La brutalité de l'accident vasculaire cérébral (AVC) rend nécessaire une adaptation rapide des soins et du mode de vie. Nous avons réalisé une étude rétrospective sur 61 patients de 40 à 75 ans, parmi 94 admis pour AVC dans un service de neurologie sur 7 mois, en moyenne 17 mois après leur AVC. Les paramètres pris en compte ont été : les conditions de vie, l'autonomie, les aides techniques et humaines, les attentes des patients et de leurs familles et le rôle des médecins généralistes. Les résultats montrent que la majorité des patients récupèrent une autonomie satisfaisante et peuvent réintégrer leur logement antérieur notamment grâce à l'aide familiale. Sur le plan psychologique, la dépression post-AVC reste fréquente. L'admission en rééducation permet aux patients initialement les plus déficitaires de récupérer une autonomie comparable à celle des autres mais leur qualité de vie reste inférieure. Les attentes des patients à distance de l'AVC concernent essentiellement la maladie et ses conséquences personnelles directes tandis que les familles sont plus préoccupées par l'information et le délai d'obtention des aides. Les médecins généralistes apprécient correctement l'autonomie physique de leurs patients, un peu moins leur autonomie domestique. Leur rôle de prescripteur est prépondérant par rapport à l'information et à la coordination des aides. La création des maisons du handicap devrait permettre de faciliter les démarches du patient et de sa famille.

Cette étude s'est attachée à souligner les difficultés auxquelles les médecins traitants sont confrontés dans ce processus. Il en ressort de grandes carences dans la communication entre le réseau de ville et l'hôpital et au sein même du réseau de ville. Le délai de mise en place des aides et la lourdeur administrative sont également des contraintes importantes. Ni la charge imputée au médecin traitant, ni le temps passé à organiser le retour ne sont cités comme obstacles. Les problèmes de communication sont largement dépeints dans la littérature internationale et demeurent l'axe principal d'amélioration souhaité par les médecins traitants. La facilitation du retour à domicile par des plateformes de communication entre les professionnels de santé devrait accroître le confort, tant des patients et de leurs aidants, que des professionnels de santé et diminuer le risque de ré-hospitalisation.

Le nombre d'AVC tend à augmenter en raison du vieillissement de la polulation. L'objectif de cette étude est de décrire le devenir médico-social et les complications médicales après un AVC ischémique ou hémorragique dans l'année suivant le retour à domicile d'une population de plus de 70 ans. Matériel et méthodes : il s'agit d'une étude descriptive observationnelle incluant des patients de plus de 70 ans rentrant à domicile entre 2014 et 2017. Le devenir des patients et les complications ont été recueillis auprès du médecin traitant. Le critère de jugement principal est le nombre de réhospitalisations à 1 mois, 3 mois, 6 mois et dans les 6 derniers mois. Les critères de jugement secondaires sont le nombre de décès et d'institutionnalisations et la survenue de complications (chutes, épisode dépressif majeur, escarres, récidives d'AVC, pneumopathies infectieuses, trouble vésicosphinctériens, évènements cardiovasculaires, évènements hémorragiques, crises d'épilepsie). Une analyse univariée et multivariée a été appliquée sur les facteurs de devenir et les complications avec la survenue de plus de dix cas afin de mettre en évidence les facteurs risques de leurs survenues en médecine de ville. Résultats : soixante-douze patients sont inclus avec une moyenne d'âge de 83 ans. Le score de Rankin s'améliore dans les 3 mois après le retour à domicile; 50% des patients sont réhospitalisés dans l'année. Les facteurs de risques indépendants de réhospitalisations sont le sexe masculin (p = 0,049), le score NIHSS initial (p = 0,02), l'insuffisance rénale chronique (p = 0,02), la présence d'une aide-ménagère (p = 0,03), les récidives d'AVC (p = 0,01), les escarres (p = 0,01), les chutes (p
