Book excerpt: Nous avons réalisé une étude rétrospective afin de déterminer la prévalence de l'insuffisance rénale chronique chez les patients âgés hospitalisés en médecine gériatrique aiguë à Saint-Nazaire. Pour estimer la fonction rénale nous nous sommes basé sur le calcul de la clairance de la créatinine, par la méthode de Cockcroft et Gault. Nous avons demandé aux médecins traitants de ces patients d'évaluer leur fonction rénale selon une méthode de leur choix. Dans la majorité des cas, l'insuffisance rénale est sous-estimée par les médecins généralistes. Nous avons étudié les prescriptions d'antibiotiques en fonction du statut rénal des patients pendant leur hospitalisation. Une estimation de la clairance de la créatinine se révèle indispensable durant un épisode médical aigu notamment lors d'une infection. Il est nécessaire d'adapter la prescription des traitements à élimination rénale et d'ajuster régulièrement les doses.
