Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.

Les pays de la région ont investi massivement dans le développement des compétences†‰; les dépenses publiques consacrées à l’éducation ont été multipliées par sept au cours des 30 dernières années, et le nombre d’enfants scolarisés est aujourd’hui plus élevé que jamais. Pourtant, les systèmes d’éducation de cette population n’ont pas donné les résultats escomptés, et ces insuffisances entravent considérablement les perspectives économiques. Dans la moitié des pays, moins de deux enfants sur trois terminent l’école primaire, et encore moins atteignent des niveaux supérieurs d’enseignement et obtiennent un diplôme. Les acquis d’apprentissage demeurent faibles, ce qui entraîne des lacunes dans les compétences cognitives de base telles que la lecture, l’écriture et le calcul, aussi bien chez les enfants, les jeunes que les adultes. Le taux d’alphabétisation de la population adulte est inférieur à 50 % dans de nombreux pays†‰; la part des adultes sachant lire, écrire et compter de manière fonctionnelle est encore plus faible. Une réforme du système est nécessaire pour réaliser des progrès significatifs. De multiples agences aux niveaux national et local sont impliquées dans les stratégies de développement des compétences, faisant des compétences «†‰le problème de tous, mais la responsabilité de personne†‰». Les politiques et les réformes doivent permettre de renforcer les capacités des politiques qui ont fait leurs preuves et d’instaurer des mesures incitatives visant à faire converger le comportement de tous les acteurs dans la poursuite des objectifs de développement des compétences au niveau national. Le développement des compétences en Afrique subsaharienne, un exercice d’équilibre. Investir dans les compétences pour la productivité, l’inclusion et l’adaptabilité présente des données qui éclaireront les choix stratégiques des pays en matière d’investissements dans les compétences. At the heart of the book is a set of topics which have long attracted the attention of urbanists and urban historians all over the world: the growth and reform of urban local government, local-centre relationships, public health and pollution, local government finance, the nature of local social élites and of participation in local government. Approaching these topics through the concept of governance not only raises a series of new questions but also extends the scope of enquiry for the historian seeking to understand towns and cities all over the world in a period of rapid change. Questions of governance must be central to a variety of enquiries into the nature of the urban place. There are questions about the setting of agendas, about when a localised or neighbourhood issue becomes a big city or even national political issue, about what makes a ’problem’. Public health and related matters form a central part of the ’issues’ especially for the British; in North America fire and the development of urban real estate have dominated; in India the security of the colonial government had a prominent place. The historical dynamic of these essays follows the change from the chartered governments of the seventeenth and eighteenth centuries towards the representative regimes of the nineteenth and twentieth. However, such historical change is not regarded as inevitable, and the effects of bureaucratic growth, regulatory regimes, the legitimating role of rational and scientific knowledge as well as the innovatory use of ritual and space are all dealt with at length.
