Book excerpt: Les objectifs de cette thèse sont de réaliser une étude épidémiologique des facteurs de risque et protecteurs du suicide, de mettre en place et d'évaluer un programme de promotion des habiletés d'adaptation; le CAST. Etude épidémiologique: des tests de comparaisons de moyennes sur échantillons indépendants révèlent que les sujets à risque suicidaire sont dans un plus grand mal-être que ceux non à risque: ils se sentent plus en colère, en dépression, anxieux, impulsifs, ont une plus faible estime d'eux-mêmes, sont moins engagés socialement, connaissent moins de support social, usent de stratégies de coping davantage centrées sur l'émotion et ont un focus de contrôle plus externe. Que ce soit pour les filles ou les garçons, le désengagement social apparaît comme un facteur clé dans la prédiction du risque suicidaire. Etude appliquée: des analyses qualitatives et quantitatives révèlent un déclin significatif des idéations suicidaires, une baisse de la dépression et du désespoir, une diminution de l'anxiété et de la colère qui sont des problèmes connus comme des facteurs de risque du suicide. Nous observons également une hausse du contrôle personnel, de l'estime de soi, de l'engagement social, des aptitudes à résoudre les conflits et de la capacité à user d'un support social, d'un locus de contrôle interne qui sont des facteurs protecteurs. En conclusion, nous retrouvons les mêmes facteurs de risque et protecteurs du suicide que ceux évoqués dans la littérature avec une prédominance pour le désengagement social. En outre, nous pouvons affirmer que le CAST est prometteur en matière de prévention du suicide et plus généralement, de promotion de la santé mentale des adolescents.

Au Luxembourg, entre 2000 et 2012, il y a eu 25 suicides chez des jeunes en-dessous de 20 ans :1 garçon en-dessous de 10 ans, 2 garçons entre 10 et 14 ans, 18 garçons et 3 filles entre 15 et 19 ans [2]. D'après l'OMS, les tentatives de suicide (TS) sont 10 à 20 fois plus fréquentes que les suicides. Beaucoup de personnes qui essaient de se suicider échouent dans leur tentative. C'est pourquoi, avoir un aperçu des tentatives de suicide peut donner une image plus complète de la problématique suicidaire. Une étude rétrospective sur les jeunes adolescents hospitalisés peut apporter un éclairage supplémentaire sur le problème du mal-être s'exprimant par des TS. Il y a peu de données sur la problématique suicidaire chez les jeunes adolescents en général, et au Luxembourg en particulier. La prévalence des idées suicidaires est recensée au cours des études internationales HBSC (Health Behaviour in School-Aged Children). A partir de ces données de 2006, Mark L. et col. [3] ont comparé les jeunes de 15 ans de trois pays (Lituanie, Estonie et Luxembourg). Cette étude montre que 17,9% des jeunes au Luxembourg (14,6% des garçons et 21,3% des filles) déclarent avoir sérieusement pensé vouloir mettre fin à leurs jours au cours des douze derniers mois. L'objectif de notre étude est d'apporter une meilleure connaissance sur les tentatives de suicide du jeune adolescent au Luxembourg, de décrire le contexte psycho-social des comportements suicidaires qui aboutissent à une hospitalisation, d'évaluer ia qualité de la prise en charge en fonction des recommandations internationales et de discuter les résultats en fonction des études HBSC sur la même période. La prévention du suicide est une priorité pour le Ministère de la Santé pour la période 2014-2019. Un plan national de prévention du suicide pour le Luxembourg (2015-2019) a été élaboré, dont l'approche biopsychosociale promeut le travail en réseau.
