinicians laboratory technicians microbiologists public health workers veterinarians and biologists with an interest in zoonoses having no specialized knowledge of leptospirosis but who wish to be generally informed about the microorganism concerned and the disease that it may cause. This is not a handbook and avoids technical details but the interested reader can find further information in the annexes and the general bibliography. These guidelines are concerned essentially with human leptospirosis.

The reasons for this under-recognition are numerous, including the fact that it mainly affects the disadvantaged rural poor in areas that are poorly supplied with the diagnostic capability to make the diagnosis. In 22 separate articles, expert authors from around the world have summarized what is known about the burden of the disease in humans and animals and the presence of the causative bacterium in the environment in their countries or regions. They have also identified the main obstacles and challenges to establishing the true burden, and to ensure that patients receive accurate diagnosis and optimal care for this all too frequently fatal disease. Rather than focusing on the theoretical risk of the use of Burkholderia pseudomallei as a biological weapon, this book highlights its importance as a clear and present danger to global public health.

Book excerpt: Pathogens transmitted among humans, animals, or plants by insects and arthropod vectors have been responsible for significant morbidity and mortality throughout recorded history. Such vector-borne diseases â€" including malaria, dengue, yellow fever, and plague â€" together accounted for more human disease and death in the 17th through early 20th centuries than all other causes combined. Over the past three decades, previously controlled vector-borne diseases have resurged or reemerged in new geographic locations, and several newly identified pathogens and vectors have triggered disease outbreaks in plants and animals, including humans. Domestic and international capabilities to detect, identify, and effectively respond to vector-borne diseases are limited. Few vaccines have been developed against vector-borne pathogens. At the same time, drug resistance has developed in vector-borne pathogens while their vectors are increasingly resistant to insecticide controls. Furthermore, the ranks of scientists trained to conduct research in key fields including medical entomology, vector ecology, and tropical medicine have dwindled, threatening prospects for addressing vector-borne diseases now and in the future. In June 2007, as these circumstances became alarmingly apparent, the Forum on Microbial Threats hosted a workshop to explore the dynamic relationships among host, pathogen(s), vector(s), and ecosystems that characterize vector-borne diseases. Revisiting this topic in September 2014, the Forum organized a workshop to examine trends and patterns in the incidence and prevalence of vector-borne diseases in an increasingly interconnected and ecologically disturbed world, as well as recent developments to meet these dynamic threats. Participants examined the emergence and global movement of vector-borne diseases, research priorities for understanding their biology and ecology, and global preparedness for and progress toward their prevention, control, and mitigation. This report summarizes the presentations and discussions from the workshop.
