The contributions presented here reflect a broad geographical spectrum as well as a wide range of policy models. From an analysis of these studies, it is apparent that this is a field in which there has been much innovation and which encompasses markedly varying approaches in response to different national conditions.

Pandemic cholera reached Iran for the first of many times in 1821, assisted by Britain's territorial expansion and growing commercial pursuits. The revival of Iran's trade arteries after six decades of intermittent civil war, fractured rule, and isolation allowed the epidemic to spread inland and assume national proportions. In A Modern Contagion, Amir A. Afkhami argues that the disease had a profound influence on the development of modern Iran, steering the country's social, economic, and political currents. Drawing on archival documents from Iranian, European, and American sources, Afkhami provides a comprehensive overview of pandemic cholera in Iran from the early nineteenth century to the First World War. Linking the intensity of Iran's cholera outbreaks to the country's particular sociobiological vulnerabilities, he demonstrates that local, national, and international forces in Iran helped structure the region's susceptibility to the epidemics. He also explains how Iran's cholera outbreaks drove the adoption of new paradigms in medicine, helped transform Iranian views of government, and caused enduring institutional changes during a critical period in the country's modern development. Cholera played an important role in Iran's globalization and diplomacy, influencing everything from military engagements and boundary negotiations to Russia and Britain's imperial rivalry in the Middle East. Book excerpt: Drawing on various disciplinary perspectives, this book re-focuses the debate on what makes a good health system, with a view to clarifying the uses of social science research in thinking about health care issues in Africa. The explosion of the HIV/AIDS pandemic, the persistence of malaria as a major killer, and the resurgence of diseases like tuberculosis which were previously under control, have brought about changes in the health system, with implications for its governance, especially in view of the diminished capacity of the public health facilities to cope with a complex range of expanded needs. Government responsibilities and objectives in the health sector have been redefined, with private sector entities (both for profit and not-for profit) playing an increasingly visible role in health care provisions. The reasons for collaborative patterns vary, but chronic under-funding of publicly financed health services is often an important factor. Processes of decentralisation and health sector reforms have had mixed effects on health care system performance; while private health insurance markets and private clinics are pointers to a growing stratification of the health market, in line with the intensified income and social differentiation that has occurred over the last two decades.These developments call for health sector reforms.
