Les autorités sanitaires et les pouvoirs publics cherchent à améliorer les conditions de soin et de vie des patients atteints de cette pathologie. La Haute Autorité de Santé recommande un dépistage précoce afin de maximiser l'efficacité de la prise en soin des patients. Aux stades précliniques (I et Il) de la maladie, des plaques amyloïdes et des dégénérescences neurofibrillaires apparaissent au sein du cortex entorhinal provoquant des troubles de l'identification olfactive. Ces troubles sont un marqueur particulièrement intéressant pour contribuer au diagnostic. Il existe des tests évaluant les capacités d'identification olfactives administrables par des orthophonistes et des médecins généralistes. Toutefois, cette évaluation est peu pratiquée en structure de soin. Nous avons élaboré un questionnaire permettant de comprendre quelles sont les connaissances des orthophonistes et des médecins généralistes concernant ce marqueur diagnostic, et leur réinvestissement dans leur pratique. Ce questionnaire comporte des questions communes sur la connaissance des données de la recherche sur le sujet puis des questions plus spécifiques à chaque profession concemant leurs pratiques. Les résultats montrent que les données de la recherche concemant les troubles olfactifs dans la maladie d'Alzheimer sont peu connues des professionnels, toutefois ces derniers orientent les patients vers des spécialistes en cas de besoin. Les professionnels disposant de connaissances concemant l'apparition précoce de troubles olfactifs dans la maladie d'Alzheimer sont plus susceptibles de proposer une évaluation des capacités d'identification olfactive s'ils ont le matériel nécessaire à disposition.

Toutes les taches olfactives n'étant pas atteintes, l'olfaction pourrait aussi être considérée comme thérapie complémentaire, voire alternative aux approches médicamenteuses existantes. Il sera démontré que le sens olfactif a son utilité tant dans le diagnostique que dans le traitement de la maladie d'Alzheimer.

Tout d'abord, deux études longitudinales ont évalué l'effet du traitement antidépresseur sur les atteintes olfactives et émotionnelles. Deux autres études ont étudié quelles atteintes olfactives peuvent aider à différencier la dépression d'une maladie d'Alzheimer débutante. Les résultats de ce travail ont confirmé la présence de marqueurs olfactifs d'état (pour les aspects hédonique et émotionnel) et de trait (pour les atteintes en rapport avec des capacités cognitives) dans la dépression. Par ailleurs, nos données ont mis en évidence des profils d'altérations différents dans la dépression et la maladie d'Alzheimer débutante, concernanat la mémoire de reconnaissance et l'identification des odeurs, ce qui pourrait aider à différencier ces deux maladies. D'autres travaux devront confirmer l'implication de ces marqueurs dans l'efficacité de la thérapie, le suivi et le diagnostic précoce des patients.

Les bénéfices de l'étude de l'olfaction ont ainsi été évalués dans le diagnostic précoce, la prise en charge du patient, et la prévention des risques encourus à travers l'information de l'entourage. un test olfactif a été d'abord élaboré à la recherche d'un insctrument de diagnostic précoce de la maladie d'Alzheimer. Soixante personnes ont été testées, trentes atteintes de la maladie d'Alzheimer au stade prodromal ou léger, et trente à titre de témoins. Leurs capacités olfactives ont été comparées et la fiabilité du test mesurée. Nous avons ainsi pu décrire une atteinte précoce de la reconnaissance olfactive dans la maladie et monter l'intérêt de ce test. L'apport d'un atelier olfactif dans la prise en charge orthophonique a été aussi évalué et les moyens mis en place ont été décrits. Il concernait un groupe de huit patients souffrant de la maladie d'Alzheimer au stade modéré à sévère. A l'aide de la grille des actes de communication mise en place par Thierry Rousseau, nous avons montré dans ce cadre les bénéfices d'un support olfactif. Enfin, un fasicule d'information sur la perte d'odorat survenant dans la maladie d'Alzheimer a été conçu et transmis à l'entourage du patient. L'analyse des réponses données par les proches du malade après leur lecture de cette brochure a permis de juger de son utilié.

Dans notre étude, nous avons recherché une corrélation entre ces troubles langagiers et olfactifs, puis nous avons affiné ce lien par critère de catégorie et de sous-catégorie, afin de mieux comprendre l'organisation cérébrale dans les processus sémantiques et olfactifs. Nous avons présenté des épreuves de dénomination lexicale, de fluence catégorielle, et d'identification olfactive, à neuf patients atteints de la maladie d'Alzheimer. Nos résultats ont mis en évidence une corrélation entre le manque du mot et le défaut d'identification olfactive. Plus précisément, nous avons observé une atteinte spécifique de la dénomination dans la catégorie biologique, que nous n'avons pas retrouvé en olfaction. Enfin, nous avons observé une corrélation entre ces deux troubles dans la catégorie biologique, mais pas dans la sous-catégorie les fruits. Ainsi, les troubles olfactifs et langagiers, qui présentaient déjà des corrélats cérébraux, ont révélé par leurs corrélations cliniques des similarités de fonctionnement et de traitement. L'organisation cérébrale sémantique serait établie selon les concepts et leurs réseaux, expliquant pourquoi nous n'avons pas retrouvé de corrélation pour la sous-catégorie de fruits. Notre étude a mis en évidence les liens entre mémoire sémantique et olfactive et interroge sur l'intérêt d'une amorce olfactive dans le cadre d'une prise en charge orthophonique du manque du mot.
