L'amélioration significative et durable de l'hygiène orale est un des piliers de la prévention et des soins bucco-dentaires. De nombreuses stratégies ont été testées pour induire et/ou soutenir des changements de comportement positifs en matière d'hygiène bucco-dentaire avec des résultats variables. L'utilisation des appareils mobiles et autres supports connectés connait une croissance exponentielle en santé (mSanté) et, depuis quelques années, dans le domaine bucco-dentaire mais l'intérêt de cette approche dans ce contexte n'est pas clairement établi. L'essentiel de ce manuscrit est consacré à une revue systématique de la littérature, qui examine l'efficacité de la mSanté pour améliorer l'hygiène orale, en particulier dans un contexte parodontal. Neuf essais contrôlés randomisés ont été sélectionnés parmi 1729 références. Six études sur neuf (67% tous types d'interventions confondues) indiquent un effet positif de la mSanté sur l'amélioration des scores de plaque et d'inflammation gingivale (4 études). En conclusion, Les stratégies de mSanté semblent efficaces pour améliorer à court terme le contrôle de plaque individuel et, dans une moindre mesure, la santé gingivale, lorsqu'elles sont utilisées en complément de l'OHI/OHM conventionnelle, en particulier chez des adolescents suivis en orthodontie. La supériorité d'une stratégie par rapport à une autre n'a pas pu être établie. Ces résultats sont encourageants mais encore insuffisants pour justifier la généralisation de ce type d'intervention en pratique clinique. Faute de données, aucune conclusion ne peut être tirée concernant le bénéfice de la mSanté chez des patients atteints de gingivite ou de parodontite.

Le projet Santé Orale et Autonomie (SOA) est une mesure développée dans le cadre du plan national de Prévention bucco-dentaire 2006-2009 qui visait à rémunérer des chirurgiens-dentistes, pour qu'ils mettent en place, après une formation en ligne, une action de promotion de la santé orale dans un de ces établissements, selon un protocole commun standardisé qui comportait : 1) une conférence de sensibilisation auprès du personnel 2) l'évaluation de l'état de santé bucco-dentaire des résidents 3) des ateliers de démonstration individualisée des techniques d'hygiène impliquant le personnel et les résidents. Ce travail décrit et évalue l'impact du projet SOA. Vingt-six chirurgiens-dentistes ont participé au projet dans sa totalité. A l'issue de leur formation en ligne, une augmentation significative de leur sentiment de compétence pour 16 aptitudes spécifiques a été observée. Une amélioration du contrôle de plaque a été notée pour 33,7% des résidents (n=691). Une amélioration des habitudes d'hygiène bucco-dentaire au sein des établissements a été observée pour 11,5% des résidents (n=814). Cette étude est la première à montrer l'impact relativement faible de ce type d'action et incite leur association à d'autres dispositifs de promotion de la santé orale.

This phenomenal growth is undoubtedly a reflection of industrial and public demands: not only was sulfur recently in overall surplus for the first time in the history of the chemical industry but it has now become a prin cipal environmental hazard in the form of sulfur dioxide, sulfuric acid and hydrogen sulfide. Another reason, discernible in the last fifteen years, has been the desire, on the part of individual chemists and all types of research managers, to move away from the established chemistry of carbon into the less well understood and sometimes virgin chemistries of the other elements which form covalent bonds. As a result of this movement the last decade has seen the development of sulfur chemistry into a well-organized and now much better understood branch of organic chemistry. Enough of the detail has become clear to see mechanistic interrelationships between previously unconnected reactions and with this clarification the whole subject has in tum become systema tized and subdivided. The divalent sulfur chemistry of thiols, monosulfides, disulfides and polysulfides is a large area in itself, much of it devoted to oxidation-reduction and the breakage and formation of sulfur-sulfur bonds, although interesting discoveries are now being made about the reac tivity of certain sulfur-carbon bonds. Of course, this area has its own mas sive biochemical branch involving enzymes and proteins.
