J'ai souhaité, au travers de cette thèse, revenir sur l'expérimentation faite en 2014 sur la dispensation des antibiotiques à l'unité, et discuter ses résultats en termes d'observance, en termes écologiques, économiques et organisationnels. Cette expérimentation a entrainé de vives réactions des acteurs du secteur de la santé. La faisabilité de la généralisation de cette pratique à l'officine, avec ses intérêts et ses limites, est présentée dans ce travail. La mise en place d'une telle pratique nécessitera indéniablement une évolution de la législation encadrant la dispensation des médicaments, mais aussi des préparations de doses à administrer, ainsi que du conditionnement proposé par l'industrie pharmaceutique.

Ses principaux avantages, la réduction du gaspillage et des coûts pour la sécurité sociale et la diminution de l'antibiorésistance sont largement prônés alors que ses inconvénients sont occultés. Ceux-ci ne sont pourtant pas négligeables : tâches supplémentaires à l'officine, perte de traçabilité et, en corollaire, réduction du niveau de sécurité pour le patient. Pour illustrer les aspects positifs et négatifs de ce type de dispensation et en mesurer l'impact, une étude observationnelle et une enquête ont été menées sur le terrain.
